    Fundamento: La evaluación del proceso es condición imprescindible para medir correctamente el impacto de intervenciones educativas sobre las conductas, sus determinantes psicosociales y el estadio de cambio, en el marco de la promoción de la salud. El objetivo de este trabajo es evaluar la calidad de implementación del Programa FAPACAN, diseñado para prevenir el riesgo conductual de cáncer en Atención Primaria y para mejorar sus determinantes psicosociales en el Modelo A.S.E. y el estadio de cambio según Prochaska y DiClemente. Métodos: Mediante visita al centro de salud y la cumplimentación de una lista de comprobación in situ y mediante una encuesta telefónica a los individuos que formaban la muestra, se midió la calidad de implementación. Se hallaron medidas de centralización y de asociación (coeficiente de Pearson y de Spearman). Se obtuvo un modelo de regresión múltiple con la puntuación obtenida por el entrevistado (rango de 0 a 8) y las covariables: género, edad, nivel de estudios, localidad e historia familiar de cáncer. Resultados: Las puntuaciones de calidad obtenidas oscilan entre el 72% y el 81% de la calidad óptima. Se encontraron diferencias significativas debidas al administrador (mejor cuanto menos años de ejercicio) y a la persona encuestada (mejor cuanto mayor nivel de estudios). Conclusiones: En conjunto, la calidad de implementación fue más que suficiente, a pesar de la escasa facilitación proporcionada por el sistema sanitario.

    Background: The evaluation of the process is an essential condition to correctly measure the impact of educational interventions on behaviour, its psychosocial determinants and the state of change, in the context of health promotion. The aim was to evaluate the quality of the implementation of the FAPACAN Programme, designed to prevent behavioural risk of cancer in Primary Care, and to improve its psychosocial determinants in the A.S.E. Model and the state of change according to Prochaska and DiClemente Theory. Methods: The quality of implementation was measured by means of a visit to the health centre, by filling in a checklist 'in situ', and a phone survey with the patient. Centralisation and association measures were found ( Pearson and Spearman's coefficient). A multiple regression model was obtained with the score made by the patient (range of 0 to 8) and the covariables: gender, age, level of education, locality and family history of cancer. Results: The quality scores obtained oscillate between 72% and 81% of optimum quality. Significant differences were found owing to the administrator (better with fewer years of exercise) and the patient (better with higher level of study). Conclusions: In general, the quality of implementation was more than sufficient, in spite of the poor provision by the health system.
  • Intención de compra de medicamentos genéricos por parte de los usuarios de Asturias Originales

    Fundamento: Conocer las percepciones de los consumidores acerca del riesgo asociado al uso de medicamentos genéricos y los factores que más influyen en la intención de solicitar un genérico al médico (prescriptor) y/o al farmacéutico, a fin de determinar posibles barreras o frenos a la aceptación de los mismos y obtener información que apoye la toma de decisiones de los gestores sanitarios. Métodos: Estudio sobre utilización de medicamentos centrado en la disposición de los pacientes a solicitar una EFG. En esta investigación transversal cuantitativa se entrevistó personalmente a 542 individuos, a la salida de un centro de salud o de un establecimiento de farmacia en Asturias. En el cuestionario se incluía una escala de medición del riesgo percibido en la compra de un medicamento con 15 atributos agrupados en cinco dimensiones. Asimismo se recogió información sobre la intención de consumir medicamentos genéricos y sobre las características demográficas y socioeconómicas de los entrevistados. Para el análisis de los resultados se aplicaron un análisis factorial confirmatorio, regresión múltiple y análisis univariable. El tratamiento de los datos se efectuó con los programas estadísticos EQS y SPSS. Resultados: Percepción media del riesgo (escalas de 1 a 7): funcional: 2,75; físico: 2,68; financiero: 2,19; psicológico: 1,99; social: 1,42. Factores influyentes sobre la intención de solicitar genéricos al médico: riesgo psicológico (p=0,000). Sobre la solicitud al farmacéutico: riesgo psicológico (p=0,000) y riesgo social (p=0,020). Conclusiones: Los agentes interesados en el desarrollo en el mercado de las EFG deben mantener sus esfuerzos de comunicación hacia la equiparación de los aspectos funcionales y financieros entre especialidades del fabricante y especialidades genéricas, pero no deben dejar de lado aspectos psicológicos y sociales del comportamiento de compra del consumidor.

    Background: Ascertaining how consumers perceive the risk related to the use of generic prescription drugs and those factors which have the greatest impact on the intention to request a generic drug from the prescribing physician and/or the pharmacist for the purpose of determining any possible barriers or hindrances to the acceptance of generics and to gather information to aid healthcare managers in their decision-making processes. Methods: Study on prescription drug use revolving around the degree to which patients are willing to request an EFG. In this quantitative transversal study, a total of 542 individuals were individually surveyed upon exiting a healthcare center or pharmacy in Asturias. A scale for measuring the perceived risk involved in the purchase of a prescription drug including 15 attributes grouped into five aspects was included in the questionnaire. Information was also gathered regarding the intention of using generic prescription drugs and on the demographic and socioeconomic characteristics of those surveyed. For the analysis of the results, a factorial confirmational analysis, multiple regression and univariate analysis were used. The data was processed using the EQS and SPSS statistics programs. Results: Mean perception of the risk (scales 1-7): functional: 2.75; physical: 2.68; financial: 2.19; psychological: 1.99; social: 1.42. Factors having a bearing on the intention of requesting generic prescription drugs from their physician: psychological risk (p=0.000). On requesting the same from their pharmacist: psychological risk (p=0.000) and social risk (p=0.020). Conclusions: The agents interested in the development on the EFG market should target their communication efforts on putting the functional and financial aspects of the manufacturer's specialties and generic specialties on the same level, but should not leave out psychological and social aspects of the consumers' purchasing behavior.
  • Aplicación del método captura-recaptura en la evaluación del sistema de vigilancia epidemiológica de la enfermedad meningocócica en Tenerife (1999-2001) Originales

    Fundamento: La vigilancia de la Enfermedad Meningocócica en Canarias es fundamentalmente pasiva, sirviéndose del circuito habitual de las Enfermedades de Declaración Obligatoria. El objetivo de este trabajo incluye describir los atributos cualitativos y cuantitativos del sistema y evaluar la exhaustividad mediante el sistema de captura-recaptura de tres fuentes de información. Métodos: El estudio abarca los años 1999-2001 en Tenerife. La información se obtuvo de tres fuentes: el Sistema de las Enfermedades de Declaración Obligatoria, los Laboratorios de Microbiología y el Conjunto Mínimo Básico de Datos hospitalario. Se aplicaron los Protocolos de Evaluación de un Sistema de Vigilancia de los Centers of Disease Control and Prevention, de Atlanta. Para la estimación del número de casos se usó un modelo log-linear. Los cálculos de la exhaustividad y los intervalos de confianza al 95% se realizaron en el paquete estadístico SPSS10. Resultados: La sensibilidad del sistema resultó ser del 84,9% y el valor predictivo positivo del 80,4%. El retraso en la notificación (oportunidad) osciló entre los 0,5 y 13 días, con una mediana de 3 días. La aceptabilidad global del sistema ha sido del 76,64%. El valor de la exhaustividad fue del 98,1%. Conclusiones: La vigilancia de esta enfermedad es buena, con una sensibilidad que hablaría de un buen nivel de declaración, reafirmada por la exhaustividad. Aunque el valor predictivo positivo es alto, podría apuntar hacia la rápida instauración de tratamiento antibiótico que impediría la confirmación microbiológica. El sistema es oportuno, permitiendo la rápida adopción de medidas de intervención.

    Background: Meningococcal Disease is mainly monitored passively on the Canary Islands, the regular Compulsory Disease Notification channels being used. The objective of this study includes describing the qualitative and quantitative aspects of this system and evaluating the exhaustiveness, by means of the capture-recapture system, of three information sources. Methods: This study covers the 1999-2001 period in Tenerife. The information was gathered from three sources: the Compulsory Disease Notification System, the Microbiology Laboratories and the hospital Minimum Basic Data Set. The Evaluation Protocols of the Monitoring System of the Atlanta Centers of Disease Control and Prevention were used. A log-linear model was used for estimating the number of cases. The calculations of the exhaustiveness and the 95% confidence intervals were done in the SPSS10 statistics package. Results: The system was found to have an 84.9% sensitivity, and an 80.4% positive predictive value. The delay in notification (timeliness) fell within the 0.5 - 13-day range, averaging 3 days. The system was found to have a 76.6% overall acceptability. The exhaustiveness value was 98.1%. Conclusions: This disease is being monitored well, with a degree of sensitivity which would be revealing of a good notification level, also confirmed by its exhaustiveness. Although the positive predictive value is high, this could be indicative of the expeditious starting of antibiotic treatment which would hinder microbiological confirmation. The system is timely, affording the possibility of measures being taken for fast intervention.
  • Diagnósticos al alta hospitalaria de las personas inmigrantes en la ciudad de Valencia (2001-2002) Originales

    Fundamento: La inmigración en España se ha incrementado de forma notable en la última década. Por razones teóricas se ha planteado que esta población pudiera ser responsable de un aumento de la incidencia y/o transmisión de enfermedades infecciosas. Sin embargo, existen escasos estudios acerca de las enfermedades de este colectivo que permitan conocer la importancia de esta afirmación. El objetivo de este estudio es cuantificar la atención hospitalaria a esta población en la ciudad de Valencia. Métodos: Se obtuvieron los registros de altas de los Servicios de Admisión de los hospitales públicos de la ciudad de Valencia. Se desarrolló un estudio observacional retrospectivo para el año comprendido entre el 1 de octubre de 2001 al 30 de septiembre del 2002. Resultados: Recuperamos 8.444 ingresos que identificaron 1.577 ingresos de inmigrantes. Se evidenció una proporción similar de inmigrantes registrados con documentación reglada (841, 51,16%) e inmigrantes supuestamente no regularizados (803, 48,84%). Predominaron las mujeres (68,3%) frente a los varones (31,7%) y una media de edad inferior a los 30 años. El diagnóstico de alta más frecuente fue «complicaciones del embarazo, parto y puerperio» con 37,7% del total. Los «traumatismos y envenenamientos», «enfermedades del aparato digestivo» y «enfermedades del aparato respiratorio» representaron el 12,9%, el 7,8% y el 5,4%, respectivamente. Las «enfermedades infecciosas y parasitarias» supusieron el 4,4%. Conclusiones: La proporción de enfermedades infecciosas e infecto-contagiosas observadas no contribuyen a alentar las opiniones difundidas sobre importación o reemergencia de enfermedades desde los colectivos de inmigrantes. La inmigración masiva acaecida requiere múltiples adaptaciones del Sistema de Salud que permitan establecer el perfil de salud de este colectivo, basado en estimaciones poblacionales.

    Background: Immigration has increased remarkably in Spain over the past ten years. For theoretical reasons, the question has been raised as to whether this population may possibly be responsible for a rise in the incidence and/or transmission of infectious diseases. However, very few studies have been conducted regarding the diseases among this group which would afford the possibility of ascertaining the importance of this statement. This study is aimed at quantifying the hospital care provided to this population in the city of Valencia. Methods: The hospital release records were obtained from the Admissions Departments of the public hospitals in the city of Valencia. A one-year retrospective observational study was conducted throughout the October 1, 2001 - September 30, 2002 period. Results: We retrieved 8,444 hospital admissions identifying 1,577 hospital admissions of immigrants. Similar percentages of immigrants recorded with their documentation in proper order (841; 51.16%) and immigrants supposedly undocumented (803; 48.84%) were found. Females (68.3%) were predominant over males (31.7%), and a mean age of under 30 years of age. The most frequent diagnosis on release was «pregnancy, childbirth and post-partum-related complications», totaling 37.7% of all releases, others involving. «injuries and poisoning» (12.9%), «digestive tract diseases» (7.8%) and «respiratory tract diseases» (5.4%), «Infectious and parasitic diseases» totaled 4.4%. Conclusions: The percentage of infectious and infectious-contagious diseases found did not contribute to encouraging the opinions disseminated regarding the importing or re-emergence of diseases from the immigrant groups. The en masse immigration which has taken place requires many adaptations of the Healthcare System to afford the possibility of setting out the health profile for this group based on population estimates.
  • Evolución del consumo de fármacos antipsicóticos en Castilla y León (1990-2001) Originales

    Fundamento: A lo largo de la última década se han introducido nuevos fármacos y nuevos abordajes terapéuticos que permiten suponer cambios en la utilización de antipsicóticos en nuestro medio. El objetivo del presente estudio fue caracterizar el patrón de utilización de antipsicóticos en Castilla y León para el periodo 1990-2001 así como conocer la influencia que la introducción de los nuevos antipsicóticos ha podido tener en el patrón de consumo de estos fármacos. Métodos: Los datos de consumo de medicamentos se obtuvieron de la base de datos ECOM (Especialidades Consumo de Medicamentos) del Ministerio de Sanidad y Consumo. Esta base contiene información sobre el consumo de medicamentos dispensados con cargo al Sistema Nacional de Salud en farmacias comunitarias en todo el territorio nacional. Con el fin de estimar el consumo fuera del Sistema Nacional de Salud, se han utilizado datos de la empresa IMS (International Marketing Services) referidos a los años 2000 y 2001. Los datos se expresaron en Dosis Diarias Definidas por 1.000 habitantes y día. Resultados: El uso de antipsicóticos creció un 146 % desde 1990 al 2001. A lo largo del periodo estudiado, haloperidol ha sido el antipsicótico más utilizado en España y en Castilla y León. Los antipsicóticos atípicos representaron un 49% del consumo total en el año 2001 y un 90% de los costes; se observa una fuerte tendencia hacia un aumento de su consumo en detrimento de los antipsicóticos típicos. Se ha estimado que un 14% de los antipsicóticos utilizados en Castilla y León lo fue fuera del Sistema Nacional de Salud. Existen notables diferencias entre las distintas provincias. Conclusiones: El consumo de antipsicóticos en Castilla y León creció un 146% durante los 12 años estudiados. En este incremento, la oferta de nuevos antipsicóticos atípicos y las medidas legales relacionadas con la desinstitucionalización de los enfermos mentales han podido jugar un papel importante. La introducción en el mercado de los nuevos antipsicóticos ha modificado el patrón de uso de los mismos y ha ocasionado un incremento en los costes directos. El consumo sin cargo al Sistema Nacional de Salud de estos medicamentos es pequeño, pero no despreciable.

    Background: Over the past ten years, new drugs and new approaches to treatment have been implemented making it possible to assume changes in the use of antipsychotic drugs in our environment. This study is aimed at characterizing the pattern of use of antipsychotic drugs in Castile and Leon throughout the 1990-2001 period as well as ascertaining the bearing which the marketing of new antipsychotic drugs may have had on the pattern of consumption of these drugs. Methods: The drug consumption data was obtained from the Ministry of Health and Consumer Affairs' consumption database ECOM (Especialidades Consumo de Medicamentos). This database contains information on the consumption of medications dispensed charged to the Social Insurance system in community pharmacies nationwide. To estimate the consumption outside of the National Health System, data from the IMS (International Marketing Services) firm for the years 2000 and 2001 was used. The data was given in Defined Daily Doses/1000 inhabitants/day. Results: The use of antipsychotic drugs rose by 146% within the 1990-2001 period. Throughout the period studied, haloperidol was the antipsychotic drug most used in Spain and in Castile and Leon. The atypical antipsychotic drugs totaled 49% of the total consumption for 2001 and 90% of the costs, a strong trend being found toward an increase in the consumption of these atypical antipsychotic drugs in detriment to the typical antipsychotic drugs. It has been estimated that 14% of the antipsychotic drugs used in Castile and Leon were used outside of the National Health System.. Appreciable differences exist among the different provinces. Conclusions: The consumption of antipsychotic drugs in Castile and Leon grew by 146% throughout the twelve months studied. The marketing of new atypical antipsychotic drugs and the legal measures related to the deinstitutionalizaton of mental patients may have played a major role in this increase. The marketing of the new antipsychotic drugs has led to a change in their pattern of use and has give rise to an rise in the direct costs. The consumption of these medications not charged to the National Health System is minor, but not negligible.
  • Perfil de la demanda urgente e influencia del fútbol televisado en un servicio extrahospitalario en la Zona Básica de Salud de Tafalla, Navarra Originales

    Fundamento: La demanda a los servicios de urgencias, tanto hospitalarios como extrahospitalarios, mantiene un ritmo creciente. El objetivo del presente trabajo es doble: cuantificación y estudio de las características personales de los usuarios demandantes de atención urgente y valorar si el fútbol televisado influye en la utilización del servicio de urgencias. Métodos: Estudio descriptivo longitudinal (9.723 usuarios demandantes) y estudio de casos y controles (1.284 usuarios demandantes) según presencia o no de fútbol televisado mediante modelo de regresión logística. Las asociaciones se han cuantificado mediante la odds ratio (OR). Se excluyen del estudio las consultas telefónicas y las de enfermería. Resultados: El 10,6% de la demanda ha correspondido a consultas en domicilio y se han remitido al hospital el 4,8% del total. El 13,3% de la demanda corresponde a personas desplazadas de otras zonas de salud y son 65 los usuarios hiperfrecuentadores con 8 o más consultas. El mes de agosto (32,3%), el domingo (44,56%) y el grupo horario de 12 a 14 horas (8,38%) son los momento de mayor demanda de atención, siendo las diferencias estadísticamente significativas. El fútbol televisado se asoció a un aumento de la demanda del 19,8% (p<0,001) respecto al periodo control. Conclusiones: Se observa una alta frecuentación a los servicios de urgencia extrahospitalaria, con una importante concentración de la demanda en momentos muy concretos. Es muy importante la atención prestada a personas desplazadas. La retransmisión de fútbol televisado se asocia con una mayor utilización del servicio de urgencias.

    Background: The demand placed on both hospital as well as extrahospital emergency care units currently continues to increase at a growing rate. This study has a twofold objective: the quantification and study of the personal characteristics of the users who are demanding emergency care and assessing whether televised soccer games have any bearing on the utilization of emergency care services. Methods: A longitudinal descriptive study (9,723 users demanding care) and study of cases and controls (1,284 users demanding care) according to whether or not a soccer game was being televised by means of a logistic regression model. The associations were quantified by means of the odds ratio (OR). Those dealt with by telephone or in infirmaries were not included in the study. Results: A total 10.6% of the demand involved home visits, 4.8% of this total having been sent to hospital. A total 13.3% of the demand corresponds to individuals visiting from other healthcare districts, a total of 65 being overusers of emergency care (8 or more visits). The month of August (32.3%), Sundays (44.56%) and the 12:00 p.m.-2:00 p.m. timeframe (8.38%) are the times when the greatest demand for care occurs, the differences being statistically significant. Televised soccer games were associated to a 19.8% (p< 0.001) rise in demand as compared to the control period. Conclusions: A high degree of use of the extrahospital emergency care units has been found to exist, a major part of the demand being concentrated at highly specific points in time. A major degree of care is provided to those visiting from outside their own healthcare districts. Television soccer game broadcasts is associated with the greater utilization of the emergency care services.
  • Estudio microbiológico de las comidas servidas en los comedores escolares de la isla de Tenerife Originales

    Fundamento: Valorar la calidad higiénico-sanitaria de las comidas servidas en comedores escolares con la finalidad de conocer si es adecuada o por el contrario su ingesta puede representar un grave problema de salud para este colectivo de alto riesgo. Métodos: Estudio epidemiológico descriptivo transversal. Se analizan 898 muestras de alimentos recogidos en comedores de 101 colegios de Tenerife, seleccionados por un muestreo probabilístico aleatorio estratificado, 58 con elaboración propia de los alimentos (gestión directa) y 43 con comidas servidas por un catering (elaboración contratada). Resultados: En ninguna de las muestras analizadas se aislaron los patógenos Salmonella spp. y Listeria monocytogenes. El 79% de los alimentos estudiados presentó recuentos para este parámetro, (91%) de ensaladas y (85%) de segundos platos. Para Enterobacteriaceae totales, el 15% de las muestras analizadas fueron positivas. Escherichia coli se aisló en el 24% de las ensaladas, el 4% de los complementos y el 1% de los segundos platos y Staphylococcus aureus se aisló en tres alimentos. Los recuentos más elevados se obtuvieron para los microorganismos aerobios mesófilos totales. Del total de muestras analizadas un 8,24% de las mismas superaron uno o más de los límites establecidos para los parámetros estudiados. Conclusiones: La calidad microbiológica de las comidas servidas en los comedores escolares es aceptable, si bien al existir un porcentaje de alimentos que superan los límites establecidos en microorganismos indicadores y testigos de falta de higiene y al ser los escolares un colectivo de alto riesgo, será necesaria la revisión de la vigilancia en los puntos de control crítico.

    Background: School lunchrooms and catered meals are of major importance from the Public Health standpoint. This study is aimed at evaluating the microbiological quality of the meals served in school lunchrooms for the purpose of ascertaining whether it is suitable or, to the contrary, the intake thereof may involve a serious health problem for this high-risk group. Methods: A transversal descriptive epidemiological study. An analysis was conducted of a total of 898 food samples collected from the lunchrooms at 101 schools in Tenerife, selected by a stratified random probabilistic sampling procedure, fifty-eight of which were prepared at the school proper (direct management) and 43 involving meals served by a catering firm (prepared under contract). Results: No disease-causing Salmonella spp. or Listeria monocytogenes bacteria were isolated from any of the samples. A total 79% of the foods studies showed counts for this parameter, (91%) in salads and (85%) in main courses. A total 15% of the samples analyzed tested positive for total Enterobacteriaceae. Escherichia coli was isolated in 24% of the salads, in 4% of the side dishes and in 1% of the main dishes, Staphylococcus aureus having in isolated in three foods. The highest counts were found for the total aerobic mesophyllic microorganisms. A total 8.24% of the samples analyzed exceeded one or more of the limits stipulated for the parameters studies. Conclusions: The microbiological quality of the meals served in these school lunchrooms is acceptable, although due to a certain percentage of the foods having exceeded the stipulated limits for microorganisms indicative of and revealing a lack of hygiene, and schoolchildren being a high-risk group, a revision of the surveillance related to critical checkpoints will be necessary.
