    OBJETIVO: Examinar la relación entre componentes de redes sociales y calidad de vida relacionada con la salud (CVRS) de adultos mayores con o sin síntomas depresivos. MATERIAL Y MÉTODOS: Estudio transversal comparativo con datos de la cohorte "Estudio Integral de Depresión", realizado en la Ciudad de México en 2004. La muestra fue seleccionada por diseño probabilístico multietápico. La CVRS se midió con SF-36, mientras que Geriatric Depression Scale y Short Anxiety Screening Test determinaron síntomas de depresión y ansiedad. El análisis consistió de prueba T y regresiones lineales múltiples. RESULTADOS: Ancianos con síntomas de depresión reportaron puntuaciones más bajas en todas las escalas de CVRS; una red más grande de familiares y amigos se asoció con mejor CVRS en varias escalas. Vivir solo no afectó la CVRS en grupos de estudio y comparación. CONCLUSIONES: Existe una asociación positiva de algunos componentes de las redes sociales con la CVRS, incluso en ancianos con síntomas depresivos.

    OBJECTIVE: To examine the relationship between components of social networks and health-related quality of life (HRQL) in older adults with and without depressive symptoms. MATERIAL AND METHODS: Comparative cross-sectional study with data from the cohort study "Integral Study of Depression", carried out in Mexico City during 2004. The sample was selected through a multi-stage probability design. HRQL was measured with the SF-36. Geriatric Depression Scale (GDS) and the Short Anxiety Screening Test (SAST) determined depressive symptoms and anxiety. T-test and multiple linear regressions were conducted. RESULTS: Older adults with depressive symptoms had the lowest scores in all HRQL scales. A larger network of close relatives and friends was associated with better HRQL on several scales. Living alone did not significantly affect HRQL level, in either the study or comparison group. CONCLUSIONS: A positive association between some components of social networks and good HRQL exists even in older adults with depressive symptoms.

    OBJETIVO: Determinar la prevalencia de hipertensión arterial (HTA) y los factores de riesgo asociados en escolares de 6 a 12 años, con la metodología aceptada. MATERIAL Y MÉTODOS: Se examinaron 329 alumnos de educación primaria para investigar asociaciones entre HTA, índice de masa corporal (IMC), edad y género, usando un estudio transversal. Se midió la presión arterial y se utilizó el percentil 95 para diagnosticar HTA. Se compararon niños con y sin HTA. Se utilizaron las razones de momios simple y ajustada con la X² de Mantel-Haenszel como medidas de asociación. RESULTADOS: Los niños con sobrepeso (IMC > 85 percentil) y del grupo de 11-12 años tuvieron al menos cuatro veces más posibilidades de HTA después de controlar variables confusoras. CONCLUSIONES: Los niños que presentaron HTA ascendieron a 4.9%, con mayor prevalencia entre aquellos con sobrepeso y pertenecientes a un grupo etario mayor. Las asociaciones encontradas pueden ser marcadores de HTA en niños mexicanos.

    OBJECTIVE: To determine the prevalence and associated risk factors of high blood pressure (HBP) in school children 6 to 12 years of age with the accepted methodology. MATERIAL AND METHODS: Elementary school children were examined (n=329). Associations between HBP, body mass index (BMI), age and gender were investigated using a cross-sectional study. Blood pressure was measured and the 95th percentile was used to determine HBP. Comparisons between children with and without HBP were made. The crude and X² Mantel-Haenszel adjusted odds ratios were used as measures of association. RESULTS: Overweight children (BMI >85th percentile) and those in the age group 11-12 years were at least four times as likely to present with HBP after controlling for confounding factors. CONCLUSION: A total of 4.9% of school children had HBP, with a higher prevalence occurring among overweight and the older group. The associations found here can be used as markers for an increased likelihood of HBP in Mexican children.

    OBJETIVO: Evaluar la asociación entre aborto espontáneo (AE) y el consumo dietético de vitaminas B en mujeres mexicanas portadoras de los polimorfismos de la metilentetrahidrofolato reductasa (MTHFR) (677 C>T y 1298 A>C). MATERIAL Y MÉTODOS: Mediante un diseño de casos y controles anidados en una cohorte, se comparó la ingesta dietética materna de vitaminas B y folato, los polimorfismos maternos de la MTHFR y la concentración sérica de homocisteina de 23 casos de AE (< 20 semanas) y 74 controles (mujeres con embarazos > 20 semanas). RESULTADOS: Las portadoras de los genotipos MTHFR 677TT y 1298AC presentaron un incremento significativo en el riesgo de AE (RM 677TT vs. CC/CT=5.0; IC 95%: 1.2, 20.9 RM 1298 AC vs. AA=5.5; IC95%: 1.1, 26.6), respectivamente. CONCLUSIONES: Nuestros resultados apoyan el papel de la mutación de la MTHFR como posible factor de riesgo para el AE, independientemente del consumo de vitaminas B.

    OBJECTIVE: To asses the association between intake of folate and B vitamins and the incidence of spontaneous abortion (SA) according to the maternal methylenetetrahydrofolate reductase (MTHFR) polymorphisms (677 C>T and 1298 A>C). MATERIAL AND METHODS: We conducted a nested case-control study within a perinatal cohort of women recruited in the state of Morelos, Mexico. Twenty-three women with SA were compared to 74 women whose pregnancy survived beyond week 20th. Intake of folate and B vitamins respectively, was estimated using a validated food frequency questionnaire. Maternal MTHFR polymorphisms were determined by PCR-RFLP and serum homocysteine levels by HPLC. RESULTS: Carriers of MTHFR 677TT and 1298AC genotypes respectively showed an increased risk of SA (OR 677TT vs. CC/CT=5.0; 95% CI: 1.2, 20.9 and OR 1298 AC vs. AA=5.5; 95% CI: 1.1, 26.6). CONCLUSIONS: Our results support the role of MTHFR polymorphisms as a risk factor for SA, regardless of dietary intake of B vitamins.

    OBJETIVO: Analizar los resultados del Programa de Medicamentos Antirretrovirales Gratuitos del Programa de VIH/SIDA de la Ciudad de México (PVSCM) en la población afectada del Distrito Federal. MATERIAL Y MÉTODOS: Se sistematizaron y analizaron datos de la Coordinación del PVSCM sobre atención médica, tratamiento antirretroviral (ARV) y resultados de laboratorio especializado en un análisis retrospectivo del periodo 2001-2006, para evaluar su efecto en los pacientes atendidos. RESULTADOS. Se incluyen datos de 5 146 pacientes que recibieron tratamiento ARV. Al final del periodo, 74% de ellos permanecía vigente, 12.1% se perdió en el seguimiento y 13.9% había fallecido. CONCLUSIONES: En el Distrito Federal, durante el periodo evaluado se logró la ampliación de cobertura, eficacia en el tratamiento ARV e incremento de la sobrevida de los pacientes.

    OBJECTIVE: To analyze the results of the Free Antiretroviral Medication Program of the Mexico City HIV/AIDS Program among the affected population in Mexico City. MATERIAL AND METHODS: A retrospective analysis was conducted of medical attention, antiretroviral treatment and specialized laboratory results data from 2001 to 2006 from patients who sought services from the Mexico City HIV/AIDS Program. RESULTS: Data from 5 146 patients who were undergoing ARV are presented. At the end of the period studied, 74% were current, 12.2% were not found for follow-up and 13.9% had died. CONCLUSIONS: During the period evaluated, wider coverage was achieved in Mexico City as well as increased efficiency in ARV treatment and increased patient survival.

    OBJETIVO: Prevenir el contagio de parotiditis al personal y potencialmente a la colonia de primates no humanos (PNH), tras detectarse un caso en el personal técnico de laboratorio en el Centro de Investigación de Enfermedades de la Marina de los EUA (NMRCD). MATERIAL Y MÉTODOS: El personal fue entrevistado y se hizo una prueba de IgG para parotiditis a los contactos potencialmente susceptibles. RESULTADOS: En total, 81 de 106 miembros del personal tuvo contacto cercano con el caso. Sólo 6/81 (7%) tenían vacuna y 33 (41%) reportaron haber tenido parotiditis, y 8 de 45 (18%) de los susceptibles potenciales no tenían inmunidad (IgG > 20.0). Todos los susceptibles expuestos fueron vacunados y no hubo casos secundarios. Se restringió el acceso a la colonia de PNH. CONCLUSIÓN: La investigación inmediata y la respuesta de salud ocupacional fue imperativa para prevenir casos secundarios de parotiditis en el personal y los NHP.

    OBJECTIVE: To prevent transmission among the staff and potentially among the non-human primate (NHP) colony at the U.S. Naval Medical Research Center Detachment in Peru, where an active case of mumps was discovered in a senior laboratory technician in Sep 03, 2007. MATERIAL AND METHODS: Subjects at the research facility were interviewed and potentially susceptible contacts were tested for mumps IgG. RESULTS: In total, 81 out of 106 staff members (76%) had close contact with the case. Only 6/81 (7%) had MMR, 33 (41%) reported having had mumps, and 8 of 45 (18%) of the potentially susceptible individuals did not have immunity (IgG > 20.0). All the susceptible, exposed individuals received MMR vaccine. There were no secondary cases and access to the NHP colony was restricted. DISCUSSION: Immediate and thorough investigation and occupational health response were imperative in preventing secondary cases of mumps among humans and NHP.

    OBJETIVO: Analizar el efecto vacuolizante de cepas de V. cholerae O1 ElTor aisladas de diferente origen geográfico, incluyendo México. MATERIAL Y MÉTODOS: Se realizaron pruebas de hemolisis, vacuolización y citotoxicidad en células Vero, así como PCR, análisis por RFLP y clonación molecular. RESULTADOS: Todas las cepas indujeron el efecto vacuolizante. Las cepas del ribotipo 2, aisladas de las costas del Golfo en Estados Unidos, presentaron títulos altos de vacuolización. El gen hlyA fue amplificado en las nueve cepas mediante PCR, aunque sólo ocho fueron hemolíticas. Se clonó el gen hlyA de una cepa toxigénica (2514-88, ctxAB+) y de una cepa no toxigénica aislada en México (CM 91-3, ctxAB-). El sobrenadante de las clonas recombinantes indujo hemólisis, efecto vacuolizante y citotoxicidad. El RFLP mostró alta similitud del gen hlyA de las cepas estudiadas. CONCLUSIÓN: El efecto vacuolizante es un fenotipo ampliamente distribuido en cepas epidémicas de V. cholerae O1 biotipo ElTor.

    OBJECTIVE: To investigate whether the HlyA-induced vacuolating effect is produced by V. cholerae O1 ElTor strains isolated from different geographic origins, including Mexico. MATERIAL AND METHODS: Supernatant-induced haemolysis, vacuolating activity and cytotoxicity in Vero cells were recorded. PCR, RFLP analysis and molecular cloning were performed. RESULTS: All ElTor strains analyzed induced cellular vacuolation. Ribotype 2 strains isolates from the U.S. gulf coast yielded the highest titer of vacuolating activity. Eight of nine strains were haemolytic, while all strains were PCR positive for the hlyA gene. We cloned the hlyA gene from two ElTor strains, a toxigenic (2514-88, ctxAB+) and a non-toxigenic Mexican strain (CM 91-3, ctxAB-). Supernatant from those recombinant E. coli strains induced haemolysis, cell vacuolation and cytotoxicity. RFLP-PCR analysis revealed similarities in the hlyA gene from all strains tested. CONCLUSION: The HlyA-induced vacuolating effect is a widespread phenotype of epidemic V. cholerae O1 ElTor strains.
  • Eficiencia técnica de la atención al paciente con diabetes en el primer nivel Artículos Originales

    OBJETIVO: Cuantificar en un primer nivel la eficiencia técnica de la atención al paciente con diabetes y distinguir la provisión de servicios y los resultados en salud, además de reconocer las fuentes potenciales de variación. MATERIAL Y MÉTODOS: Se incluyeron insumos, actividades clínicas y resultados en salud de 47 clínicas familiares del IMSS Nuevo León. La medida de la eficiencia se realizó mediante el análisis envolvente de datos y se aplicaron modelos de regresión de Tobit. RESULTADOS: Siete clínicas fueron eficientes en cuanto a la provisión de servicios y nueve en salud; dos coincidieron en ambos aspectos. La localización metropolitana y el total de consultas favorecieron la eficiencia en relación con la provisión de servicios, cualesquiera que fueran los atributos del paciente; y la edad del médico, la eficiencia de los resultados en salud. CONCLUSIONES: El desempeño varió en una misma unidad y entre una y otra; algunas fueron eficientes para suministrar servicios y otras para mejorar la salud. Las fuentes de variación también difirieron. Se recomienda incluir ambos productos en el estudio de la eficiencia en diabetes en el primer nivel.

    OBJECTIVE: To quantify the technical efficiency of diabetes care in family practice settings, characterize the provision of services and health results, and recognize potential sources of variation. MATERIAL AND METHODS: We used data envelopment analysis with inputs and outputs for diabetes care from 47 family units within a social security agency in Nuevo Leon. Tobit regression models were also used. RESULTS: Seven units were technically efficient in providing services and nine in achieving health goals. Only two achieved both outcomes. The metropolitan location and the total number of consultations favored efficiency in the provision of services regardless of patient attributes; and the age of the doctor, the efficiency of health results. CONCLUSIONS: Performance varied within and among family units; some were efficient at providing services while others at accomplishing health goals. Sources of variation also differed. It is necessary to include both outputs in the study of efficiency of diabetes care in family practice settings.

    OBJETIVO: Determinar la relación del polimorfismo TaqI del gen del receptor de la vitamina D (RVD) con la lepra lepromatosa (LL) en individuos originarios de Sinaloa, México. MATERIAL Y MÉTODOS: Se amplificó un fragmento de 740 pb del gen RVD en muestras de ADN de 71 pacientes con LL y 144 controles en el Hospital General de Culiacán durante el periodo 2004-2007. El polimorfismo se identificó mediante la endonucleasa TaqI. RESULTADOS: Se observó un aumento de relevancia estadística del genotipo TT en pacientes con LL en comparación con los controles (p= 0.040; RM= 1.82). CONCLUSIÓN: Se demuestra un nexo entre el genotipo TT y la susceptibilidad a la LL.

    OBJETIVE: To establish the association of the vitamin D receptor gene TaqI polymorphism with lepromatous leprosy (LL) in individuals from Sinaloa, Mexico. MATERIAL AND METHODS: A 740 bp fragment was amplified from the VDR gene in DNA samples of 71 patients with LL and 144 controls in the Hospital General de Culiacán during 2004-2007. Polymorphism was identified through TaqI endonuclease. RESULTS: A significant increase in the genotype TT of the VDR gene was observed in patients when compared to controls (p = 0.040; OR = 1.82). CONCLUSIONS: Our data support the association between the TT genotype and susceptibility to LL in this Mexican population.
