  • Las mujeres y el tabaco en la Unión Europea Colaboraciones Especiales

    A lo largo de las últimas décadas, la prevalencia del tabaquismo ha seguido en la Unión Europea tendencias diferentes según el sexo. Mientras que entre los varones la prevalencia ha disminuido notablemente, entre las mujeres se ha incrementado o se ha estancado. En algunos países en que estos cambios se iniciaron más precozmente, la prevalencia del tabaquismo es hoy mayor entre las mujeres que entre los varones. Este documento cuantifica estas tendencias, con referencia a la situación española. En 1995, fumaban 60 millones de hombres y 42 millones de mujeres en la Unión Europea. El 48% de las fumadoras consumían cigarrillos light, lo que supone veinte millones de mujeres que los utilizan. Hay documentos de la industria del tabaco que indican que la promoción de cigarrillos light tiene como intención tranquilizar a sus clientes y frenar a quienes se preocupan por el mantenimiento de su salud y se plantean dejar de fumar. Hay datos que confirman que esta política consigue sus objetivos y que las concepciones erróneas de los fumadores sobre los cigarrillos light puede hacerles persistir en el consumo. Existen datos que indican que el tabaco puede jugar un papel en el control del peso. Las mujeres están más preocupadas por su peso que los hombres, y para las mujeres jóvenes la imagen corporal es muy importante. Éstas tienen a menudo la percepción de tener exceso de peso. Aunque es más común el miedo al sobrepeso que el exceso de peso real, la evidencia sugiere que el temor a ganar peso puede mantener a las mujeres en el hábito de fumar, y que las consideraciones sobre cómo mantener el control del peso influyen en las mujeres jóvenes al comenzar a fumar. El consejo sobre cómo controlar el peso debería formar parte de los programas de tratamiento del tabaquismo dirigidos a las mujeres. Además, cualquier intervención preventiva sobre mujeres adolescentes fumadoras debería abordar la cuestión del aumento de peso.

    Over the last decades, the prevalence of smoking in the European Union has followed different trends by sex. While the prevalence has declined for men, it has increased or is stable for women. In some countries where these changes took place earlier, the prevalence of smoking is now higher among women than among men. This document provides quantitative data for these trends in the European Union, with special reference to the situation in Spain. In 1995 there were 60 million men and 42 million women smokers in the European Union. Up to 48% of female smokers used light cigarettes, that is 20 million women. There are documents from the tobacco industry showing that the goal of the promotion of light cigarettes is to reassure their clients and to keep as smokers those concerned about their health who are considering the possibility of quitting. There is evidence confirming that this policy is successful, and that the misconceptions of smokers about light cigarettes deter them from quitting. There is evidence on the role of smoking in weight control. Women are more concerned than men about their weight, and for young women body image is very relevant. Young girls often have the perception that they are overweight. Although fear of being overweight is more common than actual overweight, the evidence suggests that being afraid of gaining weight plays a role in keeping women as smokers, and that young girls begin smoking taking into consideration weight issues. The advice on how to maintain weight should be included in any smoking cessation program for women. Any preventive action for teen age smokers should include weight control issues.
  • El ejercicio de la medicina en el contexto médico-social del año 2000 Colaboraciones Especiales

    En el presente trabajo se expone el convencimiento sobre el cambio cualitativo que se va a producir en las próximas décadas en el ejercicio de la medicina, en relación a los cuatro momentos rectores que influyen en la praxis médica: la ciencia pura y aplicada, las políticas de la salud, la economía y la ética. Los conocimientos de la ciencia médica se van a aplicar en base a la evidencia científica, sustituyendo al empirismo científico que ha caracterizado al ejercicio médico en la última centuria. Las políticas de salud desde la sociedad organizada (Sanidad o Salud Publica) van a transformar sus modelos centralizados, transfiriendo competencias de gestión a los propios sanitarios de los centros públicos, que estarán orientados a objetivos de salud. A la vez, la administración sanitaria va a exigir mayor rigor en el reconocimiento de las prestaciones sanitarias fundadas en la evidencia científica. La economía de la salud del sistema público va a requerir que la prescripción de las intervenciones médicas y la introducción de nuevas tecnologías se atiendan en base a la efectividad probada en resultados en salud y, en consecuencia, la eficiencia en términos gerenciales va a ser sustituida por el coste/efectividad. Finalmente, la evolución de la ética médica unitaria seguirá evolucionando por la multiplicidad de éticas individuales y colectivas, por lo que, en defensa de la propia naturaleza (physis), el ejercicio médico requerirá el desarrollo del consentimiento informado, la objeción de conciencia y el consenso, ante nuevos códigos de conducta profesional de ámbito universal.

    This study discusses the conviction that the qualitative change which is going to be taking place over the next few decades with regard to the practice of medicine as far as the four governing aspects which bear an impact on medical practice are concerned: pure applied science; health-care policies; the economy and ethics. Medical science’s know-how is going to be applied based on scientific evidence, the empirical scientific approach that has characterized medical practice over the past century being replaced. The health care policies from the organized society (Public Health Care System) are going to be changing its centralized models, handing over management authorities to the health-care professionals proper working at the public medical centers oriented toward health-care objectives. In turn, the health-care administration is going to be demanding a greater deal of strictness with regard to the honoring of health-care benefits based on scientific proof. The health-care economy of the Public Health-Care System is going to be requiring that the prescribing of medical procedures and the implementation of new technologies be dealt with based on results-based proven effectiveness related to health and, therefore, efficiency in management terms is going to be replaced by cost-effectiveness. Finally, the trend in individual medical ethics is going to continue evolving due to the many different individual and group ethics, which, in defense of the very nature (physis) of the medical practice is going to be requiring the development of informed consent, conscientious objection and a consensus being reached regarding new codes of professional conduct of a universal scope.
  • Seroprevalencia de VIH en pacientes de consultas de enfermedades de transmisión sexual y planificación familiar. España, 1995-1996 Originales

    FUNDAMENTO: Las consultas de enfermedades de transmisión sexual (ETS) son buenos lugares para la vigilancia centinela de la transmisión sexual del VIH. El presente trabajo analiza los resultados de pruebas voluntarias de VIH, realizadas en 1995 y 1996, en una red de centros de ETS, VIH y planificación familiar de España. MÉTODO: Desde 1995 se remite anualmente a dichos centros un cuestionario para recoger el número de personas que analizaron para el VIH en el año previo, y el de aquellas que se confirmaron seropositivas. Se calculó la seroprevalencia en función del sexo y las principales categorías de transmisión del VIH. RESULTADOS: Contestaron 81 centros a la encuesta de 1995 y 78 a la de 1996. El 95% ofrecen la prueba de VIH a demanda. Entre los 39 centros que aportaron datos sobre las pruebas de VIH realizadas en 1996 se analizaron un total de 15.964 personas. El 4,8% (n=760) resultaron positivas (IC: 4,4%-5,1%). La seroprevalencia fue mayor en hombres (5,9%) que en mujeres (2,6%; p<0,001). La mayor seroprevalencia de VIH se encontró en usuarios de drogas inyectadas (UDIs) (18,7%). Al comparar la seroprevalencia de 1995 con la de 1996 se observan descensos en hombres homo/bisexuales, de 7,4% a 6,1% (p=0,049), y en mujeres que ejercen la prostitución, de 2,0% a 1,3% (p=0,034). Las parejas de UDIs pasaron del 3,4% al 4,6% (p=0,267). CONCLUSIONES: Estos resultados aportan una referencia nacional de la seroprevalencia de VIH en poblaciones con exposiciones sexuales de riesgo. La evolución es favorable en hombres homo/bisexuales y en prostitutas.

    BACKGROUND: The sexually-transmitted diseases (STDs) centers are good sentinels for monitoring the sexual transmitting of HIV. This study analyzes the results of voluntary HIV tests run in 1995 and 1996 within a network of STD, HIV and family planning centers in Spain. METHODS: Starting as of 1995, a questionnaire was sent out yearly to said centers for recording the number of individuals found to have HIV the previous year and the number of those who are proven to be HIV positive. A calculation of HIV incidence was based as a function of sex and the main HIV transmission categories. RESULTS: Eighty-one centers answered the survey in 1995, 78 having done so in 1996. Ninety-five percent of these centers ran the HIV test at the request of the interested parties. At the 39 centers which provided data regarding the HIV tests conducted in 1996, a total of 15,964 individuals had been tested. Positive: 4.8% (n=760) (CI: 4.4%-5.1%). The HIV seroprevalence was higher for males (5.9%) than for females (2.6%; p<0,0001). The highest HIV seroprevalence was found among injecting drug users (IDUs) (18.7%). On comparing HIV seroprevalence in 1995 with that of 1996, drops were detected in the number of homosexual/bisexual men from 7.4% to 6.1% (p=0.049) and in women prostitutes, from 2.0% to 1.3% (p=0.034). The sexual partner of IDU went from 3.4% to4.6% (p=0.267). CONCLUSIONS: These results provide a nationwide reference regarding HIV incidence among populations having high-risk sexual relations. The trend therein is favorable among homosexual/bisexual men and female prostitutes.
  • Evaluación de las propiedades psicométricas de la versión española del Rotterdam Symptom Checklist para medir calidad de vida en pacientes oncológicos Originales

    FUNDAMENTO: el presente trabajo aborda la validación de la versión española del Rotterdam Symptom Checklist (RSCL) para evaluar la calidad de vida de las personas con cáncer. El objetivo es evaluar las propiedades psicométricas del cuestionario y su equivalencia con la versión original inglesa. MÉTODO: Se cumplimentó el cuestionario por 162 pacientes oncológicos terminales y 59 pacientes sometidos a quimioterapia. Se evaluó su estructura interna a través de análisis factorial confirmatorio en pacientes oncológicos terminales, capacidad discriminante según situación clínica de los mismos, consistencia interna en ambas muestras y sensibilidad a los cambios, repitiendo el cuestionario tres veces a los pacientes sometidos a quimioterapia. RESULTADOS: Los pacientes terminales presentaron peor puntuación que los sometidos a quimioterapia para todas las escalas excepto la psicológica. El análisis factorial mostró una estructura interna similar a la versión original. La consistencia interna fue ³ 0,70 para todas las escalas y ³ 0,50 para las subescalas excepto la de quimioterapia. Se encontraron diferencias de puntuación (p<0,05) para todas las subescalas (excepto la psicológica) antes y después del tratamiento con quimioterapia. CONCLUSIONES: La similitud de las propiedades psicométricas de la versión española del RSCL con la versión original indica su utilidad como medida subjetiva de bienestar en la investigación clínica de personas con cáncer.

    BACKGROUND: In this work the validation of the Spanish version of the Rotterdam Symptom Checklist (RSCL) to assess quality of life in cancer patients has been approached. The main goal of this study is to evaluate the psichometric properties of the questionnaire and its equivalence with the original English questionnaire METHODS: the questionnaire was administered to a sample of 162 terminal cancer patients and 59 cancer patients treated with chemotherapy . The internal structure, was evaluated through a confirmatory factor analysis in TCP, the discriminatory power according to clinical situation of cancer patientes, the internal consistency in both samples and the responsiveness to the changes over time after chemotherapy treatment was evaluated in PQT. RESULTS: The internal structure of scales was similar in TCP to the original version. The cancer patients showed worse scores than the PQT. The reliability was ³ 0.70 for the scales and ³ 0.50 for the subscales except chemotherapy subscale. The scores of the scales were different (p< 0.05) before and after treatment, except for the psychological scale. CONCLUSIONS: These results confirm the usefulness of the spanish version of the RSCL as a subjective measure of well-being in cancer patients.
  • Influencia de los factores ambientales en el número de ingresos por urgencias en el complejo hospitalario "Juan Canalejo" de la Coruña: elaboración de un modelo de predicción Originales

    FUNDAMENTO: En este trabajo se trata de establecer, respecto al servicio de urgencias del hospital Juan Catalejo de A Coruña, las posibles asociaciones entre el número de ingresos por causas orgánicas, cardiovasculares y respiratorias, y las variables meteorológicas introducidas como variables exógenas con el fin de elaborar un modelo de predicción. MÉTODOS: Se utilizó la metodología de Box-Jenkins para obtener modelos ARIMA univariados de las series temporales consideradas. Se establecen funciones de correlación cruzada (FCC) entre las series de residuales que permitan establecer pesos y retrasos entre las variables, para una posterior modelización mediante modelos ARIMA multivariantes que incluyen variables ambientales. El periodo de tiempo estudiado fue de 1994 a 1996. RESULTADOS: Los ingresos urgentes por causas orgánicas aumentan significativamente entre 0 y 2 días después de un ascenso de la temperatura ambiental. Los ingresos por causas respiratorias se asocian con los descensos de temperatura con 10-14 retrasos, mientras que los ingresos por causas circulatorias se ven aumentados significativamente por el calor a largo plazo (10 retrasos). En mayores de 65 años se registran, además, aumentos significativos de los ingresos urgentes por causas circulatorias, en relación con el frío, a corto plazo. Los modelos ARIMA multivariantes que contemplan el efecto de variables ambientales ofrecieron un mejor ajuste para todas las variables de ingresos. CONCLUSIONES: El número de ingresos en el servicio de urgencias en el Complejo Hospitalario Juan Canalejo de A Coruña por causas orgánicas, respiratorias y circulatorias, presenta un patrón de comportamiento estacional. Los ingresos por causas respiratorias están asociados al descenso de la temperatura ambiental, mientras que los ingresos por causas circulatorias se ven afectados fundamentalmente por el calor, aunque también por el frío, en mayores de 65 años. Los modelos ARIMA multivariantes, incluyendo variables climatológicas, ofrecen un sistema de predicción de ingresos en función de dichas variables que puede ser útil desde el punto de vista de la gestión hospitalaria.

    BACKGROUND: This study is aimed at establishing the possible associations between the number of admissions through the emergency room at the "Juan Canalejo" Hospital in Corunna in 1994-1994 due to organic, circulatory and respiratory reasons and the weather variables introduced as being exogenous for the purpose of preparing a prediction model. METHODS: The Box-Jenkins methodology is used for obtaining univariate ARIMA models of the time-based series taken into consideration. Cross-Correlation Functions (CCFs) are established among the series of residuals which afford the possibility of establishing weights and lags among the variables for a subsequent modeling by means of multivariate ARIMA models which include environmental variables. RESULTS: The emergency admissions for organic reasons significantly increase 0-2 days following a rise in temperature. The admissions due to respiratory ailments are associated with drops in temperature with 10-14 lags, whilst the admissions for circulatory reasons increase significantly due to long-lasting spells of hot weather (10 lags). For people over age 65, significant increases in emergency admissions for circulatory reasons are also recorded with cold snaps. The multivariate ARIMA models that take into account the effect of environmental variables provided the best adjustment for all of the admissions variables. CONCLUSIONS: The number of emergency room admissions at the "Juan Canalejo" Medical Center Complex in Corunna due to organic, respiratory and circulatory causes shows a seasonal behavior pattern. The admissions for respiratory reasons are associated with a drop in temperature, whilst the admissions for circulatory reasons are affected fundamentally by hot weather, although also by cold weather as regards people over age 65. The multivariate ARIMA models including climate-related variables provide a system for predicting admissions in terms of said variables that can be useful from the standpoint of hospital management.
  • El conjunto mínimo básico de datos al alta hospitalaria como fuente de información para el estudio de las anomalías congénitas Originales

    FUNDAMENTO: El objetivo del estudio es evaluar la validez de los diagnósticos informatizados de alta hospitalarios de anomalías congénitas, comparándolos con la información contenida en la historia clínica. MÉTODOS: A partir de los registros de alta generados durante un año en 7 hospitales de la Comunidad Valenciana, se seleccionó aleatoriamente a 100 niños por cada hospital. Como estándar, se indicaron y codificaron los diagnósticos de las historias clínicas. Sólo se consideraron las altas generadas durante el primer año de edad. También se estudió el género, gravedad y la presentación aislada o múltiple de las anomalías congénitas. Se calculó la sensibilidad, especificidad, valores predictivos y sus intervalos de confianza al 95% por el método binomial exacto para el estudio de casos (niños) y la sensibilidad y el valor predictivo positivo para el estudio de diagnósticos. RESULTADOS: Se detectaron 126 niños con anomalías congénitas y 201 diagnósticos en las historias clínicas, y 83 niños con anomalías congénitas y 108 diagnósticos en los registros. Para la detección de casos los registros presentaron una sensibilidad del 64%, una especificidad del 99,1% y unos valores predictivos positivos y negativos mayores del 90%. En la detección de diagnósticos, la sensibilidad fue del 46% y el valor predictivo positivo del 83%. La sensibilidad varió mucho según los diagnósticos. CONCLUSIONES: Los registros de alta hospitalaria presentan una especificidad y unos valores predictivos elevados pero una baja sensibilidad. Estos hechos se deben considerar cuando se utilicen estos registros como fuente de casos para los estudios epidemiológicos de las anomalías congénitas.

    PURPOSE: The purpose of this study is that of assessing the validity of the computerized diagnoses of hospital discharges of congenital defects by comparing them with the information included in the medical history. MEANS AND METHODS: Based on the discharge records generated over a one-year period at 7 hospitals in the Autonomous Region of Valencia, 100 children were selected at random from each hospital. As a standard, the diagnoses stated in the medical histories were indexed and coded. Solely those discharges having taken place during the first year of life were considered. A study was also made of the type, seriousness and individual or combinations of congenital defects. A calculation was made of the sensitivity, specificity, predictive values and the 95% confidence intervals thereof by the exact binomial method for the case studies (children) and the positive predictive value and sensitivity for the study of diagnoses. RESULTS: 126 children were detected as having congenital defects, and 201 diagnoses in medical records, and 83 children with congenital defects and 108 diagnoses on record. For the detection of cases, the records showed a 64% sensitivity, a 99.1% specificity and some positive and negative predictive values of over 90 %. With regard to the detection of diagnoses, the sensitivity was 46% and the positive predictive value 83%. The sensitivity varied a great deal depending upon the diagnoses. CONCLUSIONS: The hospital discharge records revealed a high degree of specificity and high predictive values, but a low degree of sensitivity. These facts must be considered when these records are used as a source of cases for the epidemiological studies of congenital defects.
  • Caracterización de la cirugía mayor ambulatoria en un hospital general básico Originales

    FUNDAMENTO: La cirugía ambulatoria surge y se desarrolla como consecuencia del aumento progresivo de la demanda y del gasto sanitario. Aunque ha permitido reducir el coste por proceso y mejorar la eficacia de los hospitales, manteniendo los niveles de satisfacción y seguridad del paciente, también ha obligado a cuestionar los indicadores de actividad habituales; esto nos ha llevado a evaluar la unidad de cirugía ambulatoria del hospital de Algeciras, de reciente creación, en base al sistema de pago habitual en otros países, los Grupos Relacionados de Diagnóstico (GRD). MÉTODO: Estudio descriptivo sobre las 3.051 intervenciones quirúrgicas realizadas de forma programada durante 1997 (exceptuando la cirugía menor con anestesia local), utilizando como fuente de información el conjunto mínimo básico de datos al alta hospitalaria, obteniéndose y analizando los grupos relacionados de diagnóstico. Se han calculado las estancias evitadas en base a la estancia media de los pacientes ingresados por el mismo grupos relacionados de diagnóstico en el período de tiempo de estudio. RESULTADOS: La cirugía mayor ambulatoria permitió un porcentaje de sustitución global del 50,4% (33.3 % de todos los pacientes intervenidos de forma reglada; 4,1 % sobre el total de ingresos del hospital), lo que supuso un ahorro de 2.112 estancias hospitalarias. Los grupos relacionados de diagnóstico más frecuentes y con mayor impacto en estancias evitadas fueron los procedimientos quirúrgicos del cristalino y los legrados uterinos o conizaciones realizados por causa distinta a neoplasia maligna. La tasa de reingresos fue del 1,5 %. CONCLUSIONES: El porcentaje de sustitución global obtenido por esta unidad de cirugía mayor ambulatoria es considerado aceptable, si bien, debe incrementarse en los próximos años. Encontramos diferencias en la distribución de los procesos atendidos en relación a otras unidades de nuestro entorno, explicables por las dificultades que supone comenzar una cirugía de estas características.

    BACKGROUND: Outpatient surgery initially came into being and further developed as a result of the progressive increase in the demand and in hospital expenses. Although it has afforded the possibility of lowering the cost per procedure and of improving the efficiency of the hospitals by maintaining the degrees of patient satisfaction and safety, it has also made it necessary to question the customary activity indicators. This had led us to assess the outpatient surgery units of the recently-opened Algeciras Hospital based on the customary system of payment in other countries, the Related Diagnostic Groups (RDGs). METHODS: Descriptive study regarding 3,051 surgical procedures performed on a scheduled basis in 1997 (not including the minor surgery using a local anesthetic), using as a source of information the minimum basic set of data for hospital discharge, obtaining and analyzing the RDGs. The hospital stays avoided based on the average stay of those patients admitted for the same RDG within the time period under study have been calculated. RESULTS: Major surgery performed on an outpatient basis afforded the possibility of an overall replacement percentage of 50.4% (33.3% of all of the patients who underwent surgery on a scheduled basis, that is, 4.1% of all of this Hospitals admissions), which meant an savings of 2,112 hospital stays. The most frequent RDGs having the greatest impact on stays avoided were the surgical procedures performed on the crystalline lens and the scraping or conization procedures performed for reasons other than malignant neoplasia. The readmission rate was 1.5%. CONCLUSIONS: The overall replacement rate obtained by this major outpatient surgery unit is considered to be acceptable, although it must increase within the next few years. We found differences in the spread of the processes dealt with as compared to other units in our environment, this being explainable due to the difficulties involved in starting up surgery of these characteristics.
  • Ejercicio físico y hábitos alimentarios: un estudio en adolescentes de Cádiz Originales

    FUNDAMENTO: Actualmente, se describe un incremento de trastornos alimentarios en los adolescentes. El ejercicio físico, sobre todo practicado de forma individual, es uno de los métodos elegidos para perder peso. En la realización de este trabajo se partió de la hipótesis de una mayor coincidencia de conductas alimentarias alteradas en los sujetos que realizan deporte individual. En él se describen y comparan los hábitos alimentarios de los adolescentes que realizan ejercicio de forma individual frente a los deportistas colectivos o en equipo. MÉTODO: Se realizó un estudio transversal en 532 adolescentes escolarizados que realizan deporte, y que tienen edades de 14 a 18 años, los cuales fueron seleccionados aleatoriamente mediante muestreo bietápico estratificado. Se establecieron dos grupos según practicaran deporte individual (n:216) o colectivamente (n:316). Se analizaron los hábitos alimentarios en ambos grupos a partir de un cuestionario autocumplimentado. RESULTADOS: En el grupo de los estudiantes con preferencia por los deportes individuales predominaban las mujeres (razón de masculinidad 0,44). En éstas era 3,12 veces más frecuente el seguimiento de dietas de adelgazamiento y 3,73 veces la existencia de episodios de ingesta compulsiva. Respecto a las conductas que pudieran considerarse compensatorias existe clara concentración de éstas en deportistas individuales; así, el 43% manifestó pasar voluntariamente episodios de ayuno (4,96 veces más que en deportistas colectivos), un 46% manifestó haberse provocado el vómito alguna vez con fines "dietéticos" (3,76 veces más) y hasta un 26% había utilizado laxantes con intención de perder peso (2,56 veces más que en deportistas en equipo). CONCLUSIÓN: La existencia de conductas alimentarias alteradas parece darse más en jóvenes con prácticas deportivas individuales que en los que practican deportes en equipo.

    BACKGROUND: A increased number of eating disorders among teen-agers are currently being reported. Physical exercise, especially when done individually, is one of the methods chosen for losing weight. We are basing this study on the hypothesis of a larger number of eating habit disorders (EHDs) in subjects who do physical exercise alone. This study describes and compares eating habits among teen-agers that do individual exercise as opposed to athletes who work out in groups or on teams. METHOD: Cross-section study of 532 teen-agers ages 14-18 who are enrolled in school and who do physical exercise, having been selected at random by means of a two-stage, stratified sampling process. The subjects were divided into two groups according whether they did individual physical exercise alone (Number: 216) or in groups (Number: 316). The eating habits of both groups were analyzed based on a questionnaire filled out by the subjects themselves. RESULTS: In the group preferring individual sports, females were predominant (degree of males 0.44). Of these females, their being on diets in order to lose weight was 3.12 times more frequent, compulsive eating episodes being 3.73 times more frequent. As regards behaviors which might be considered to be compensatory, there is a clear concentration thereof among those who do sports individually, hence 43% stated to voluntarily undergo periods of fasting (4.96 times more than those who exercise in groups), 46% stating to have brought on vomiting at one time or another for "dieting" purposes (3.76 more) and up to 26% have used laxatives with the intention of losing weight (2.56 times more than among athletes who play on teams). CONCLUSION: The existence of EHDs seems to be associated with teen-agers who play individual sports as opposed to those who play on teams.
