    Puesto que cada año se realizan docenas de estudios transversales que estiman la prevalencia de factores de riesgo de las enfermedades no transmisibles, es posible que exista mucha información de gran utilidad desde el punto de vista de la vigilancia de dichos factores. No obstante, hay fuertes motivos para poner en tela de juicio el rigor metodológico y la fe que se puede depositar en los resultados de muchos de estos estudios. Los posibles beneficios que aportan estos datos se ven limitados por las deficiencias de los estudios, en parte porque no hay información metodológica clara y explícita con los detalles necesarios para evaluar los procedimientos empleados y porque no se aplica una metodología uniforme que permita hacer comparaciones a lo largo del tiempo y entre distintos estudios. Este trabajo tiene por objetivo servir como herramienta didáctica y práctica. En él se describe la construcción de un instrumento para evaluar la utilidad de los estudios de prevalencia efectuados en el contexto de las actividades de vigilancia, utilizando a manera de ejemplo el caso de la hipertensión. El trabajo también examina e ilustra las deficiencias más comunes de los estudios de prevalencia y ofrece pautas metodológicas que servirán para orientar cualquier estudio futuro y estimular investigaciones adicionales en este campo.

    Every year dozens of cross-sectional studies are carried out that estimate the prevalence of risk factors for chronic noncommunicable diseases. Given that, there is potentially a large amount of information that could be extremely useful for risk factor surveillance. However, there are good reasons to question the methodological rigor and the reliability of the results coming from many of these studies. The potential benefits of the data are curtailed by the studies' shortcomings, in part often because there is no clear and explicit methodological information providing the details needed to assess the procedures that were actually used, as well as a failure to apply a uniform methodology that would allow comparisons over time or across studies. This paper is intended to be both a didactic and practical tool. It describes the construction of an instrument for assessing the usefulness of prevalence studies done within the context of surveillance activities, using the example of hypertension. The paper discusses and illustrates the most common pitfalls found with prevalence studies, and it also offers methodological standards that can guide future prevalence studies as well as stimulate future research efforts in this area.

    Objetivo. Hacer llegar a través de los niños mensajes educativos a las familias en Neiva, capital del Departamento del Huila, Colombia. Métodos. Se desarrolló un proyecto educativo para incluir en el currículo de Ciencias Naturales y Educación Ambiental de los escolares de educación básica contenidos sobre el dengue, el vector y su control. Mediante encuestas y escalas tipo Likert se determinaron los conocimientos y las actitudes en los hogares de los escolares, y luego se complementaron con entrevistas abiertas que reflejaron aspectos culturales en el campo de la salud y la forma como esta enfermedad se integra a la vida de las personas. Antes y después de la intervención, se establecieron en los hogares los índices larvarios de infestación con el vector. Resultados. Se determinó la presencia de un patrón cultural en torno a la enfermedad que supone una diferencia entre "rompehuesos" y "dengue". El "rompehuesos" corresponde al dengue clásico según el modelo popular de la enfermedad. Es aquella que puede ser afrontada con los recursos propios de la familia y la comunidad. La medicina institucional, por medio de sus organismos y agentes de salud, así como de los medios de comunicación, ha logrado superponer el otro modelo: el del dengue hemorrágico. Discusión. Se examinan las alternativas de prevención adecuadas para las condiciones en que se presenta el ciclo del vector en Neiva, los modelos de percepciones acerca de la enfermedad presentes en la comunidad y los resultados de las encuestas, que orientaron la formulación y desarrollo del programa educativo.

    Objective. To deliver, through children, educational messages to families in Neiva, the capital city of the department of Huila, in Colombia. Methods. An educational project was developed to include, in schoolchildren's natural sciences and environmental education curriculum, basic education on dengue, its vector, and its control. Through surveys and Likert scales, the knowledge and attitudes in the schoolchildren's homes were determined. This was complemented with open-ended interviews that reflected cultural aspects in the area of health and how dengue disease is a part of people's lives. Before and after the intervention, larval infestation indices were determined for the schoolchildren's homes. Results. A cultural pattern was found for the disease that differentiated between rompehuesos (breakbone fever) and "dengue". Rompehuesos corresponds, in popular terms, to classical dengue, a disease that can be dealt with using family and community resources. Institutional medicine, by means of its health agencies and health workers, as well as the communications media, has managed to superimpose another model: that which is associated with dengue hemorrhagic fever. Discussion. Alternatives are considered for prevention efforts that would be sufficient for the conditions in which the vector's cycle occurs in Neiva. Also presented are the community's disease model and the survey results. Together, these elements can guide the formulation and development of educational efforts.
  • Impacto de la vacunación contra Haemophilus influenzae tipo b en Cuba Artículos

    Objetivo. Determinar el impacto de la vacunación de menores de 2 años en Cuba contra Haemophilus influenzae tipo b (Hib), principal agente causal de la meningitis bacteriana en ese país. Métodos. La disponibilidad de vacunas conjugadas eficaces contra Hib motivó la vacunación nacional en 1999 de niños menores de 2 años, que alcanzó una cobertura de 97%. El impacto se evaluó mediante el Sistema Nacional de Vigilancia de Meningoencefalitis Bacterianas (SNVMEB). Resultados. La eficacia global de la vacunación se estimó en 99% y la incidencia general de la meningoencefalitis por Hib disminuyó de 1,3 a 0,6 por 100 000 habitantes (46,1%), observándose la mayor reducción en niños menores de 5 años (56,1%). En los menores de 1 año se redujo 70,5% y en el resto de los grupos de menores de 5 años disminuyó entre 25,9 y 49,6%. En el grupo diana para la vacunación, la incidencia se redujo 61,1%; entre los niños de este grupo que contrajeron la meningoencefalitis por Hib, solamente 8 (24,2%) estaban vacunados, 7 de ellos con una sola dosis, aplicada 1 mes antes de enfermar. Conclusiones. Se ha demostrado que la vacunación a gran escala de los niños menores de 2 años contra Hib en Cuba a través del SNVMEB ha logrado disminuir notablemente la incidencia de meningoencefalitis por Hib.

    Objective. To assess the effectiveness of mass vaccination in Cuba of children under 2 years of age against Haemophilus influenzae type b (Hib), the most common causative pathogen of bacterial meningitis. Methods. The availability of effective Hib conjugate vaccines led to a nationwide vaccination program in 1999 targeting all children under 2 years of age, with a 97% coverage rate achieved. To assess the program's impact, data from 1998 and 1999 from the National Bacterial Meningitis Reporting System were used. Results. Vaccination efficacy was estimated at 99%. The overall incidence of Hib meningitis declined 46.1%, from 1.3 to 0.6 cases per 100 000 population. The greatest overall reduction, of 56.1%, occurred among children under 5 years of age. Among children under 1 year of age, the reduction was 70.5%, and among the rest of the age groups of children under 5, incidence decreased between 25.9% and 49.6%. In the group targeted for vaccination, incidence decreased 61.1%. Among children in the target group who contracted Hib meningitis, only 8 cases (24.2%) had been vaccinated, most with a single dose applied 1 month before becoming ill. Conclusions. Hib vaccination of all children under 2 years of age in Cuba greatly reduced the incidence of Hib meningitis, as measured by the National Bacterial Meningitis Reporting System.

    Objetivos. Determinar la prevalencia de la resistencia a los fármacos y analizar la presencia de mutaciones de resistencia genotípica en los subtipos B y no B del VIH-1 en Cuba. Métodos. Entre un total de 1 950 personas que se estima que están infectadas por el VIH-1 en Cuba, se estudió una muestra de 103 pacientes, de los cuales 76 habían recibido antirretrovíricos y 27 no. Se determinó la carga de ARN vírico en el plasma y se utilizó la secuenciación automatizada para detectar mutaciones de resistencia a los inhibidores de la transcriptasa inversa (ITI) y de la proteasa (IP). También se procedió a la subtipificación de la región V3 con prueba de movilidad de heteroduplex (HMA). Para confirmar los resultados de esta prueba se secuenció el gen env (C2-V3- C3) en un tercio de las muestras de cada uno de los subtipos detectados por HMA. Resultados. De las 103 muestras, 81 (78,6%) fueron clasificadas como pertenecientes al subtipo B, 19 (18,5%) al A y 3 (2,9%) al C. La prevalencia de mutaciones de resistencia fue del 26,2% para los ITI y 3,9% para los ITI más IP. Para los ITI nucleosídicos fue del 27,6% en los pacientes tratados y del 7,4% en los no tratados; para los ITI no nucleosídisos, las cifras correspondientes fueron del 5,3% y 0%, respectivamente. En los pacientes tratados se detectó una baja frecuencia (2,6%) de resistencia a la zidovudina más lamivudina y abacavir, y no se encontró multirresistencia a los ITI nucleosídicos. Para las combinaciones de IP e ITI, la prevalencia de la resistencia fue del 5,3% en los pacientes tratados y del 0% en los no tratados. Conclusiones. Aunque generalmente se considera que en Cuba predomina el sub-tipo B, en este estudio se detectó una alta proporción de virus del subtipo no B. La baja prevalencia de mutaciones de resistencia a los IP e ITI refleja la introducción más tardía de estos fármacos en el país. Como no se observó multirresistencia a los ITI, el empleo de estos fármacos sigue siendo una opción válida para los pacientes cubanos.

    Objectives. To determine the prevalence of drug resistance and to analyze the subtyping in HIV-1 samples from Cuba. Methods. From an estimated total number of 1 950 HIV-1-infected persons in Cuba, a sample of 103 patients were studied, 76 of whom had received drug treatment for HIV and 27 who had not. The RNA plasma viral load was measured, and automated sequencing was used to assess resistance mutations to reverse transcriptase inhibitors (RTIs) and to protease inhibitors (PIs). Subtyping in the V3 region was performed using heteroduplex mobility assay (HMA). In order to corroborate the HMA results, sequencing of env (C2-V3-C3) was done with one-third of the samples in each of the subtype groups detected by HMA. Results. Out of the 103 samples, 81 of them (78.6%) were classified as subtype B, 19 (18.5%) as subtype A, and 3 (2.9%) as subtype C. The prevalence of resistance mutations was 26.2% to RTIs, none to PIs alone, and 3.9% to both categories of drugs. The prevalence of resistance to nucleoside RTIs (NRTIs) was 27.6% in treated patients and 7.4% in the untreated patients, and for nonnucleoside RTIs (NNRTIs) it was 5.3% and 0%, respectively. Among treated patients a low frequency (2.6%) of dual resistance to zidovudine (ZDV) plus lamivudine (3TC) and abacavir (ABC) was detected, and multidrug resistance to NRTIs was not found. In relation to PIs together with RTIs, the prevalence of resistance was 5.3% for treated patients and 0% for untreated patients. Conclusions. Even though Cuba is generally considered an area where subtype B is dominant, we detected a high proportion of non-B subtype viruses. The low prevalence of resistance mutations to RTIs and PIs reflects the delay in introducing these drugs to Cuba. Multidrug resistance to RTIs was not found, so, as of now, the use of these drugs continues to be an option for Cuban patients.

    In this day and age, public policies that aim to improve equity cannot limit themselves to seeking greater access for all to the job market; the lack of equity is also reflected in unequal access to health services, to education, and to political representation. In order to understand and attempt to correct this unequal access, an approach is needed that takes into account all the sociodemographic factors that shape inequality in the Region of the Americas, most notably sex, ethnic origin, and race. This paper is the product of a request by the Member States of the Pan American Health Organization for PAHO to make known the influence that race, ethnic origin, and sex have on the state of health and on access to health care services. The paper examines how racial discrimination and other forms of intolerance, the low socioeconomic and educational level of certain ethnic and racial groups, and cultural beliefs exert a decisive influence on individuals' search for health care and their possibilities of enjoying good health. This subject is particularly important this year, when the United Nations is holding its World Conference against Racism, Racial Discrimination, Xenophobia and Related Intolerance.
  • Directrices para el uso de antirretrovíricos en adolescentes y adultos infectados por VIH Temas de Actualidad

    This document presents the most recent update of recommendations from the Department of Health and Human Services of the United States of America concerning the antiretroviral treatment of adolescents and adults who are infected with human immunodeficiency virus (HIV). Among the subjects covered in the document are assessment tests for the viral load, the CD4+ T cell count, and resistance to antiretrovirals; when to begin treatment and with which drugs; when to change treatment and the therapeutic options in that situation; aspects of treating adolescents and pregnant women; treatment adherence; and the drugs' principal side effects. Treatment is indicated for all patients with acute HIV infection, in those who have seroconverted in the preceding 6 months, and in symptomatic patients. With asymptomatic patients, the need for treatment depends on several real or potential risks and benefits. Treatment is usually indicated for asymptomatic individuals with CD4+ T cell counts of < 350/mm³ or with plasma HIV RNA > 55 000 copies/mL with the reverse transcriptase polymerase chain reaction test. Once treatment has begun, goals should include a maximum and durable suppression of the viral load, restoring or maintaining immune function, improving the quality of life, and reducing HIV-related morbidity and mortality. Treatment results are assessed mainly in terms of the plasma HIV RNA level, which should be undetectable (< 50 copies/mL) after 4 to 6 months of treatment. Treatment failure after that period of time can be due to poor treatment adherence, drugs being incompletely suppressive, viral resistance, or other poorly known factors. Treatment should be changed if it fails despite good adherence. This change should be based on a complete analysis of the patient's therapeutic history and on the results of resistance tests.
