    Los primeros estudios epidemiológicos ecológicos pusieron de manifiesto el posible papel preventivo de los alimentos que contienen compuestos antioxidantes, sobre la enfermedad cardiovascular y algunos tipos de cáncer. Sin embargo, ulteriores estudios prospectivos, y los primeros ensayos poblacionales de suplementación a dosis terapéuticas han ofrecido resultados contradictorios. Para dilucidar la relación causa-efecto de los nutrientes antioxidantes se planteó el estudio SU.VI.MAX en Francia. Se trata de un estudio poblacional prospectivo de intervención, randomizado y doble ciego, cuyo objetivo primario es evaluar la efectividad de una combinación de vitaminas y minerales antioxidantes a dosis nutricionales, sobre la morbimortalidad por cáncer y cardiopatía isquémica de una cohorte de 12.749 voluntarios de ambos sexos, de 35 a 60 años, distribuidos por toda la geografía francesa, a lo largo de 8 años de seguimiento. Asimismo, se evalúa el impacto del suplemento sobre la incidencia de patología infecciosa, cataratas, el bienestar subjetivo y la utilización de recursos sanitarios, los marcadores biológicos del estatus vitamínico y mineral, la actividad antioxidante, el estado inmunitario y la evolución de los factores de riesgo cardiovascular. Los puntos finales del estudio son: la mortalidad global, la mortalidad específica por cáncer y/o cardiopatía isquémica, la incidencia de cáncer (de cualquier origen) y la incidencia de cardiopatía isquémica. Los instrumentos de valoración empleados son: un cuestionario de salud mensual cumplimentado telemáticamente, un reconocimiento de salud anual, clínico o analítico, en años alternantes, así como la documentación y seguimiento exhaustivo de los acontecimientos adversos e incidencias graves de salud. Para conocer el comportamiento alimentario y su evolución en los individuos de la cohorte se cumplimenta una Encuesta alimentaria bimensual . Se describe la metodología del estudio, cuya originalidad y magnitud ofrece peculiaridades que dan respuesta a algunas dificultades de la epidemiología nutricional, así como las estrategias desplegadas para el mantenimiento de la colaboración activa de la cohorte y su seguimiento exhaustivo. A los dos años del comienzo, la consolidación de la estructura de seguimiento, y la baja tasa de abandonos, permiten esperar que el estudio SU.VI.MAX aporte respuestas de interés en la relación antioxidantes-salud

    The first ecological epidemiological studies revealed the possible preventive role of some foods which contain antioxidant compounds on cardiovascular disease and on some types of cancer. Nevertheless, later further research and the initial population-based supplementary tests at therapeutic doses have rendered contradictory results. To elucidate the cause-effect relationship of the antioxidant nutrients, the SU.VI.MAX study was considered in France. This is a prospective population-based study of randomized, double blind intervention, the prime objective of which is that of evaluating the effectiveness of a combination of antioxidant minerals and vitamins at nutritional dosages on the morbimortality by cancer and ischemic heart disease of a cohort of 12,749 volunteers of both sexes, ranging from 35 to 60 years of age, spread throughout France over an eight-year period. Likewise, an evaluation was also made of the impact of the supplement on the incidence of infectious disease, cataracts, the individual feeling of well-being and the use of health care resources, the biological markers of the vitamin and mineral status, the antioxidant activity, the immune condition and the evolution of the cardiovascular risk factors. The final points of this study are: the overall mortality, the specific mortality by cancer and/or ischemic heart disease, the incidence of cancer (of any type) and the incidence of ischemic heart disease. The assessment tools employed were: a monthly health questionnaire filled out telematically, a yearly clinical or analytical health check-up every other year, as well as the exhaustive monitoring and documentation of the adverse events and serious health problems. To ascertain the eating behavior and the evolution thereof in the individuals comprising the cohort, an eating survey was filled out twice a month. A description is provided of the means and methods employed in this study, the originality and magnitude of which entail unique aspects which serve to remedy some problems of nutritional epidemiology, as well as the strategies employed for maintaining the active collaboration of the cohort and the exhaustive monitoring thereof. Two years into this study, the consolidation of the monitoring organization and the low drop-out rate afford the possibility of anticipating that the SU.VI.MAX study is going to provide some answers of interest with regard to the relationship between antioxidants and good health.
  • Aprendizaje y satisfacción de los talleres de pre y postgrado de medicina para la mejora en la certificación de las causas de defunción, 1992-1996 Originales

    FUNDAMENTO: Ampliar las experiencias de evaluación del aprendizaje y de la satisfacción, en los talleres de pre y postgrado de medicina para la mejora en la certificación de las causa de defunción, con las aportaciones de la Región de Murcia, desde 1992 a 1996. MÉTODOS: Con un diseño epidemiológico cuasi-experimental del tipo pretest-postest, se imparte un taller teórico-práctico de 2 horas, a estudiantes de medicina, MIR en formación de Medicina Familiar y Comunitaria, y médicos generales en curso de obtener dicha especialidad. El taller pretende enseñar la utilidad de las estadísticas de mortalidad y las normas internacionales de certificación de la OMS. Al principio y al final se evalúa la formación mediante un supuesto. Se elaboran indicadores de calidad formal, concepto, y resultado en la cumplimentación. También se administra un cuestionario de satisfacción. RESULTADOS: En cinco años se imparten 23 talleres a 646 asistentes, y se incluyen 472 pares de ejercicios y 586 encuestas de satisfacción. En el pretest, más del 78% de los indicadores individuales son correctos y el 52,3% de los participantes los realizan todos bien. Existen mejoras estadísticamente significativas en los grupos de indicadores de concepto y resultado, pero no en la forma, que parte de una alta calificación (77,5%). El 24,6% de los estudiantes mejora la asignación de la causa de defunción frente al 14,4% de los médicos. Según los participantes, el contenido del curso es muy-adecuado/adecuado (92,7%), sin diferencias entre los tipos de asistentes, y equilibrado en teoría y práctica (64,7%). El 81,3% de los médicos y el 80,2% de los estudiantes afirman que es útil para certificar correctamente una defunción, independientemente de la formación previa. CONCLUSIONES: Las buenas calificaciones iniciales son superadas tras la intervención educativa. De acuerdo con la eficacia demostrada en los talleres para la mejora de la certificación de las causas de defunción, se propone su generalización y el análisis de su impacto en las estadísticas de mortalidad.

    BACKGROUND: To broaden the experiences of evaluation of the learning and satisfaction of the workshops of pre and postgraduates of medicine for the improvement in the certification of the causes of death with contributions from the Region of Murcia, 1992-1996. METHODS: With a quasi-experimental pre-posttest epidemiological design, a workshop is given, being both theoretical and practical of 2 hours addressed to students of medicine, interns in Family and Community Medicine and doctors in the course of obtaining said specialty. The goal of the workshop is to teach the usefulness of Mortality Statistics and the International WHO norms of certification. At start and finish we evaluated the formation by research, using indicators of formal quality, concept and result of the completion. Also, we administrate a questionnaire of satisfaction. RESULTS: In 5 years 23 workshops with 646 assistents were given, including 472 pairs of exercises and 586 satisfaction’s questionnaires. In the pretest, more than 78% of the individual indicators were correct and 52.3% of the participants completed everything well. There were statistically significant improvements in the groups of indicators of concept and result, but not that of form, which showed a high qualification from the beginning (77.5%). The assignation of cause of death has improve more with the students (24.6%) than in physicians (14.4%). The contents of the course are very adequate/adequate (92.7%), without difference among the types of helpers. There is an equilibrium between theory and practice (64.7%). Eighty-one point three of physicians and 80.2% of the students stated that the workshop was useful for correctly certifying a death, independent of the previous background. CONCLUSIONS: The good initial qualifications were improved after the educational intervention. In accordance with workshops demonstrated efficacy, we propose their generalization and to follow its impact on Mortality Statistics.
  • Enfermedades infecciosas y características sociodemográficas de los inmigrantes extranjeros del centro penitenciario de hombres de Barcelona Originales

    FUNDAMENTO: La inmigración de países de bajo desarrollo socio-económico es relativamente reciente en nuestro país. Analizar el impacto de enfermedades como el VIH/sida y la tuberculosis en estas poblaciones, presenta especial interés en materia de salud pública. MÉTODOS: Estudio de casos y controles, realizado en el Centro Penitenciario de Hombres de Barcelona, entre el 1/01 y el 30/09 de 1996, siendo los casos los inmigrantes extranjeros de países de bajo desarrollo socio-económico, y los controles los internos autóctonos apareados por edad (+/- 5 años). Se describen las características epidemiológicas y sociodemográficas de los inmigrantes, y se comparan ambas poblaciones. RESULTADOS: Se incluyeron 450 internos, 300 autóctonos y 150 inmigrantes. El 63% de los inmigrantes eran magrebíes, un 72% se hallaban en situación irregular, el 55% llevaban más de 5 años de residencia en nuestro país y el 53% no disponía de convivencia familiar. Los inmigrantes consumían menos drogas por vía parenteral que los autóctonos (OR: 0,2; IC95% 0,09-0,41), presentaban menos infección VIH (OR:0,2; IC95% 0,05-0,49) y vivían menos en familia (OR: 0,2; IC95% 0,11-0,36). No había diferencias en cuanto a infección por Mycobacterium tuberculosis ni a enfermedad tuberculosa. Los inmigrantes regularizados eran mayores (OR:1,07; IC95% 1,01-1,13), convivían más en familia (OR:2,7; IC95% 1,23-5,80) y consumían más alcohol (OR:1,7; IC 95%1,07-2,59) que los no regularizados. CONCLUSIONES: Los internos autóctonos estaban más afectados por el VIH que los inmigrantes, influyendo el uso parenteral de drogas, más frecuente entre los autóctonos. En los inmigrantes la elevada frecuencia de situaciones no regularizadas y la ausencia de núcleo familiar estable constituyen indicadores de marginación social.

    BACKGROUND: Immigration from socioeconomically underdeveloped countries is relatively recent in our country. Analyzing the impact of diseases such as HIV/AIDS and tuberculosis among these populations is of particular interest with regard to public health. METHODS: Case study and tests conducted at the Male Prison Facility in Barcelona from January 1, 1996 to September 30, 1996, the cases being the foreign immigrants from socioeconomically underdeveloped countries, and the checks of the native prisoners paired by age (+/- 5 years). A description is provided of the epidemiological and sociodemographic characteristics of the immigrants, and a comparison is drawn between the two populations. RESULTS: 450 prisoners, 300 native and 150 immigrants. Sixty-three percent (63%) of the immigrants were from the Maghreb, 72% being in this country illegally, 55% having resided in our country for over 5 years and 53% not living with any family members. The immigrants used less injected drugs that the native prisoners (OR: 0.2; IC95% 0.09-0.41), had less HIV infection (OR: 0.2; IC95% 0.05-0.49) and lived to a lesser extent with family (OR: 0.2; IC95% 1.011-0.36). No differences were found to exist with regard to infection by Mycobacterium tuberculosis nor tubercular disease. The legal immigrants were older (OR: 1.07; IC95% 1.01-1.13) lived to a greater extent with family (OR: 2.7; IC95% 1.23-5.80) and drank more alcohol (OR: 1.7; IC95% 1.07-2.59) than the illegal residents. CONCLUSIONS: The native prisoners had HIV to a greater extent than the immigrants, the use of injected drugs, more common among the natives, bearing an influence on this aspect. As regards the immigrants, the high degree of illegal residence and the absence of a stable core family comprise indicators of a situation of being socially disadvantaged.
  • Consumo de medicamentos en los ancianos: resultados de un estudio poblacional Originales

    FUNDAMENTO: Evaluar la magnitud, naturaleza y cronicidad del consumo de medicamentos en ancianos no institucionalizados, así como la complejidad de su régimen terapéutico. MÉTODO: Estudio transversal a través de encuesta a domicilio de todas las personas de 65 o más años. Municipio de Villanueva de los Castillejos (Huelva). El entrevistador pedía a la persona que mostrara todos los medicamentos que usaba sin olvidar ninguno, anotaba el nombre comercial, dosis y frecuencia con que el medicamento era consumido, y el tiempo aproximado en meses o años en que lo venía utilizando. Se introducen métodos de medida para evaluar la complejidad del régimen terapéutico. RESULTADOS: Se obtuvo información de 362 sujetos (83,8% de la población de hecho). El 83,1% de ellos utilizaba uno o más medicamentos a diario, con una media (desviación estándar) de 2,6 (2,0). Los más usados fueron los antihipertensivos (41,8%), los vasodilatadores periféricos dirigidos a la circulación cerebral (21,3%), las benzodiacepinas (17,4%) y los antinflamatorios no esteroideos (14,1%). El 91,3 y 46,1% de los sujetos con consumo diario tomaba uno o más fármacos desde hacía al menos 1 y 5 años respectivamente. La medicación era consumida como promedio entre una y dos veces al día. Aproximadamente un 20% de la población estudiada tomaba medicamentos sólo cuando aparecían ciertos síntomas, y un 7,3%de los fármacos estaban prescritos de forma regular pero no diaria. CONCLUSIONES: La cantidad y cronicidad del consumo de medicamentos es elevada, aunque comparable con el de otros estudios españoles con similar metodología. El régimen de prescripción parece sencillo para las personas mayores.

    BACKGROUND: To evaluate the magnitude, nature and chronicity of the use of medications on the part of non-institutionalized elderly individuals as well as the complexity of their treatment routine. METHOD: Cross-section study through door-to-door survey of all individuals age 65 or above. Municipality of Villanueva de los Castillejos (Huelva). The surveyor asked the person to show him/her all of the medications he/she was using without overlooking any, recorded the brand name, dosage and frequency with which the medication in question was taken, and the approximate length of time, in months or in years, over which said medication had been taken. Measurement methods were included for the purpose of assessing the complexity of the treatment routine. RESULTS: Information was gathered from 362 subjects (83.8% of the actual population). Of all those surveyed, 83.1% was taking one or more medications daily, for an average (standard deviation) of 2.6 (2.0). Those used most were antihypertensive drugs (41.8%), peripheral vasodilators for circulation to the brain (21.3%), benzodiazepine derivatives (17.4%) and non-steroid antiinflammatory drugs (14.1%). 91.3% and 46.1% of the subjects who were taking medication daily, had been taking one or more drugs for at least 1 and 5 years, respectively. The medication in question was taken on the average of one to two times a day. Approximately 20% of the population surveyed was taking medication solely when certain symptoms arose, and 7.3% of the drugs were prescribed regularly, but not daily. CONCLUSIONS: The quantity and chronicity of the consumption of medications is high, although comparable to that of other Spanish studies conducted employing similar means and methods. The routine prescribed seems simple for seniors.
  • Calidad de vida relacionada con la salud y obesidad en un centro de atención primaria Originales

    FUNDAMENTO. Evaluar la asociación entre la obesidad y dos aspectos de la calidad de vida relacionada con la salud: autopercepción de salud y bienestar psicológico en pacientes adscritos a un centro de salud. MÉTODOS. Estudio transversal descriptivo sobre 167 pacientes entre 20 y 70 años, con índice de masa corporal ³ 30. Se realizó entrevista clínica para la obtención de las variables sociodemográficas y clínicas de cada paciente y la administración de 2 cuestionarios: 1) Perfil de Salud de Nottingham (PSN) y 2) Psychological General Well-Being (PGWB) índex. RESULTADOS. Las puntuaciones medias del PSN indicaron un deterioro global moderado de la salud percibida en las pacientes obesas siendo las áreas afectadas "dolor", "movilidad física" y "reacciones emocionales". Las variables asociadas a una peor puntuación fueron el sexo femenino, la edad superior a 50 años, el bajo nivel de estudios y la presencia de patología psiquiátrica o artrosis. La puntuación total media del PGWB indica un deterioro pequeño del bienestar psicológico. El sexo femenino, la ansiedad y el bajo nivel de estudios se asociaron, asimismo, a peores puntuaciones. CONCLUSIONES. En nuestra población, se identifica un perfil de pacientes obesos con mala calidad de vida (mujeres de edad superior a 50 años con bajo nivel de estudios y presencia de patología psiquiátrica o artrosis) sobre los que se plantea la conveniencia de realizar una actuación prioritaria.

    BACKGROUND. To evaluate the association between obesity and two aspects of health-related quality of life: self-perception of health status and psychological well-being in patients assigned to a primary care center. METHODS. 167 patients between 20 and 70 years old with a body mass índex ³ were included in the study. A clinical interview was carried out to get sociodemographic and clinical data and to administer two questionnaires: 1) Nottingham Health Profile (NHP) and 2) Psychological General Well-being (PGWB) índex. RESULTS. The mean scores of the NHP indicated a moderate overall impairment of perceived health in females. Pain, emotional reactions and physical mobility were the affected dimensions. Females, age group over 50, low level of education and the presence of psychiatric pathology or osteoarthritis were significantly associated with worse scores. The mean scores of PGWB indicates a small impairment of psychological well-being. In this case, worse scores were associated with females, anxiety and low level of education. CONCLUSIONS. In the study population, we identify a profile of obese patients with worse quality of life: women with age over 50, low level of education and the presence of psychiatric pathology or osteoarthritis. We think it would be a good idea to achieve a prior performance towards them.
  • Prevalencia de actividad física y su relación con variables sociodemográficas y ciertos estilos de vida en escolares cordobeses Originales

    FUNDAMENTO: El ejercicio físico regular es una práctica poco extendida entre la población. La infancia es una etapa crucial para adquirir este hábito. El objetivo del estudio fue conocer la conducta de los escolares respecto al grado de actividad física que desarrollan y examinar los factores que se relacionan. MÉTODOS: Se llevó a cabo un estudio observacional mediante encuesta basada en un cuestionario autoadministrado a los escolares de 6º y 8º de Enseñanza General Básica de una Zona Básica de Salud de Córdoba (N=585). Se interrogó sobre la frecuencia de ejercicio, preferencias y expectativas para la práctica deportiva, estado de forma física y clases de educación física del colegio, además de otras conductas constitutivas de los estilos de vida. Se llevó a cabo un análisis bivariado (p<0,05) y multivariado con aplicación de regresión logística. RESULTADOS: Contestaron la encuesta un total de 548 alumnos (tasa de respuesta=97,8%). El 79,3% (IC 95%: 75,7-82,6) realizaban actividades físicas extraescolares, y el 21,0% (IC 95%: 17,7-24,7) ejercicio físico intenso. El 50,5% (IC 95%: 46,3-54,8) no consideran bueno su estado de forma física. La edad, el sexo, el tipo de residencia, la práctica deportiva en los padres o el amigo y estado de salud percibido constituyen las variables más fuertemente asociadas a la realización de ejercicio (p<0,001). CONCLUSIONES: Aunque la prevalencia de ejercicio físico es elevada, va declinando con la edad, sobre todo en las niñas. Un importante número de escolares consideran insatisfactorio su estado de forma física.

    BACKGROUND: Regular physical exercise is not a widespread habit among the population. Early childhood in a crucial stage for acquiring this habit. The objective of this study was that of ascertaining the behavior of school-age children with regard to the degree of physical activity they do and to examine the factors involved. METHODS: An observational study was conducted by means of a survey based on a questionnaire which 6th and 8th grade students in a Basic Health Care District of Cordoba (N-585) filled out on their own. Questions were asked regarding the frequency of exercise, preferences and prospects for practicing sports, the childrens physical shape and school physical education classes, in addition to other behavior comprising their lifestyles. A dual-variable (p<0.05) and a multiple-variable analysis was conducted employing logistic regression. RESULTS: A total of 548 students filled out the survey (response rate=97.8%). 79.3% (IC95% 75.7-82.6) were involved in extracurricular physical activities, 21.0% (IC95% 17.1-24.7%) being involved in intense physical exercise. 50.5% (IC95% 46.3-54.8) did not think that they were in good physical shape. The age, sex, type of residence, the involvement in sports on the part of their parents or a close friend and what they considered their degree of healthiness to be comprised the variables quite strongly linked to doing exercise (p<0.001). CONCLUSIONS: The prevalence of physical exercise is high, although it progressively decreased with age, especially for girls. A major number of students considered themselves to be in unsatisfactory physical condition.
  • Anticuerpos frente a virus West nile y otros virus transmitidos por artropodos en la poblacion del Delta del Ebro Originales

    FUNDAMENTOS: El virus West Nile (VWN) es un Flavivirus que se transmite al hombre a través de distintas especies de mosquitos y produce brotes y casos esporádicos de enfermedad en distintas regiones del Viejo Mundo, incluída la Cuenca Mediterránea. Las zonas húmedas europeas que acogen aves migratorias procedentes de África constituyen áreas de alto riesgo para esta infección, así como para otras infecciones víricas transmitidas por artrópodos. MÉTODOS: Con objeto de investigar la prevalencia de la infección por el VWN y otros virus de transmisión similar en la población humana del Delta del Ebro, se estudiaron 1037 muestras de suero, obtenidas en 10 localidades de la zona, para presencia de anticuerpos frente a VWN y otros 12 virus transmitidos por artrópodos (3 Alfavirus, 8 Flaviviridae y 1 Bunyaviridae) mediante titulación por inhibición de la hemaglutinación (IHA). En algunos casos se estudió la presencia de IgM específica por IHA tras fraccionar el suero por centrifugación en gradientes de sacarosa. RESULTADOS: En total, se encontró reactividad significativa frente a alguno de los virus probados en 130 casos (12.5%; 4.1% frente a Alfavirus, 8.0% frente a Flaviviridae y 0.4% frente a Bunyaviridae). El análisis de los títulos de anticuerpos reveló porcentajes significativos de muestras con títulos elevados frente a antígenos de VWN y otros. La distribución de la seroprevalencia fue muy desigual, concentrándose fundamentalmente en 3 localidades del interior del Delta (Ampolla, San Jaime y Montells), donde la prevalencia de anticuerpos frente a Flaviviridae llegó a alcanzar el 30% y se observaron niveles residuales de IgM frente a VWN en algunos sueros. CONCLUSIONES: Estos resultados y los obtenidos previamente en otras regiones de la Península Ibérica sugieren que el VWN circula en la población humana de las zonas de riesgo y produce brotes epidémicos periódicos. Habida cuenta del alto porcentaje de complicaciones neurológicas observado en los brotes más recientes de infección por VWN registrados en la cuenca Mediterránea (Bucarest, Argelia), cabe pensar que el VWN juegue algún papel en la etiología de las meningitis y encefalitis víricas que sucedan en la población de las áreas de riesgo del territorio español.

    BACKGROUND: The West Nile Virus (WNV) is a Flavivirus which is transmitted to Man by means of different species of mosquitoes and causes outbreaks and sporadic cases of illness in different regions of the Old World, including the Mediterranean Basin. Europes wetlands which comprise a stopping-off point for birds migrating from Africa are high-risk areas as regards this infection, as well as other arthropod-borne virus infections. METHODS: For the purpose of researching the prevalence of the WNV infection and other viruses transmitted similarly among the human population of the Ebro Delta, 1037 samples of serum taken in 10 towns in this area were analyzed to detect the presence of WNV antibodies and antibodies of another 12 arthropod-borne viruses (3 Alphaviruses, 8 Flaviviridae and 1 Bunyaviridae) by means of titration by inhibition of hemagglutination (HAI). In some cases, the presence of HAI-specific IgM was analyzed after breaking down the serum by centrifuging into sucrose curves. RESULTS: In all, a significant degree of reaction was found to some of the viruses testes in 130 cases (12,5%; 4,1% to Alphavirus; 8% to Flaviviridae and 0,4% to Bunyaviridae). The analysis of the antibody titers revealed significant percentages of samples showing large-scale titers of WNV and other types of antigens. The spread for the serum prevalence was highly unaven, being focused mainly in 3 localities located in land on the Delta (Ampolla, San Jaime and Montells), where the prevalence of Flaviviridae antibodies totaled as high as 30% residual levels of WNV-related IgM having been found in some serum samples. CONCLUSIONS: These results and those obtained previously in other parts of the Iberian Peninsula suggest that the WNV is moving throughout the human population in the areas where this risk is found to exist and periodically gives rise to epidemic outbreaks. Bearing in mind the high percentage of neurological complications found to exist in the most recent outbreaks of WNV infections recorded in the Mediterranean Basis (Bucharest, Algeria), it can be thought that the WNV plays some role in the factors contributing to viral meningitis and encephalitis which occur within the population of the areas at risk within Spain.
  • El modelado familiar y el papel educativo de los padres en la etiología del consumo de alcohol en los adolescentes Originales

    FUNDAMENTO: Se pretende determinar las relaciones existentes entre el consumo de bebidas alcohólicas por parte de los adolescentes y dos variables psicosociales: el consumo alcohólico de los otros miembros del sistema familiar y las estrategias educativas paternas. MÉTODOS: Se seleccionó una muestra de 1.100 adolescentes de ambos sexos, de edades comprendidas entre los 15 y los 19 años en la ciudad de Valencia. Se evaluaron a través de los instrumentos pertinentes tres variables: a) estrategias paternas de socialización familiar; b) percepción del adolescente respecto al consumo de siete tipos de bebidas alcohólicas en los miembros de su familia; y c) consumo de siete tipos diferentes de bebidas alcohólicas por parte de los adolescentes en los fines de semana. RESULTADOS: Una vez aplicado el análisis de varianza, se detectó que el consumo de alcohol en los adolescentes está significativamente relacionado con el consumo de los otros miembros de su familia, y con la utilización por parte de los padres de estrategias educativas basadas principalmente en la reprobación y en la ausencia de comprensión y afecto. CONCLUSIONES: Se infiere la necesidad de implicar a los padres en los procesos preventivos a fin de crear un ambiente familiar positivo y además ofrecer a los hijos un modelado racional y controlado en el uso familiar de bebidas alcohólicas. La prevención debería incluir a los padres como un agente central de la intervención.

    BACKGROUND: This analysis is aimed at determining the relationships existing between alcoholic beverage consumption on the part of adolescents and two psychosocial variables: the consumption of alcohol on the part of other members of the family system and the paternal educational strategies. METHODS: A sample of 1100 adolescents of both sexes was taken, ranging 15 to 19 years of age in the city of Valencia. Three variables were evaluated using the pertinent tools: a) paternal family socializing strategies; b) the adolescent’s feeling about the consumption of seven types of alcoholic beverages by the members of his/her family; and c) consumption of seven different types of alcoholic beverages on the part of the adolescents on weekends. RESULTS: Following the application of the variance analysis, it was found that the consumption of alcohol among adolescents is significantly related to the consumption thereof on the part of other members of his/her family, and to the use on the part of the parents of educational strategies based mainly on disapproval and on the lack of understanding and affection. CONCLUSIONS: The need is inferred of involving parents in the preventive processes for the purpose of creating a positive family environment and also of offering children a rational, controlled model with regard to the family use of alcoholic beverages. Prevention should include the parents as a central active figure of involvement.
  • Incidencia de infección por hepatitis C en donantes de cabezas femorales para el banco de tejidos Carta al Director

    Los donantes vivos de cabezas femorales constituyen un grupo de edad y características patológicas muy concreto, y forman el núcleo más importante que nutre de hueso esponjoso a muchos Bancos de Tejidos, para dar cobertura a las necesidades de diversas especialidades quirúrgicas reparadoras y correctoras. Dentro de los programas de control de calidad, se analizó la prevalencia de infección por virus de hepatitis C entre esta población, tomando como grupo de control poblacional a donantes de sangre habituales de la misma localización geográfica, y cuyos datos constaban en los registros del Centro de Transfusión de la Comunidad Valenciana, en donde está ubicado el propio Banco de Tejidos. Aunque la comparación de resultados puede dar origen de interpretaciones erróneas debido al desajuste cualitativo de los grupos, se encontró una incidencia mucho más elevada de infección por VHC entre los donantes de hueso esponjoso que entre los donantes de sangre (3,816% vs. 0,569%), aunque con una proporción menor de donantes con alteraciones enzimáticas hepáticas (por medición de transaminasas). A pesar de que la incidencia hallada de VHC fue casi 6 veces la de la población general, la decisión de prescindir de los donantes vivos de hueso esponjoso crearía serios problemas de abastecimiento a los Bancos de Tejidos. Por ello se propone extremar la aplicación de los criterios de exclusión en la selección de donantes de hueso, y la implantación de técnicas de esterilización por procedimientos físico-químicos (liofilización, deshidratación, tratamiento químico, irradiación) para procesar estos tejidos, al tiempo que emplear los donantes multiorgánicos de tejidos, de edad más joven, como fuente de hueso esponjoso destinado a criopreservación sin tratamiento de esterilización adicional.

    The live donors of femoral heads belong to a highly specific group in terms of age and pathological characteristic, forming the core of the group who supply many tissue banks with spongy bone, to cover the needs of several specialities in the field of repair and corrective surgery. Analysis for the presence of the hepatitis C virus in this population was conducted as a part of quality control programs, while habitual blood donors from the same geographical location were used as the control group. Although the comparison of results could give rise to erroneous interpretations, due to the lack of qualitative fit between the groups, a much higher incidence of infection by VHC was found in the donors of spongy bone than was the case for blood donors (3.816% vs. 0.569%), although a smaller proportion of donors had hepatic alterations (measured by transaminases). In spite of the fact that the prevalence of VHC is almost 6 times that corresponding to the population as a whole, the elimination of live donors of spongy bone would create serious problems with supply to tissue banks. We therefore propose that more severe exclusion criteria be applied to the selection of bone donors, and also that sterilisation techniques be employed, using physical - chemical procedures (liophilisation, dehydration, chemical treatment, irradiation) to process these tissues. We also recommend that younger multiple organ tissue donors be used as sources of spongy bone for cold storage that is not to be subjected to any additional sterilisation treatment.
