    En este artículo discutimos algunas aportaciones del filósofo austro-británico Karl R. Popper, uno de los más influyentes pensadores contemporáneos, cuyas teorías epistemológicas y sociopolíticas han llegado también al ámbito de la epidemiología. Nos centramos principalmente en el llamado problema de la inducción. Sostenemos, siguiendo a Popper, que el método científico no usa un razonamiento inductivo, sino hipotético-deductivo. Aunque el paso desde los datos que evalúan una hipótesis a una conclusión sobre ésta va de lo particular a lo general, e.d., en dirección inductiva, no existe la inducción como razonamiento o inferencia. Es decir, no existe un método que permita inducir o verificar las hipótesis o teorías (no es posible explorar todas las situaciones posibles para ver si la teoría se mantiene), ni siquiera hacerlas muy probables. Además, los científicos buscan teorías altamente informativas, no altamente probables. Lo que se hace realmente es proponer una hipótesis como solución tentativa de un problema, confrontar la predicción deducida de la hipótesis con la experiencia, y evaluar si la hipótesis queda rechazada o no por los hechos. Al no poder verificarse las teorías sólo podemos aceptarlas si resisten el intento de rechazarlas. Por tanto, la contrastación consiste en la crítica o intento serio de falsación, es decir, la eliminación de error dentro de una teoría, para rechazarla si es falsa. El objetivo es, pues, la búsqueda de teorías verdaderas. Para ello, el método científico utiliza un conjunto sistemático de reglas metodológicas (no lógicas), es decir, decisiones. Estas reglas o principios metodológicos se resumen en dos: ¡sea inventivo y crítico!, e.d., proponga hipótesis audaces y sométalas a tests rigurosos de la experiencia. La lógica juega principalmente su papel al permitir deducir de la hipótesis las predicciones que se confrontarán con los hechos o evidencias. Esto es aplicable tanto a la inferencia estadística como a la inferencia causal. Argumentamos que los criterios de causalidad usados en epidemiología no son sino reglas del método destinadas a lo mismo: tratan de eliminar o reducir el error (azar, sesgos...) al contrastar una hipótesis causal. Por tanto, la llamada inferencia causal, el paso de la evidencia a la teoría causal, no es un proceso lógico inductivo o probabilístico sino decisión basada en la evaluación de una hipótesis causal gracias a reglas metodológicas como los criterios de causalidad. Pensamos que el interés del debate entre los epidemiólogos popperianos y los inductivistas no es meramente verbal, pues si somos conscientes de que no operamos inductivamente, que no podemos establecer firmemente una hipótesis, ni siquiera afirmarla probabilísticamente, seremos presuntamente más humildes en nuestra actitud y buscaremos más los errores en las teorías que sus fáciles ejemplos confirmadores.

    In this article we are discussing a few of the contributions by the Austro-British philosopher Karl R. Popper, one of our most influential contemporary thinkers, whose epistemological and socio-political theories have also penetrated the sphere of epidemiology. We are focusing mainly on the so-called problem of induction. We sustain, in line with Popper, that the scientific method does not use inductive reasoning, but rather hypothetical-deductive reasoning. Although the movement from the data evaluating a hypothesis to a conclusion on the latter goes from the specific to the general, that is, in an inductive direction, the induction does not exist as a reasoning process or inference. That is, there is no method that enables us to infer or to verify hypotheses or theories (we cannot explore all of the possible situations to see whether the theory stands up), or even to render them very probable. Besides, scientists look for highly informative theories, not highly probable ones. What we actually do is to propose a hypothesis as a tentative solution to a problem, to confront the prediction deduced from the hypothesis with actual experience, and evaluate whether the hypothesis is rejected or not by the facts. As theories cannot be verified, we can only accept them if they withstand an attempt to reject them. Consequently, the test of a theory consists of criticism or a serious attempt at falsification, that is, the elimination of error within a theory, in order to reject it if it is false. The objective is, thus, the search for true theories. For this purpose, the scientific method uses a systematic set of methodological (not logical) rules, that is, decisions. These methodological rules or principles can be summed up in two: be inventive and critical!, that is, propose bold hypotheses and subject them to severe tests of experience. Logic plays its role mainly by allowing us to deduce from a hypothesis the predictions to be confronted with the facts or evidence. This is applicable both to statistical inference as well as to causal inference. We argue that the criteria of causality used in epidemiology are none other than rules of the method designed for the same purpose: they are concerned with eliminating or reducing errors (chance, bias ) on testing a causal hypothesis. Consequently, the so-called causal inference, the step from evidence to causal theory, is not a logical inductive or probabilistic process but rather a decision based on the evaluation of a causal hypothesis thanks to methodological rules such as the criteria of causality. We believe that the interest of the debate between the Popperian and the inductivist epidemiologists is not merely a matter of words, as, if we are aware that we do not operate inductively, that we cannot establish firmly hypotheses, not even affirm them probabilistically, we will presumably adopt a humbler attitude and look more for the errors in our theories than for their facile examples of confirmation.

    FUNDAMENTO: Analizar el efecto que tiene la elección de una población de referencia sobre diferentes indicadores derivados de las tasas de mortalidad ajustadas por edad. MÉTODOS: Las tasas de mortalidad por diferentes causas de muerte en España de 1971 a 1992 son ajustadas empleando cuatro poblaciones de referencia: poblaciones españolas de 1971 y 1992, población estándar europea y población estándar mundial. Los resultados obtenidos con las cuatro poblaciones se comparan empleando tres indicadores: diferencia entre las tasas de 1992 y 1971, razón entre las tasas de 1992 y 1971y cambio anual porcentual entre 1971 y 1992. RESULTADOS: En la mayor parte de las causas de muerte estudiadas incluyendo el total de causas, la cardiopatía isquémica y casi todos los tumores, la razón de tasas y el porcentaje de cambio anual son similares con independencia de cuál sea la población estándar empleada. En cambio, la diferencia de tasas es muy diferente en función de la población de referencia. En las enfermedades infecciosas y el cáncer de testículo se produce la situación contraria: la diferencia de tasas es robusta mientras que la razón de tasas varía con la población estándar. Finalmente, la mortalidad por neumonía, enfermedad de Parkinson y cáncer de encéfalo muestra cambios en los tres indicadores utilizados. CONCLUSIONES: Es necesario realizar un análisis de las tasas específicas por edad antes de proceder a su ajuste. Este análisis permite averiguar si el ajuste por edades es correcto y qué indicador (diferencia, razón o cambio porcentual) será adecuado para realizar comparaciones.

    BACKGROUND: To analyse the effect the choice of reference population has on different indicators derived from mortality rates age-adjusteing. METHODS: The mortality rates for different causes of death in Spain from 1971 to 1992 were adjusted by using four reference populations: Spanish populations from 1971 and 1992, the Standard European population and the standard world population. The results obtained with the four populations were compared by applying three indicators: difference between the rates of 1992 and 1971, ratio between the rates of 1992 and 1971 and annual percentage change between 1971 and 1992. RESULTS: In the majority of the causes of death studied, including the total, ischemic heart disease and almost all of the tumours, the ratio between the rates and the percentage of annual change are similar regardless of the standard population used. However, the difference in rates is very noticeable in relation to the reference population. In the infectious diseases and testis cancer, the opposite occurs: the different in rates is steadfast while the ratio between the rates varies with the standard population. Finally, the mortality due to pneumonia, Parkinsons disease and encephalic cancer show changes all three indicators used. CONCLUSIONS: An analysis of the specific rates by age must be made before proceeding with their adjustment. This analysis will enable us to ascertain whether the adjustment by age is correct and which indicator (difference, ratio or percentage change) will be appropriate for making comparisons.
  • Utilización de anfotericina B no convencional en el Hospital Clínico de San Carlos Originales

    FUNDAMENTO: La anfotericina B es el tratamiento de elección de las infecciones fúngicas sistémicas, pero su utilidad clínica está limitada por su toxicidad. Las formulaciones lipídicas parecen igualmente eficaces y más seguras, pero tienen un mayor coste. El incremento del consumo y del gasto de estas formulaciones nos llevo a plantear un estudio para conocer su perfil de utilización (cuantitativo y cualitativo) y evaluar la repercusión económica de su uso inapropiado. MÉTODOS: Se desarrollaron unas normas de uso de la anfotericina B y se evaluó de forma retrospectiva, la calidad de la prescripción de la anfotericina B no convencional (anfotericina B noC) y la repercusión económica de su uso incorrecto. RESULTADOS: En el 54% de los tratamientos se hizo una mala selección de anfotericina B; en el 3.5% no estaba indicada la utilización de anfotericina B. El exceso de gasto derivado de la prescripción incorrecta fue de 42 millones de pesetas, un 35% del gasto total en medicamentos; el gasto por prescripción innecesaria fue de 1.720.327 pesetas. CONCLUSIONES: La evaluación retrospectiva ha mostrado que existe un elevado porcentaje de tratamientos que no se adecuan a lo recomendado en las normas de prescripción. La puesta en marcha de intervenciones informativas permitiría realizar una selección más eficiente de la anfotericina B noC, mejorando la calidad de la prescripción, lo que podría suponer un importante ahorro económico.

    BACKGROUND: Amphotericin B is the treatment of choice for systemic fungal infections, however, its clinical usefulness is limited by its toxicity. The lipid formulations appear to be equally effective and safer, but are more costly. The increase in the consumption of , and expenditure on these formulas led us to undertake a study in order to identify their profile of use (quantitative and qualitative) and to assess the financial repercussions when used inappropriately. METHODS: A set of rules were developed for the use of amphotericin B, and the quality of the prescription of non-conventional amphotericin B (amphotericin B notC) was evaluated retrospectively together with the financial repercussions of its inappropriate use. RESULTS: In 54% of the treatments studied, a poor selection of amphotericin B was made; in 3.5%, the use of amphotericin B was not indicated. The excess expenditure derived from the inappropriate use amounted to 42 million pesetas, 35% of the total expenditure on medicines; the expenditure due to unnecessary prescription was 1,720,327 pesetas. CONCLUSIONS: The retrospective evaluation has shown that there is a high percentage of treatments that do not conform with the recommendations contained in the prescription rules. The holding of information sessions would assist in achieving a more efficient selection of the amphotericin B notC; this would improve prescription quality, which might also deliver significant financial savings.
  • Prevalencia de tratamiento de la infección tuberculosa en la prisión provincial Originales

    FUNDAMENTOS: La tuberculosis en prisión es un grave problema de salud pública. En aquellos centros con elevadas prevalencias de coinfección por M. tuberculosis e infección VIH el tratamiento de la infección tuberculosa puede ser una importante herramienta de prevención y control. Conocer la prevalencia de infección tuberculosa, el acceso y la adherencia al tratamiento de la infección tuberculosa en población reclusa sin antecedentes de tratamiento de enfermedad o infección tuberculosa puede ser de gran interés para valorar la efectividad de esta medida. MÉTODOS: Se realizó un estudio transversal en la población ingresada a fecha 28 de junio de 1999 en un Centro Penitenciario con un programa de prevención y control de la tuberculosis basado en la terapia directamente observada (TDO) de los enfermos, en el diagnóstico precoz y el tratamiento de la infección tuberculosa. El tratamiento de la infección tuberculosa se prescribió tras consejo médico individualizado consensuándose con el paciente la modalidad del mismo (TDO, pauta diaria o bisemanal). Se revisaron las historias clínicas de los pacientes ingresados para conocer la prevalencia de pacientes infectados por M. tuberculosis, de los que habían finalizado y de los que se encontraban realizando tratamiento de la infección tuberculosa. Se realizó el test de Eidus-Hamilton en el caso de los pacientes en tratamiento frente a la infección tuberculosa para valorar su cumplimiento. RESULTADOS: De los 219 internos, 127 (58 %) presentaban criterio de infección tuberculosa. Se indicó tratamiento de la infección tuberculosa en 113 pacientes de los que 29 no aceptaron (25,7 %). De los 84 pacientes que iniciaron tratamiento de la infección tuberculosa, 22 (26,2 %) lo abandonaron, 39 (46,4 %) habían finalizado el tratamiento de la infección tuberculosa, y 23 (27,4 %) se encontraban realizándola. De estos últimos el 95,7 % resultaron positivos al test de Eidus-Hamilton. El 48 % de los infectados por M. tuberculosis, o habían finalizado o estaban realizando correctamente el tratamiento de la infección tuberculosa. CONCLUSIÓN: La prevalencia de infección tuberculosa observada en este colectivo es muy elevada. Se han observado aceptables niveles de acceso y adherencia al tratamiento de la infección tuberculosa lo que se traduce en una elevada cobertura de tratamiento de la infección tuberculosa en la población estudiada.

    BACKGROUND: Tuberculosis in prisons is a serious public health problem. In those penitentiaries with a high prevalence of co-infection by M. tuberculosis and HIV infection, the treatment of the tubercular infection can be an important tool for prevention and control. Ascertaining the prevalence of tubercular infection, the access and adherence to the treatment of tubercular infection in a prison population without a history of treatment of tubercular disease or infection could be of great interest in order to assess the efficacy of this measure. METHODS: A cross study was made of the population admitted at 28 June 1999 in a Penitentiary with a programme of prevention and control of tuberculosis, based on the directly observed therapy (DOT) of the patients, on early diagnosis and the treatment of the tubercular infection. The treatment of the tubercular infection was prescribed following individualised medical consultation, reaching an agreement with the patient as to the modality of treatment (DOT, daily or twice-weekly check-up). The clinical histories of the patients were reviewed in order to determine the prevalence of patients infected by M. tuberculosis, of those who had completed treatment and of those who were still undergoing treatment of tubercular infection. The Eidus-Hamilton test was performed in the case of patients under treatment for tubercular infection in order to evaluate compliance. RESULTS: Of the 219 inmates, 127 (58 %) presented criteria of tubercular infection. Treatment of the tubercular infection was indicated in 113 people of whom 29 refused (25.7 %). Of the 84 patients who initiated treatment for their tubercular infection, 22 (26.2 %) abandoned it, 39 (46.4 %) had finalised the treatment, and 23 (27.4 %) were still undergoing treatment. Of the latter, 95.7 % showed positive to the Eidus-Hamilton test. 48% of those infected by M. tuberculosis, either had finalised or were undergoing the treatment against tubercular infection correctly. CONCLUSION: The prevalence of tubercular infection observed in this group is very high. Acceptable levels of access and adherence to the treatment of tubercular infection have been observed, which translates into a high level of treatment coverage of this infection in the population studied.
  • Enfermedad meningocócica. Situación en España en la temporada 1998-1999 Segunda Jornada sobre la Enfermedad Meningocócica en España - Estudios Generales

    FUNDAMENTO: El cambio en el patrón epidemiológico sufrido por la enfermedad meningocócica en gran parte del país en la temporada 1996-1997 y la decisión de intervenir mediante una campaña de vacunación en el grupo de 18 meses a 19 años de edad con vacuna bivalente de polisacáridos, fueron el motivo para reforzar la vigilancia epidemiológica de esta enfermedad MÉTODOS: Se calcularon, para la temporada 1998-1999, tasas y otros indicadores de incidencia y letalidad por serogrupo y edad según los datos notificados al Sistema de Enfermedades de Declaración Obligatoria. RESULTADOS: La incidencia global fue superior que la temporada previa, aunque, la razón de tasas no indicó un riesgo significativamente mayor. Se produjo un aumento significativo de la tasa por serogrupo B y por segundo año consecutivo, la tasa por serogrupo C disminuyó, aunque no de forma significativa. Se produjo un aumento en el número de defunciones. La letalidad por serogrupo C aumentó en el grupo de 1 a 4 años. La incidencia se mantuvo alta y con un claro patrón estacional en las comunidades que no vacunaron. En el resto la incidencia se redujo y el patrón estacional desapareció. CONCLUSIONES: A los dos años de la intervención el predominio de casos del serogrupo B es general en todas las CCAA, excepto en las tres que no vacunaron, en ellas, durante estos dos últimos años, ha predominado el serogrupo C. En las CCAA que vacunaron se aprecia un incremento de la incidencia, aunque no significativo, entre los niños menores de cuatro años.

    BACGROUND: The change in the epidemiological pattern undergone by meningococcal disease in a large part of the country in the 1996-1997 season and the decision to intervene by means of a mass vaccination campaign in the age group between 18 months and 19 years of age, using a bivalent polyssacharide vaccine, justified to enhance the epidemiological surveillance of this disease. METHODS: Rates and other indicators of incidence and mortality were calculated for the 1998-1999 campaign by serogroup and age, according to the data notified to the Compulsory Disease Reporting System. RESULTS: The overall incidence was higher than the previous season, although, the rate ratio did not indicate a significantly greater risk. A statistically significant increase in the rate of incidence caused by serogroup B was registered, and for the second consecutive year, the serogroup C rate dropped, although not significantly. There was an increase in the number of deaths. The fatality rate due to serogroup C increased in the group between 1 and 4 years of age. The incidence remained high and with a clear seasonal pattern in the Communities that did not vaccinate. In the remainder, the incidence dropped and the seasonal pattern disappeared. CONCLUSIONS: Two years after the campaign, the predominance of serogroup B cases is generalised in all of the Autonomous Communities, except in the three where the immunisation was not performed. In the latter, in the course of the last two years, serogroup C has predominated. In the Communities where the vaccination campaign was carried out, there is an increase in the incidence, although not significantly, among children under 4 years of age.
  • Situación de la cepa epidémica de Neisseria meningitidis C:2b:P1.2,5 después de la vacunación A+C Segunda Jornada sobre la Enfermedad Meningocócica en España - Estudios Generales

    Tras la utilización de una amplia campaña de inmunización en la población de entre 2 y 19 años de edad en la mayoría de las Comunidades Autónomas, con objeto de controlar el "brote u onda epidémica" por serogrupo C de meningococos de los años 1996-97, interesaba conocer la situación de la conocida cepa epidémica C:2b:P1.2,5, que había sido identificada como la causante de los cambios producidos en el perfil epidemiológico de la enfermedad meningocócica en España, lo que nos podría permitir analizar algunas de las posibles tendencias futuras de esta enfermedad en nuestro país. El análisis de la situación en los primeros 10 meses de 1999 revela que se ha vuelto a cifras semejantes a las que se daban, tanto en numero de casos como en frecuencia de serogrupos, similar a la que se observaba en España a principios de los años 90, aunque la cepa epidémica C:2b:P1.2,5 representa el 56% del total de casos de serogrupo C. Las cepas de serotipo 2a, muy frecuentes en el serogrupo C en países Europeos, siguen representando una minoría en España. Es interesante finalmente mencionar la aparición de variantes antigénicas que podrían ser resultado de procesos de recombinación genética intraespecíficos y que serían seleccionados en función de sus ventajas evolutivas.

    Following the use of an extensive immunisation campaign targeting the population between ages 2 and 19 in the majority of the Autonomous Communities (Regions), for the purpose of controlling the "outbreak or epidemic wave" caused by serogroup C in 1996-97, there was great interest in ascertaining the situation of the well-known epidemic strain C:2b:P1.2.5, which had been identified as the cause of the changes brought about in the epidemiological profile of meningococcal disease in Spain, as this would enable us to analyse some of the possible future tendencies of this disease in our country. An analysis of the situation in the first 10 months of 1999 reveals that we have reverted to figures similar to those which, both insofar as the number of cases as well as the frequency of serogroups, were observed in Spain at the beginning of the decade of the nineties, although the epidemic strain C:2b:P1.2.5 represents 56% of the total cases of serogroup C. The strains of serotype 2a, very frequent in serogroup C in European countries, continue to represent a minority in Spain. Finally, it is of interest to mention the appearance of antigen variants which could be the result of processes of intra-specific genetic recombination and which would presumably have been selected in terms of their evolutive advantages.
  • Resultados del estudio de subdetección del meningococo en vacunados en Galicia Segunda Jornada sobre la Enfermedad Meningocócica en España - Estudios Generales

    FUNDAMENTOS: A partir la vigilancia activa y el seguimiento de la Enfermedad Meningocócica (EM) tras la campaña de vacunación realizada en Galicia, se observó que la proporción de aislamientos del serogrupo responsable de la enfermedad entre los casos de sospechosos de EM (SEM) que habían sido vacunados era menor que entre los no vacunados. Ante esta situación se realizó un estudio con el fin de determinar si en el origen de esas SEM sin aislamiento se encontraba la N. Meningitidis del serogrupo C y cuantificar la importancia de esa subdetección. MÉTODOS: Para ello, y durante el período de estudio (desde la semana 26 de 1997 a la semana 14 de 1999), se tomaron muestras de LCR y sangre de las SEM sin aislamiento, para su estudio con PCR para especie y serogrupo. El análisis de las muestras fue realizado por el laboratorio de microbiología del hospital Clínico de Santiago de Compostela. RESULTADOS: De los 120 casos notificados durante el periodo de estudio, se analizaron por PCR 65 (38 vacunados y 27 no vacunados), resultando positivas para N. meningitidis en un 65% (42 muestras), 74% en vacunados y 52% en no vacunados. . Estimando, a partir de los casos estudiados, los resultados para el total, y excluyendo los casos PCR negativo, encontramos que, para el serogrupo C, sólo en el 27% de los casos ocurridos en vacunados se consigue aislarlo, frente al 80% en los no vacunados (p<0.0001). Estos porcentajes son, para el caso del B, del 59 y 71% respectivamente, diferencia estadísticamente no significativa. CONCLUSIONES: La vacuna provocó una verdadera subdetección de meningococos del serogrupo C entre los casos vacunados

    BACGROUND: On the basis of active surveillance and the monitoring of Meningococcal Disease (MD) following the vaccination campaign carried out in Galicia, it was observed that the proportion of isolations of the serogroups responsible for the disease among individuals suspected of Meningococcal Disease (SMD) who had been vaccinated was lower than among unvaccinated individuals. In view of this situation, a study was made in order to determine whether in the origin of those SMDs that were not isolated, we would find N. Meningitidis serogroup C, and to quantify the significance of the sub-detection of same. METHODS: For this purpose, and during the period under study (from the 26th week of 1997 to the 14th week of 1999), blood and cephalorachidian fluid samples were taken from the SMDs without isolation for their study with C protein reagent for type and serogroup. The analysis of the samples was performed by the microbiology laboratory of the Clinical Hospital of Santiago de Compostela. RESULTS: Of the 120 cases notified during the period under study, 65 were analysed by C protein reagent (38 vaccinated and 27 unvaccinated), with a positive reading for N. meningitidis in 65% (42 samples) 74% in vaccinated individuals and 52% in unvaccinated. By estimating, on the basis of the cases studied, the results for the total, and excluding the C protein reagent negative cases, we find that, for serogroup C, in only 27% of the cases occurring in vaccinated individuals was it possible to isolate it, in comparison with 80% in the case of unvaccinated subjects (p<0.0001). These percentages are, in the case of serogroup B, 59% and 71%, respectively, a difference which is not statistically significant. CONCLUSIONS: the vaccine brought about an true sub-detection of serogroup C meningococci in the vaccinated cases.

    FUNDAMENTO: En España, la presentación de la enfermedad meningocócica en la década de los años ochenta estuvo estrechamente ligada al predominio de N. meningitidis serogrupo B. Esta situación cambió a principios de los años noventa, constatándose una tendencia creciente de cepas del serogrupo C en los aislamientos realizados en algunas zonas del país y un aumento de la incidencia de la enfermedad, a partir de la temporada 1995-1996. El objetivo de este trabajo es caracterizar el patrón epidemiológico de la enfermedad meningocócica en España durante el período 1990-97 y, en particular, el causado por el fenotipo C:2b:P1.2,5 en este período de cambio etiológico. MÉTODOS: Estudio retrospectivo de los casos incidentes de enfermedad meningocócica en el territorio español, excepto las CC AA de Andalucía, Cantabria, Madrid, País Vasco y Valencia, durante el período 1990-1997. Los datos epidemiológicos se han obtenido del sistema de Enfermedades de Declaración Obligatoria (EDO), a través de la notificación individualizada de casos, y los datos microbiológicos del Laboratorio de Referencia de Meningococo (LRM) del Centro Nacional de Microbiología. RESULTADOS: La incidencia de enfermedad meningocócica en el período estudiado, 1990-1997, fue de 3,81x 10-6 personas-año, aumentando 0,1851 casos por 100.000 habitantes por año. A partir de 1995, la incidencia por serogrupo C prácticamente se triplicó con respecto al período anterior, con una tasa de incidencia en 1997 de 2 casos por 100.000 habitantes. Este aumento fue en gran parte debido a la emergencia del fenotipo C:2b:P1.2,5, que ese mismo año llegó a alcanzar una incidencia de 1 caso por 100.000 habitantes. El aumento de incidencia afectó a todas las edades, pero fue sobre todo significativo en el grupo de 5-19 años (tasa de crecimiento anual: 13,3%; P<0,001). También se constató un incremento en el número de brotes declarados y casos asociados a los mismos. La tasa de letalidad global fue de 7,7% (intervalo de confianza al 95%: 7,0-8,4), y la letalidad asociada al fenotipo C:2b:P1.2,5 fue significativamente más elevada que la del serogrupo B elegido de referencia (odds ratio: 1,69; IC 95%: 1,05-2,71), previo ajuste de edad, sexo, forma clínica y año. CONCLUSIONES: El patrón de la enfermedad meningocócica en el territorio estudiado, durante 1990-1997, se ha caracterizado por un aumento de la incidencia del serogrupo C, en especial del fenotipo emergente C:2b:P1.2,5. Junto a este aumento se ha observado un desplazamiento de la incidencia a grupos de edad más elevados, tendencia a agregaciones temporo-espaciales de casos y un aumento de la letalidad asociado al nuevo fenotipo. Este patrón es característico de situaciones epidémicas de enfermedad meningocócica por el serogrupo C.

    BACKGROUND: In Spain, the presentation of meningococcal disease in the 1980s was closely linked to the predominance of N. Meningitidis serogroup B. This situation changed at the beginning of the 1990s, observing a growing trend of serogroup C strains in the isolation procedures carried out in some areas of the country, together with an increase in the incidence of the disease starting in the 1995-1996 season. The purpose of this study is to characterise the epidemiological pattern of meningococcal disease in Spain during the 1990-97 season and, in particular, where the disease was caused by the phenotype C:2b:P1.2.5 in this period of etiological change. METHODS: Retrospective study of the cases of meningococcal disease in Spain, with the exception of the Autonomous Communities of Andalusia, Cantabria, Madrid, Basque Country and Valencia, between 1990-1997. The epidemiological data were obtained from the Compulsory Disease Reporting system, through individualised notification of cases, and the microbiological data from the Meningococcus Reference Laboratory of the National Microbiology Centre. RESULTS: The incidence of meningococcal disease in the period studied, 1990-1997, was 3.81x10-6 person-years, increasing by 0.1851 cases per 100,000 inhabitants per year. Starting in 1995, the incidence caused by serogroup C practically tripled with respect to the preceding period, with a rate of incidence in 1997 of 2 cases per 100,000 inhabitants. This increase was to a large extent due to the emergence of the phenotype C:2b:P1.2,5, which in that same year registered an incidence of 1 case per 100,000 inhabitants. The increase in incidence affected all age groups, but was particularly significant in the 5-19 year-old group (annual rate of growth: 13.3%; P<0,001). It was also found that there was an increase in the number of outbreaks reported and cases associated to them. The overall mortality rate was 7.7% (95% confidence interval: 7.0-8.4), and the mortality associated with the phenotype C:2b:P1.2.5 was significantly higher than that of the serogroup B chosen as reference (odds ratio: 1.69; 95% CI: 1.05-2.71), following adjustment for age, sex, clinical form and year. CONCLUSIONS: The pattern of meningococcal disease in the territory studied, during 1990-1997, was characterised by an increase in the incidence of serogroup C, particularly the emerging phenotype C:2b:P1.2,5. Together with this increase, a displacement of the incidence to higher age groups was observed, together with a tendency towards temporospatial aggregations of cases and an increase in the mortality associated with the new phenotype. This pattern is characteristic of epidemic situations of meningococcal disease caused by serogroup C.

    FUNDAMENTO: En el año 1997 (entre el 22 de septiembre y el 14 de noviembre) se efectuó una campaña de vacunación antimeningocócica A+C en la Comunidad de Madrid, en el grupo de edad de 18 meses a 19 años, ante el aumento del número de casos de enfermedad meningocócica por serogrupo C presentado en la temporada 1996-97. Este estudio forma parte de la evaluación de dicha campaña. MÉTODOS: Se ha valorado la evolución de la incidencia de la enfermedad meningocócica, mediante la comparación de tasas; y se ha determinado la efectividad de la vacunación al año (temporada 1997-98) y a los dos años (temporadas 1997-98 y 1998-99) de seguimiento. La efectividad vacunal se ha calculado como (1-(Tasa en vacunados/Tasa en no vacunados))*100. RESULTADOS: Se ha producido un descenso significativo en la incidencia de enfermedad meningocócica por serogrupo C al comparar las temporadas 1997-98 y 1998-99 con la temporada epidémica (1996-97). La efectividad vacunal a los dos años de seguimiento tras la campaña de vacunación ha sido de un 76,9% para la población global de 18 meses a 19 años y de un 88,5% en el grupo de vacunados entre 15 y 19 años. CONCLUSIONES: La efectividad vacunal obtenida es compatible con lo descrito en la literatura. La disminución significativa de la incidencia de enfermedad meningocócica por serogrupo C ha sido debida a la efectividad vacunal obtenida.

    BACGROUND: In 1997 (between 22 September and 14 November) an A+C meningococcal mass vaccination campaign was carried out in Madrid, targeting the age group of from 18 months to 19 years of age, in the face of an increase in the number of cases of meningococcal disease caused by serogroup C occurring in the 1996-97 season. This study forms a part of the impact assessment of that campaign. METHODS: The evolution of the meningococcal disease, by means of the comparison of rates of incidence; and the efficacy of the vaccination campaign was determined after one year (1997-98 season) and after two years (1997-98 and 1998-99 seasons) of monitoring. The vaccine efficacy has been calculated as [1-(Incidence rate in vaccinated /Incidence rate in unvaccinated)]*100. RESULTS: A significant drop was registered in the incidence of serogroup C meningococcal disease on comparing the 1997-98 and 1998-99 seasons with the epidemic season (1996-97). The vaccine efficacy after two years of monitoring subsequent to the vaccination campaign was 76.9% for the global population between 18 months and 19 years of age and 88.5% in the group of vaccinated individuals between 15 and 19 years of age. CONCLUSIONS: The vaccine efficacy obtained is compatible with that described in the relevant literature. The significant reduction in the incidence of meningococcal disease caused by serogroup C was due to the vaccine efficacy obtained.
  • Evaluación de la campaña de vacunación en Cantabria dos años de evolución epidemiológica Segunda Jornada sobre la Enfermedad Meningocócica en España - Estudios Generales

    Se presentan los resultados de la campaña de vacunación antimeningocócica realizada en Cantabria en febrero-marzo de 1997. A corto plazo, la campaña tuvo el efecto de disminuir la tasa de incidencia en el grupo de riesgo desde 21,33/100.000 en los 12 meses previos, a cero en los 12 meses posteriores. A medio plazo (2 años y medio después), la incidencia se ha mantenido un 80% más baja en el grupo de riesgo y un 77,3% más baja en el conjunto de la población. La efectividad vacunal en el tercer año tras la campaña se mantiene por encima del 91% en todas las edades, aunque algunos sesgos comentados en el trabajo hacen que esta efectividad esté infraestimada. Por otra parte, la efectividad vacunal es muy superior a los porcentajes de seroprotección medidos por la tasa de anticuerpos bactericidas, especialmente en las edades menores. Los casos clínicos en vacunados han tenido un curso clínico benigno.

    The results of the meningococcal vaccination campaign carried out in Cantabria in February-March 1997 are presented. In the short term, the campaign reduced the rate of incidence in the risk group from 21.33/100,000 in the previous 12 months, to zero in the 12 months following the campaign. In the medium term (2 and a half years later), the rate of incidence was maintained 80% lower in the risk group and 77.3% lower in the population overall. The vaccinal efficacy in the third year following the campaign remained above 91% for all age groups, although some biases commented upon in the study have meant that this efficacy has been underestimated. Also, the vaccinal efficacy is much higher than the percentages of seroprotection measured by the rate of bactericidal antibodies, particularly in the younger age groups. The clinical cases in vaccinated individuals experienced a benign clinical course.
  • Encuestas sobre tasas de portadores sanos de N. Meningitidis y caracterización de cepas circulantes Segunda Jornada sobre la Enfermedad Meningocócica en España - Estudios Especiales

    FUNDAMENTO: Durante la temporada epidémica 1.996-97 el incremento de incidencia de Enfermedad Meningocócica por N. Meningitidis serogrupo C llevó a instaurar programas de vacunación y realizar distintos estudios, entre ellos dos para conocer la prevalencia de portadores sanos que presentamos. MÉTODO: Portadores sanos en población general: encuesta transversal realizada entre marzo y mayo de 1.998, entre residentes en Extremadura. Tomamos dos grupos de edad: 0-19 años, vacunados (n=1.140) y 20 y más años (n=1.193), no vacunados. Portadores sanos en un acuartelamiento: encuesta transversal en una base militar durante mayo de 1.998 (n=619), tomando dos grupos: < 20 años (n=453) vacunados, y 20 y más años (n=166) no vacunados RESULTADOS: Portadores sanos en población general El porcentaje de portadores para todos los serogrupos fue: grupo de 0-19 años 798, grupo 20 y más años 332, total 460, para serogrupo B: 443, 225 y 284; para C: 004, 010 y 008 respectivamente. Las diferencias entre ambos grupos de edad del serogrupo B y total son estadísticamente significativas, así como la mayor prevalencia del B sobre C. El subtipo de las tres muestras con aislamiento de serogrupo C fue: una cepa NT:P1.6, una 2b:P1.2, y una no pudo sub-tiparse. Una cepa no serogrupable se subtipo como 2b: P1. 5. Portadores sanos en un acuartelamiento El porcentaje de portadores para todos los serogrupos fue: < 20 años 843, 20 y más años 684, total 727, para serogrupo B: 482, 375 y 404; y para C: 060, 000 y 016 respectivamente. Las mayores prevalencias obtenidas en militares sobre población general mostraron diferencias estadísticamente significativas. También la prevalencia de serogrupo B es significativamente muy superior a la C. La única cepa de C aislada se subtipó 2b : p1.2,5. CONCLUSIONES: La prevalencia de portadores sanos de N. meningitidis en población general es de 4,60 %, siendo mayor en el grupo de menor edad y el acuartelamiento. Muy escasa prevalencia del serogrupo C. Imposible concluir que cepa N. meningitidis serogrupo C circulaba la temporada 1.997/98.

    BACKGROUND: During the epidemic season of 1996-97, the increase in the incidence of Meningococcal Disease caused by N. Meningitidis serogroup C led to the initiation of vaccination programmes and to the performance of a number of studies, among which, two were designed to ascertain the prevalence of healthy carriers and are presented here. METHOD: Healthy carriers in the population at large: cross survey performed between March and May 1998, among residents in Extremadura. We took two age groups: 0-19 years of age, vaccinated (n=1,140) and 20 years of age and over (n=1,193), unvaccinated. Healthy carriers in a military barracks: cross survey on a military base in May 1998 (n=619), taking two groups: < 20 years of age (n=453) vaccinated, and 20 years of age and over (n=166) unvaccinated. RESULTS: Healthy carrier in the population at large. The percentage of carriers for all of the serogroups was: 0-19 years of age 7.98, 20 years of age and older 3.32, total 4.60, for serogroup B: 4.43, 2.25 and 2.84; for C: 0.04, 0.10 and 0.08 respectively. The differences between the two age groups in serogroup B and the total are statistically significant, as well as the greater prevalence of B over C. The subtype of the three samples with isolation of serogroup C was: one strain NT:P1.6, another strain 2b:P1.2, and one that could not be sub-typed. A strain that could not be classified into a serogroup was sub-typed as 2b: P1. 5. Healthy carriers in a military barracks. The percentage of carriers for all of the serogroups was: < 20 years 8.43, 20 years of age and over 6.84, total 7.27, for serogroup B: 4.82, 3.75 and 4.04; and for C: 0.60, 0.00 and 0.16 respectively. The higher prevalence obtained in military subjects in comparison with the population at large revealed statistically significant differences. Also, the prevalence of serogroup B was significantly higher than C. The only isolated C strain was sub-typed as: p1.2,5. CONCLUSIONS: The prevalence of healthy carriers of N. meningitidis in the population at large was 4.60 %, being higher in the lower age group and the barracks. Very low prevalence of serogroup C. Impossible to conclude that the strain N. meningitidis serogroup C was circulating during the 1.997/98 season.

    FUNDAMENTOS: Se plantea A) Conocer la tasa de portadores y los tipos circulantes de Neisseria Meningitidis en la población residente en el área de salud de Gran Canaria. B) Conocer el patrón de distribución de estos portadores. MÉTODOS: Se realizó un diseño descriptivo transversal, con un muestreo aleatorio en etapas múltiples y por conglomerados. Se determinó un tamaño muestral mínimo de 707 personas para una prevalencia esperada del 8,6 %, con una confianza del 95,6 % y precisión de 0,02. Asumiendo que un 15 % de las personas no quisieran colaborar, se incrementó el tamaño muestral a 831 personas, distribuidas en cada conglomerado de manera proporcional a la población existente. Este tamaño se distribuyó a su vez, en cuatro grandes grupos de edad y sexo, proporcionalmente a su importancia en cada zona básica de salud seleccionada aleatoriamente. Los individuos de la muestra se identificaban entre los que acudían a las unidades de extracción, y una vez superados los criterios de exclusión se les solicitaba su colaboración voluntaria en el estudio. Si aceptaban, se les cumplimentaba un cuestionario que englobaba diferentes variables de interés epidemiológico y se les realizaba un frotis faríngeo. Al haber seleccionado los equipos de Atención Primaria con muestreo aleatorio simple y seguir el mismo método para elegir los individuos dentro de ellos, la estimación de la prevalencia se realizó mediante estimador no sesgado. RESULTADOS: Se obtuvieron un total de 828 muestras, lo que supuso un 99,6% de las previstas. Salvo tres, todos los individuos seleccionados participaron voluntariamente en el estudio, lo que le confiere una alta representatividad. Todas las cepas obtenidas correspondían a N. Meningitidis Serogrupo B, salvo una identificada como N. Meningitidis Serogrupo C Sero/Subtipo 4:P1.2,5. Las cepas de N. Meningitidis serogrupo B identificadas, correspondían a 25 serosubtipos diferentes. La prevalencia puntual tras haber estudiado la muestra fue de 6,45 %, la varianza=0,0275 y el error estándar = 1,66. Podemos afirmar con una confianza del 95 %, que la prevalencia de portadores de N. Meningitidis en el área de salud de Gran Canaria, se estima entre el 3,2 % y el 9,7 %. CONCLUSIONES: Se comprueba un claro predominio de cepas N. Meningitidis serogrupo B entre los portadores. No aparecen diferencias estadísticamente significativas en la prevalencia observada entre los distintos grupos de edad, ni entre ambos sexos.

    BACKGROUND: A) To ascertain the rate of carriers and the Types of Neisseria Meningitidis circulating in the population resident in the health jurisdiction of Gran Canaria. B) to ascertain the pattern of distribution of such carriers. METHODS: A descriptive transversal design was made, with a random sampling in multiple stages and by conglomerates. A minimum sample size was determined at 707 individuals for an expected prevalence of 8.6 %, with a rate of reliability of 95.6 % and a precision of 0.02. Assuming that 15 % if the individuals would not be willing to co-operate, the sample size was increased to 831 individuals, distributed in each conglomerate in proportion to the existing population. This size was distributed in turn into four groups by age and sex, in proportion to their significance in each basic health care zone selected at random. The individuals in the sample were identified from among those who attended the blood extraction units, and after they had passed the criteria of exclusion, their co-operation was requested as volunteers in the study. If they accepted, a questionnaire was filled out with a number of variables of epidemiological interest and a pharyngeal smear was taken. Since the Primary Care units were selected on a simple random basis, and the same method was used to select the individuals within the units, the estimate of the prevalence was made by means of an unbiased estimator. RESULTS: A total of 828 samples were obtained, that is, 99.6% of the number foreseen. With the exception of three, all of the individuals selected participated voluntarily in the study, a circumstance rendering it highly representative. All of the strains obtained corresponded to N. Meningitidis Serogroup B, except for one identified as N. Meningitidis Serogroup C Sero/Subtype 4:P1.2,5. The strains of N. Meningitidis serogroup B identified corresponded to 25 different sero-subtypes. The prevalence determined after having studied the sample was 6,45 %, the variance = 0.0275 and the standard error = 1.66. We can conclude with a 95 % degree of reliability that the prevalence of carriers of N. Meningitidis in the Gran Canaria health jurisdiction is estimated to be between 3.2 % and 9.7 %. CONCLUSIONS: A clear predominance of N. Meningitidis serogroup B strains among carriers has been verified. There are no statistically significant differences in the prevalence observed among the different age groups nor between sexes.
  • Efectividad y duración de la inmunidad de la vacuna frente al meningococo serogrupo A y C Segunda Jornada sobre la Enfermedad Meningocócica en España - Estudios Especiales

    FUNDAMENTO: La Comunidad de Madrid detectó a partir de 1995 un incremento del número de casos de enfermedad meningocócica por serogrupo C. En 1997 se realizó una campaña de inmunización masiva sobre la población de 18 meses a 19 años. El objetivo de este estudio es conocer la respuesta inmunitaria producida por la vacuna y su relación con la edad. MÉTODOS: Se seleccionó una muestra de 1.003 niños vacunados durante la campaña. Se extrajo una muestra de sangre antes de la vacunación y tras uno, seis (solo <5a) y doce meses. Para valorar la respuesta inmune se midieron niveles de anticuerpos bactericidas y totales. RESULTADOS: La prevalencia de seroconversión medida por anticuerpos bactericidas es 89,6%. La respuesta es baja en menores de 3 años (34,8%), aumenta con la edad y a partir de los 7 años supera el 90%. A los 6 meses, la prevalencia de niveles protectores en menores de 5 años desciende notablemente (31,3%). Al año, la prevalencia desciende notablemente, especialmente en menores de 7 años. La proporción de individuos con respuesta de anticuerpos totales al mes supera el 90% y se mantiene elevada al año en todos los grupos edad (97,5%). CONCLUSIONES: La respuesta medida mediante anticuerpos totales entra en contradicción con la respuesta clínica a la vacunación y la medida mediante anticuerpos bactericidas infraestima la protección si se compara con los resultados de efectividad vacunal, por lo que es necesario buscar indicadores biológicos que se correlacionen de manera adecuada con la respuesta clínica tras la vacunación.

    BACKGROUND: In 1995 the Community of Madrid detected an increase in the number of cases of meningococcal disease caused by serogroup C. In 1997 a mass vaccination campaign was carried out in relation to the population between 18 months and 19 years of age. The purpose of this study is to ascertain the immune response produced by the vaccine and its relationship to the age of the subjects. METHODS: A sample group of 1,003 children vaccinated during the campaign was selected. A blood sample was extracted prior to vaccination and after one, six (only <5a) and twelve months had transpired. In order to assess the immune response, the levels of bactericidal and total antibodies were measured. RESULTS: The prevalence of seroconversion measured by bactericidal antibodies is 89.6%. The response is low in children under 3 (34.8%), increases with age and, from 7 years on, surpasses the 90% mark. After 6 months, the prevalence of protective levels in children under 5 years of age drops noticeably (31.3%). After one year, the prevalence drops significantly, particularly in children under 7 years of age. The proportion of individuals with total antibody response after one month is over 90%, and remains high after one year in all of the age groups (97.5%). CONCLUSIONS: The response measured by means of total antibodies contradicts the clinical response to the vaccination and the measurement by means of bactericidal antibodies underestimates the protection if it is compared with the results of vaccinal efficacy, for which reason, we need to search for biological indicators that would correlate adequately with the clinical response following immunisation.
  • Niveles de anticuerpos bactericidas frente a meningococo C tras la vacunación de niños de 2 a 6 años de edad en Andalucía Segunda Jornada sobre la Enfermedad Meningocócica en España - Estudios Especiales

    FUNDAMENTO: En 1997 el 18,5% de los casos de Enfermedad Meningocócica por serogrupo C en Andalucía fueron niños de 2 a 4 años de edad; edades donde respuesta inmune inicial y duración de la vacuna antimeningocócica de polisacárido capsular A+C, es menor que en edades superiores. Se diseñó una investigación para medir la respuesta inmune producida por esta vacuna, en niños de 2 a 6 años de edad, y compararla con la inmunidad natural presente en niños no vacunados. MÉTODOS: I.- Doble estudio de seguimiento: a) grupos de niños previamente vacunados y grupos control, b) grupo de niños que iba a ser vacunado, para análisis pre y postvacunal (1, 6 y 12 meses) y grupo control. II.- La actividad bactericida se midió según protocolo estandarizado del CDC frente a cepa de N. meningítidis C-11. Los sueros con título de actividad bactericida (TAB) > o = 1:8 se consideraron protectores. RESULTADOS: Al mes y a los 2 meses de vacunarse la proporción de TAB > o = 1:8 era significativamente superior a la del grupo control (65,5% y 73,9% frente a 2,2% y 12%. En el prevacunal y en el postvacunal a los 6, 7, 12 y 13 meses no se observó diferencia significativa entre vacunados y controles. CONCLUSIONES: Las diferencias entre vacunados y no vacunados 1 y 2 meses después de la vacunación indican seroconversión en los vacunados. Para el grupo de edad de 2 a 6 años la actividad bactericida adquirida declina rápidamente, ya que a los 6 meses dejan de observarse diferencias con el grupo control.

    BACKGROUND: In 1997, 18.5% of the cases of Meningococcal Disease caused by serogroup C in Andalusia were children between 2 and 4 years of age; ages where the initial immune response and the duration of the capsular A+C meningococcal polyssacharide vaccine is less than in older age groups. Research was designed in order to measure the immune response produced by this vaccine in children from 2 to 6 years of age and to compare it with the natural immunity present in unvaccinated children. METHODS: I. Dual monitoring study: a) groups of children vaccinated previously and control groups, b) groups of children who were going to be vaccinated, for pre- and post-vaccination (1, 6 and 12 months) analysis and a control group. II. The bactericidal activity was measured according to the standardised protocol of the CDC with regard to the strain of N. meningitidis C-11. The sera with bactericidal activity (TAB) > 1:8 were considered to be protective.. RESULTS: 1 and 2 months following vaccination, the proportion of TAB > 1:8 was significantly higher than that of the control group (65.6% and 73% in comparison to 2.2% and 12%). In the pre-vaccine and post-vaccine (after 6, 7, 12 and 13 months) verification, no significant difference between vaccinated individuals and controls was observed. CONCLUSIONS: The differences between vaccinated and unvaccinated individuals 1 and 2 months following vaccination indicate seroconversion in the vaccinated individuals. For the age group of between 2 to 6 years of age, the bactericidal activity acquired decline quickly, as, after 6 months, differences between this group and the control group are no longer observed.

    BACKGROUND: El aumento de incidencia de enfermedad meningocócica en la Región de Murcia en la temporada 1996/97 motivó que la Dirección General de Salud desarrollara en septiembre-octubre de 1997 una Campaña de Vacunación frente al meningococo serogrupo C. El objetivo de este trabajo fue conocer el porcentaje de niños menores de 5 años de edad que mostraban seroconversión postvacunal al mes de la vacunación y el porcentaje de los mismos que conservaban inmunidad al año de la vacunación. MÉTODO: Estudio de seguimiento de 296 niños entre 18 y 59 meses de edad. Las determinaciones serológicas ser realizaron antes de la vacunación, al mes y al año de la vacunación. La titulación de anticuerpos se determinó según el "ensayo bactericida" de los Centers for Disease Control. RESULTADOS: De los 296 niños estudiados únicamente 11 (3,7%) mostraron títulos de anticuerpos bactericidas antes de la vacunación. Al mes, de los niños que no mostraban anticuerpos antes de la vacunación, 167 (63,7%) seroconvirtieron. Se observó una tendencia lineal estadísticamente significativa (p<0.001) de aumento del porcentaje de seroconversión con la edad de vacunación. Al año de la vacunación, de entre los niños seroconvertidos al mes de la vacunación, únicamente 6 (4,3%) mostraban anticuerpos bactericidas. CONCLUSIÓN: El porcentaje de seroconversión en menores de 5 años de edad, tras la administración de la vacuna de polisacárido capsular C, presentó un claro incremento con la edad. La seroprotección adquirida en niños vacunados declina rápidamente en el año siguiente a la vacunación.

    BACKGROUND: An increase in the incidence of meningococcal disease in the Region of Murcia during the 1996/97 season led the General Directorate for Health to carry out a Vaccination Campaign against the serogroup C meningococcus in September-October 1997. The purpose of this study was to ascertain the percentage of children under 5 years of age who ration, and the percentage who retained immunity after one year. METHOD: The monitoring of 296 children between 18 and 59 months of age. The serological determinations were performed prior to vaccination, one month following vaccination and after one year. The antibody titres were determined according to a "bactericidal test" in the Center for Disease Control. RESULTS: Of the 296 children studied, only 11 (3.7%) revealed bactericidal antibody titres prior to vaccination. After one month, of the children who did not show antibodies prior to vaccination, 167 (63.7%) seroconverted. A statistically significant linear tendency (p<0.001) of increase in the percentage of seroconversion in relation to the vaccination age was observed. One year after vaccination, from among the children seroconverted one month following vaccination, only 6 (4.3%) revealed bactericidal antibodies. CONCLUSION: The percentage of seroconversion in children under 5 years of age, following administration of the capsular polyssacharide C vaccine, was seen to clearly increase with the age of the subjects. The seroprotection acquired in vaccinated children declined quickly during the year following vaccination.
