  • Hospitalizaciones evitables por cirugía sin ingreso en la Comunidad Valenciana: un estudio Delphi Originales

    FUNDAMENTOS: El objetivo de este trabajo de investigación ha sido estimar, desde el punto de vista de los cirujanos y anestesistas que trabajan actualmente en el Sistema Nacional de Salud, el volumen de hospitalizaciones evitables por la utilización de la cirugía sin ingreso (CSI). MÉTODO: Mediante un proceso Delphi en el que participaron 25 médicos, cirujanos o anestesistas, del Servei Valencià de la Salut (SVS), se obtuvo un consenso sobre los criterios objetivos de exclusión de pacientes para CSI y sobre la proporción de determinadas intervenciones que podrían realizarse ambulatoriamente, criterios que fueron aplicados al Conjunto Mínimo de Datos Básicos (CMBD) al alta hospitalaria de la Comunidad Valenciana para estimar los ingresos y estancias evitables por CSI en una selección de 29 procesos quirúrgicos. RESULTADOS: El 83% de los médicos respondieron a los cuestionarios Delphi. La mediana de la estimación de los encuestados sobre la proporción de casos realizables con CSI en las diferentes patologías oscila entre el 40% para las reparaciones de hernia de pared abdominal o la colecistectomía laparoscópica, y el 90% para las escisiones de tejido perianal, anastomosis para diálisis o gangliectomía. La aplicación de las restricciones a la CSI definidas en el Delphi hubiera supuesto para el SVS la evitación de 12.558 ingresos en 1994, el 75% de los realizados en las intervenciones estudiadas, porcentaje casi idéntico al resultante de aplicar las estimaciones de los clínicos al CMBD. CONCLUSIONES: Los profesionales del SVS consideran factible un número de intervenciones sin ingreso muy superior al efectivamente realizado, siendo probablemente los sistemas de financiación e incentivos utilizados en el SNS, y no los criterios clínicos, las causas del escaso desarrollo de la CSI.

    BACKGROUND: To estimate the number of hospital admittances avoidable by means of ambulatory surgery, according to surgeons and anaesthetists who currently work with the National Health System, METHOD: By means of a Delphi procedure of 25 surgeons or anaesthetists of the Valencia Health Service (Servei Valencià de la Salut - SVS), a consensus was reached as to the objective criteria for excluding patients that could be treated outside a hospital and the percentage of certain operations that could be performed under day surgery. These criteria were applied to the Minimum Basic Data Set (MBDS) on hospital discharge within the Autonomous Region of Valencia in order to estimate the figure of hospital admittance and stays avoidable by means of ambulatory surgery in a selection of 29 surgery processes. RESULTS: 83% of medical personnel responded to the Delphi questionnaires. The median of the surveyed groups estimation on the proportion of cases that could be treated by means of external surgery varied from 40% for abdomen wall hernias or laparoscopical colecistectomy and 90% for perianal tissue excision, anastomosis for dialysis or gangliectomy. Application of the results of the Delphi survey would have meant that the SVS could have avoided 12,558 admittances in 1994, 75% of the operations surveyed, which is almost the same figure resulting from applying the medical personnels opinion of the CMBD. CONCLUSIONS: SVS professionals consider it feasible to perform a much higher number of ambulatory operations (without hospital admission) than those currently carried out, and that it is likely that the causes for the lack of such operations lie in SNS financing and incentive criteria, rather than medical issues.
  • Epidemiología de la fractura osteoporótica de cadera en la provincia de Zamora (1.993) Originales

    FUNDAMENTOS: Conocer los principales aspectos epidemiologicos de la fractura osteoporótica de cadera (FOC) en la provincia de Zamora en 1993, y las tasas de incidencia acumulada cruda y estandarizada respecto a la población española. PACIENTES Y MÉTODOS: Se estudian 215 personas portadoras de FOC en nuestra provincia durante 1993. De ellos, 203 fueron ingresados y, de éstos, 188 se intervinieron quirúrgicamente por parte del servicio de traumatología. Las variables estudiadas fueron: Sexo, edad, ámbito (rural, urbano), tipo de fractura (cervical, trocantérea), procedimiento quirúrgico (prótesis parcial, tallos de Ender, otros), estacionalidad y estancia media hospitalaria. RESULTADOS: La incidencia acumulada global resultó de 100,6/100.000 habitantes. En los mayores de 50 años la incidencia fue de 246,5/100.000 habitantes (según censo de 1991). Existió un predominio femenino (3,78:1). Las personas con FOC tenían una edad media 81,3± 8,8 años. Reparto al 50% del tipo de fractura tanto a nivel global como para el sexo femenino. La incidencia de fractura trocantérea fue el doble el sexo masculino que en el femenino. La estancia media en los pacientes ingresados fue de 15,35 días. La incidencia acumulada estandarizada por edad y sexo para la población española de 1991 sería de 202,99 fracturas por 100.000 habitantes/año, lo que supondría un número total de fracturas de 23.740 fracturas/año. CONCLUSIONES: La provincia de Zamora tiene una de las mayores incidencias de FOC comunicadas hasta la fecha en España, tanto cruda como específica para mayores de 50 años. La media de edad, el predominio del sexo femenino, la distribución del tipo de fractura y la estancia media son similares a las publicadas en nuestro país.

    BACKGROUND: To identify epidemiological aspects of the osteoporotic hip fractures (FOC) in the province of Zamora in 1993, and the basic and standardised accumulative incidence rates amongst the Spanish population. METHODS: 215 FOC cases were studied in the province throughout 1993. 203 were admitted to hospital, 188 of which were treated surgically by the Traumatology Department. The surveyed variables were: sex, age, location (rural, urban), type of (cervical, trochantereal) fracture, surgical procedure (partial prosthesis, Ender stems, others), seasonal factors and average hospital stay. RESULTS: Global accumulative incidence: 100.6 per 100,000 inhabitants. With respect to people over 50, the rate was 246.5 per 100,000 inhabitants (according to the 1991 census). Female predominance (3.78:1). Average age: 81.3?8.8 years. 50% distribution of fracture type, both at global and female level. Dual incidence of trochantereal fracture suffered by males. Average stay: 15.35 days. Standardised accumulative incidence according to age and sex for the Spanish population in 1991 would be 202.99 fractures per 100,000 inhabitants and year, which would result in a total number of 23,740 fractures per year. CONCLUSIONS: The province of Zamora currently has one of the highest rates of FOC in Spain, both with respect to overall population as well as people over 50 years of age. Average age, predominance of the female sex, distribution of the type of fracture and average hospital stay are similar to the figures published in our country.
  • Factores asociados a los accidentes por exposición percutánea en personal de enfermería en un hospital de tercer nivel Originales

    FUNDAMENTO: Los accidentes por exposición percutánea suponen aproximadamente un tercio de los accidentes laborales del personal sanitario hospitalario. Su importancia estriba en las enfermedades causadas por patógenos transmisibles por esta vía (hepatitis B, hepatitis C, infección VIH). Objetivos: describir los accidentes de este tipo notificados en un hospital de tercer nivel; identificar factores asociados a estos accidentes en personal de enfermería; construir un modelo predictivo del riesgo individual de accidentarse. MÉTODOS: Estudio descriptivo de una cohorte retrospectiva compuesta por todas las personas que notificaron un accidente entre el 1-1-93 y el 30-6-96. Estudio de casos y controles en el personal de enfermería durante el período 1-1-95 al 30-6-96, analizado mediante regresión logística múltiple. RESULTADOS: La incidencia acumulada de accidentes en un año fue de 0,078 para las/-os enfermeras/-os. En el 57,3% de los casos estuvieron implicadas agujas de jeringas desechables o precargadas. La incidencia acumulada en un año fue mayor para las agujas de cateterismo intravenoso (8,5 por 100.000). El riesgo de accidentabilidad, ajustado por variables confundentes, fue mayor para las/-os enfermeras/-os (OR=3,22; I.C.95%=1,96-5,27), para los trabajadores de la Unidad de Hemodiálisis (OR=35,21; I.C.95%=3,74-331,16) y para aquéllos con contrato eventual (OR=4,50; I.C.95%=2,24-9,04). CONCLUSIONES: Los accidentes por exposición percutánea en este hospital son más frecuentes entre el personal de enfermería y se producen, fundamentalmente, con algún tipo de aguja hueca. Se han identificado factores asociados a estos accidentes, lo que permite dirigir programas preventivos específicos sobre trabajadores con más riesgo. El modelo obtenido es válido para estimar el grado de accidentabilidad individual en los sujetos estudiados.

    BACKGROUND: Accidents resulting from percutaneous exposure account for approximately one third of all accidents suffered at work by health workers in hospitals. Their importance lies in the illnesses caused by pathogens that can be transmitted in this way (hepatitis B, hepatitis C, HIV virus). The aims are to describe accidents of this type notified in a tertiary level hospital, identify factors associated with these accidents in nursing staff and build a predictive model for the individual risk of having an accident. METHODS: A descriptive study of a retrospective cohort made up of all the people who notified having suffered an accident between 1-1-93 and 30-6-96. A study of cases and controls in nursing staff during the period 1-1-95 to 30-6-96, analysed through multiple logistical regression. RESULTS: The cumulative number of cases of accidents in one year was 0.078 for male and female nurses. In 57.3% of cases, disposable or pre-loaded syringes were involved. The cumulative number of cases in one year was greater for intravenous catheterisation (8.5% per 100,000). The risk of having an accident, adjusted on account of confusing variables, was greater for female and male nurses (OR=3.22; I.C.95%=1.96-5.27), for workers in the Haemodialysis Unit (OR=35.21; I.C.95%=3.74-331.16) and for those employed on a temporary contract (OR=4.50; I.C.95%=2.24-9.04). CONCLUSIONS: Accidents resulting from percutaneous exposure at this hospital are more frequent among nursing staff and are basically caused by some type of needle used for injections. Factors associated with these accidents were identified, allowing specific prevention programmes to be targeted at those workers at greater risk. The model obtained is valid to estimate the degree of individual accident probability for the subjects studied.
  • Brote nosocomial y comunitario de queratoconjuntivitis epidémica en Navarra Originales

    FUNDAMENTO: La queratoconjuntivitis epidémica (QCE) es una infección aguda, generalmente bilateral, que se presenta de forma esporádica o en brotes a partir de una fuente de infección común, que puede ser una consulta de oftalmología o una piscina. La queratitis puede evolucionar hacia la formación de lesiones subepiteliales cornéales que pueden persistir durante varios meses. El objetivo de este trabaji fue la descripción de un brote de queratoconjuntivitis epidémica ocurrido en Navarra (España) entre Abril y Agosto de 1996 que afectó al menos a 266 personas MÉTODOS: Descripción del brote, definición de caso clínico, investigación microbiológica, e investigación epidemiológica del brote mediante encuesta. RESULTADOS: En la investigación epidemiológica destaca la existencia de 2 consultas de oftalmología como mecanismo de transmisión de la enfermedad en 47 de los 116 pacientes investigados. La diseminación de la enfermedad en el entorno de los pacientes se pudo conocer en 119 casos, de éstos, 62 se consideraron casos secundarios. El diagnóstico etiológico del brote se realizó a los 11 días de iniciada la investigación microbiológica, aislándose adenovirus en muestras conjuntivales de 8 de los 19 pacientes investigados. CONCLUSIONES: Las medidas de prevención instauradas para el control del brote y la eficacia de las mismas se pone de manifiesto por la reducción paulatina de casos.

    BACKGROUND: Epidemic keratoconjunctivitis is an acute, generally bilateral infection, that appears sporadically or in outbreaks from a source of common infection which may be a visit to the ophthalmologist or a swimming pool. Keratis may evolve to form sub-epithelial injuries in the cornea lasting as much as several months. METHODS: Description of an outbreak of epidemic keratoconjunctivitis that took place in Navarre (Spain) between April and August 1996 and which affected at least 266 people. RESULTS: The epidemiological research highlights the existence of two ophthalmologist visits as the transmission mechanism for the infection in 47 of the 116 patients researched. The spread of the infection in the environment of the patients was recorded in 119 cases, 62 of which are considered secondary cases. The aetiological diagnosis of the outbreak was carried out 11 days after the start of the microbiological research, isolating adenovirus in conjunctival samples of 8 of the 19 patients researched. CONCLUSIONS: The preventive measures taken to control the outbreak and their effectiveness was highlighted by the gradual reduction in cases.
  • Valoración del cumplimiento de la quimioprofilaxis antituberculosa en la población reclusa del centro penitenciario de Jaén Originales

    FUNDAMENTO: El presente estudio resalta la importancia de la quimioprofilaxis antituberculosa con Isoniacida, para el control y prevención de la tuberculosis en el medio penitenciario. Se ha valorado la relación existente entre el cumplimiento de la quimioprofilaxis con factores de estudio; así como, hemos determinado si la educación sanitaria dispensada en nuestro centro, ha motivado a la población reclusa en el cumplimiento de este tratamiento. MÉTODOS: Se estudió el cumplimiento de la quimioprofilaxis con Isoniacida de 52 internos del Centro Penitenciario de Jaén, durante el periodo de febrero a mayo de 1995, calculando la dosis de Isoniacida tomada, mediante la determinación espectrofotométrica de metabolitos de Isoniacida en orina, por el método de Eidus Hamilton. Asímismo, se estudió la influencia sobre el cumplimiento de variables como sexo, infección por VIH, situación judicial y nivel cultural. RESULTADOS: De los 52 internos, 23 (44.23%) tomaban diariamente una dosis de Isoniacida considerada como efectiva; y 29 internos (55.77%) tomaban menos de la dosis de Isoniacida efectiva para la quimioprofilaxis. En cuanto a las variables estudiadas, los hombres (variable sexo) y aquellos internos con mayor grado educacional (nivel cultural) mostraron un mayor cumplimiento de la quimioprofilaxis, a pesar de no encontrar diferencias significativas. CONCLUSIONES: El cumplimiento de la quimioprofilaxis en nuestra población es deficiente, aunque se halla en consonancia con estudios similares realizados también sobre la población reclusa; no ocurre lo mismo con la población comunitaria donde encontramos un mayor cumplimiento de la quimioprofilaxis. ste bajo cumplimiento nos obliga a revisar el actual programa de educación sanitaria, así como aplicar medidas que puedan aumentar este grado de cumplimiento, como la observación directa del tratamiento, el régimen de tratamiento intermitentes, etc.

    BACKGROUND: This study highlights the importance of anti-tubercular chemoprophylaxis with isoniazid to control and prevent tuberculosis in prisons. The relationship between compliance with the chemoprophylaxis and study factors was evaluated and we examined whether health education given in our prison motivated the prisoners to comply with this treatment. METHODS: Compliance with chemoprophylaxis using isoniazid by 52 inmates of the Jaén Penal Institution was studied for the period February to May 1995. The dose of isoniazid taken was calculated by means of the spectrophotometric determination of isoniazid metabolites in urine using the Eidus Hamilton method. Likewise, the influence of variables such as sex, HIV infection, legal situation and cultural level on compliance was studied. RESULTS: Out of 52 inmates, 23 (44.23%) were taking a daily dose of isoniazid that was considered to be effective and 29 inmates (55.77%) were taking less than the effective isoniazid dose for the chemoprophylaxis. With regard to the variables studied, greater compliance with the chemoprophylaxis was recorded among men (sex variable) and those inmates with a higher educational level (cultural level) although no significant differences were found. CONCLUSIONS: Compliance with the chemoprophylaxis among our inmates is inadequate although it matches similar studies that have been carried out on the prison population. This is not the case for the community population where compliance with chemoprophylaxis is greater. This low compliance forces us to review our current health education programme and to take steps that might increase this degree of compliance such as direct observation of treatment, a system of intermittent treatment, etc.
  • Presencia de sulfitos en carne picada y preparados de carne elaborados en industrias de la Comunidad Valenciana Originales

    FUNDAMENTO: Desde este estudio y frente al desarrollo de disposiciones para la armonización de la legislación alimentaria sobre aditivos, se pretende conocer la utilización de sulfitos en carnes picadas y preparados de carne elaborados en establecimientos ubicados en la Comunidad Valenciana. MÉTODOS: Previa planificación de los tipos de productos y del número de muestras a investigar, se evalúan cualitativa y cuantitativamente los resultados obtenidos respecto a la presencia de sulfitos, expresados en mg/kg de SO2. RESULTADOS: Destaca la presencia de sulfitos en el 65,38% de muestras de hamburguesas de ternera/cerdo y en el 64,18% de hamburguesas de pollo. En carnes picadas, chorizo fresco y salchicha cruda se pone de manifiesto una mejor adaptación a la normativa. CONCLUSIONES: Se observa el amplio uso de sulfitos en los preparados de carne. La inminente aplicación de la normativa comunitaria va a suponer una modificación en las prácticas de elaboración de estos productos.

    BACKGROUND: In view of the development of harmonisation provisions for food legislation with regard to additives, the aim of this study is to find out the use of sulphites in minced meats and meat products prepared in establishments located in the Valencia Region. METHODS: Following planning of the types of products and number of samples to be researched, the results obtained were evaluated qualitatively and quantitatively for the presence of sulphites, expressed in mg/kg of SO2. RESULTS: The presence of sulphites was found in 65.38% of the samples of beef and pork burgers and in 64.18% of chicken burgers. Minced meat, fresh chorizo (highly-seasoned pork sausage) and raw sausages were more in line with legislation. CONCLUSIONS: The extensive use of sulphites in prepared meat products was observed. The imminent application of Community legislation will bring lead to a modification in the practices when preparing these products.
