    FUNDAMENTO: El tiempo de espera para acceder a la consulta está considerado un indicador de satisfacción. Las visitas no urgentes, realizadas sin cita previa, provocan un incremento del tiempo de espera de los pacientes citados, con la consiguiente insatisfacción de los mismos. El objetivo del estudio persigue la valoración cuantitativa de este tipo de visitas y conocer las características de las mismas, para introducir las medidas destinadas a su corrección y racionalización. MÉTODOS: Estudio descriptivo. Se realizó un registro que recogiera las características de los pacientes que originan las consultas sin cita previa, como la edad y el sexo y las relativas a la consulta: motivo, modalidad, jornada laboral y si era realizada de forma directa o indirecta. RESULTADOS: El 14,19% de consultas a demanda no urgentes son sin citación previa. Con el análisis factorial de correspondencias múltiple se identifican dos perfiles de consultas: las directas, solicitadas por pacientes jóvenes, por enfermedad o tareas burocráticas, al final de la consulta de la mañana, estando ellos presentes, y las consultas indirectas, ocasionadas durante las consultas con cita previa por pacientes que solicitan recetas. CONCLUSIONES: Hay un elevado número de visitas sin cita previa. Son precisas medidas que puedan conducir a la utilización del sistema de organización de las consultas y a la mejora de aquellas situaciones que impliquen una falta de accesibilidad, con la finalidad última de mejorar la satisfacción de los usuarios de los servicios sanitarios públicos.

    BACKGROUND: Waiting time to see a physician is considered to be an indicator of user satisfaction. Non-emergency visits made without an appointment ahead of time result in longer waiting times for the patients who have appointments, resulting in the consequent insatisfaction thereof. The purpose of this study is aimed at conducting a quantitative assessment of this type of visits and at ascertaining the characteristics thereof for the purpose of putting measures into practice for the correction and rationalization thereof. METHODS: Descriptive study. A record was made which would gather the characteristics of those patients seeing physicians without having made an appointment, such as age, gender and the characteristics related to the visit (reason, modality, working hours and whether made directly or indirectly). RESULTS: No appointment had been made ahead of time for 14.19% of all non-emergency patient-requested visits. Based on the factorial analysis of multiple correlations, two visit profiles are identified, that is, the direct visits requested by young patients due to illness or red tape at the end of the physician’s morning hours, said young patients being present, and the indirect visits during the visits for which appointments have been made ahead of time by patients requesting prescriptions. CONCLUSIONS: There are a large number of visits for which no appointment has been made ahead of time. Measures must be taken for the purpose of channeling the use of the appointment organization system and of improving those situations involving a lack of accessibility for the end purpose of improving the degree of satisfaction of those using the public health care services.
  • Calidad percibida por dos poblaciones adscritas a dos centros de salud de la provincia de Cuenca Originales

    FUNDAMENTO: Para prestar un servicio de calidad es fundamental conocer la opinión que sobre la misma tienen los usuarios del mismo. El objetivo del presente trabajo es medir y comparar la calidad percibida por dos poblaciones asignadas a dos centros de salud de la provincia de Cuenca. MÉTODO: Se trata de un estudio descriptivo transversal. Las Zonas Básicas de Salud estudiadas han sido el Centro de Salud de Cardenete y Centro de Salud de Motilla del Palancar, ambos del Área de Salud de Cuenca. Se ha hecho un muestreo aleatorio estratificado por cuotas de edad y sexo a partir de la tarjeta sanitaria individual. Se ha medido la calidad percibida con un cuestionario validado y fiable, utilizado anteriormente con fines similares por el Ministerio de Sanidad y Consumo. RESULTADOS: Se analizan un total de 295 encuestas, 147 a varones y 148 a mujeres. La media de edad es de 48,83 años (DE: 18,77) para el Centro de Salud de Motilla, en Centro de Salud de Cardenete es de 61,93 años (DE: 15,17). La media de frecuencias mensuales de visitas para Centro de Salud de MP ha sido de 1,56 (DE: 1,91); esta media para el Centro de Salud de Cardenete ha sido:2,49 (DE: 4,27). La media de frecuencias semanales para el Centro de Salud de MP ha sido: 0,32 (DE: 0,66); para el Centro de Salud de Cardenete ha sido: 0,49 (DE: 1,03). El tiempo de espera en el Centro de Salud de MP ha sido: 10,86 minutos (DE: 8,27); en el Centro de Salud de Cardenete: 7,88 (DE: 4,55). 83,4 % de los usuarios de los dos Centros de Salud de conocen la posibilidad de libre elección de médico de cabecera. Se han encontrado diferencias estadísticamente significativas en 11 de los 21 ítems estudiados sobre calidad percibida de que consta el cuestionario CONCLUSIÓN. Los resultados de los componentes de la atención según el análisis factorial de los ítems de calidad percibida son coincidentes con los resultados de calidad percibida. De los dos grupos de usuarios estudiados, los que perciben mayor calidad en el servicio sanitario que disfrutan son los del Centro de Salud de Cardenete.

    BACKGROUND: To render quality service, it is essential to know the opinion users have of the service in question. The purpose of this study is that of gauging and comparing the quality perceived by two populations assigned to two health care centers in the province of Cuenca. METHOD: This is a descriptive cross-sectional study. The Basic Health Care Districts studied were the Cardenete Health Care Center and the Motilla del Palancar Health Care Center, both within the Cuenca Health Care District. The source of information was personal health care card. A random sampling layered by age and gender percentages was taken. The perceived quality was gauged by means of a validated, reliable questionnaire used previously for similar purposes by the Ministry of Health and Consumer Affairs. RESULTS: A total of 295 surveys (147 males and 148 females) were analyzed. The average age was age 48.83 (SD: 18.77) for the Motilla Health Care Center and age 61.93 (SD: 15.17) for the Cardenete Health Care Center. The average monthly frequency of visits for the MP Health Care Center was 1.56 (SD: 1.91), this average having been 2.49 (SD: 4.27) for the Cardenete Health Care Center. The average weekly frequencies for the MP Health Care Center was 0.32 (SD: 0.66), having been 0.49 (SD: 1.03) for the Cardenete Health Care Center. The waiting time at the MP Health Care Center was 10.86 minutes (SD: 8.27), having been 7.88 (SD: 4.55) at the Cardenete Health Care Center. 83.4% of the users of the two Health Centers of are aware of the fact that they have the freedom to choose their primary care physician. CONCLUSION: Statistically significant differences were found in 11 of the 21 items studied regarding perceived quality of the which questionnaire is comprised. The results of the health care components according to the factorial analysis of the items of perceived quality are those coinciding with the perceived quality results. Of the two groups of users surveyed, those perceiving a higher degree of quality in the health care service they are rendered are those of the Cardenete Health Care Center.
  • Habitos higienicos en personas mayores de 65 años del area sanitaria de Guadalajara, sin deterioro cognitivo y residentes en la comunidad Originales

    FUNDAMENTOS: Se pretende conocer las características de los hábitos higiénicos en las personas mayores de 65 años que no presentan deterioro cognitivo, de la provincia de Guadalajara, residentes en la comunidad y a partir de la hipótesis que estos son deficientes y por ello mejorables. MÉTODOS: Se trata de un estudio observacional, analítico transversal, en el marco de la Atención Primaria, diferenciando ámbito urbano y rural. Se encuestó mediante cuestionario (con pilotaje previo y estudio de fiabilidad mediante análisis de estabilidad en las respuestas a partir de coeficiente kappa), a las personas de una muestra aleatoria estratificada, polietápica y proporcional según edad, sexo y núcleos de población. Analizamos las variables: A) Hábitos higiénicos: a.- Higiene personal-aseo diario: Procedimiento y periodicidad, Cuidados del cabello; Lavado de manos siempre antes de comer y después de ir al servicio; Afeitado en varones; Higiene bucodental, b.- Higiene del sueño, B) Variables sociodemográficas. Efectuamos análisis de datos mediante procedimientos propios de la estadística descriptiva, analítica y multivariante por regresión logística. RESULTADOS: La pérdida de sujetos de la muestra osciló entre el 16 y 26% (la negativa aparece como primera causa en el medio rural y la no localización en el medio urbano). La muestra finalmente estudiada incluyó a 388 personas en medio rural y 392 en el urbano. La mitad utilizan la ducha y lavan el cabello al menos una vez en semana. Hasta el 15% no se lavan las manos siempre antes de ir a comer y hasta el 44% no lo hacen después de ir al servicio. Mas del 50% de los varones se afeitan según el procedimiento correcto. Un alto porcentaje de edéntulos, tanto parciales como totales, no utilizan prótesis y en aquellos que sí lo hacen la higiene es defectuosa. CONCLUSIONES: Los hábitos higiénicos en las personas ancianas de nuestra provincia son manifiestamente mejorables en todos los aspectos analizados y de modo especial en el ámbito rural y en los varones. Esta debería ser una tarea prioritaria a cargo de cualquier persona de los EAP.

    BACKGROUND: The aim of this study is that of ascertaining the characteristics of the personal care habits of those individuals over age 65 who have no cognitive impairment within the province of Guadalajara who are residents of the community based on the hypotheses that the same are deficient and therefore subject to improvement. METHODS: This is an observational, analytical cross-sectional study within a Primary Care, distinguishing between the urban and rural environments. A questionnaire (involving prior trial and reliability study by means of an analysis of the stability of the answers based on a kappa factor) was given out to a random, layered ample divided into different stages and proportional by age, gender and centers of population. An analysis was conducted of the following variables: A) Personal care habits. A. Personal care- daily washing/dressing: procedure and frequency, Hair Care, Always washing hands before eating and after going to the bathroom; Shaving for men, Oral hygiene. b. Sleep routine. B) Sociodemographic variables. The data was analyzed using proprietary descriptive, analytical and multi-variable logistic regression procedures. RESULTS: The losses fell within the 16%-26% range (refusal was found to be the leading cause in the rural environmental and non-location in the urban environment). The sample finally studied included 388 individuals in the rural environment and 392 individuals in the urban environment. Half showered and washed their hair at least once a week. As high as 15% doe not wash always wash their hands before eating and as high as 44% do not do so after going to the bathroom. Over 50% of the males shave using the correct procedure. A high percentage of those missing part of their teeth as well as those completely toothless do not use dentures, and those who do are lacking in hygiene. Sleep routines shows better results. CONCLUSIONS: Personal care habits among senior citizens in our province are patently subject to improvement in all of the aspects analyzed, especially in the rural environment and among males. This should be a priority task under the charge of anyone assigned to the Primary Care Teams.
  • Evaluación de la eficacia de los cursos de formación sanitaria dirigidos a los manipuladores de alimentos de un área sanitaria Originales

    FUNDAMENTO: Las toxiinfecciones por consumo de alimentos contaminados plantean un importante problema de Salud Pública. La formación sanitaria es uno de los mecanismos disponibles para prevenir estas enfermedades. El objetivo de este estudio es evaluar si los manipuladores mejoran sus conocimientos asistiendo a los cursos que se imparten para la obtención del carnet de manipulador y analizar el efecto de las variables sociodemográficas sobre el nivel de conocimientos antes de realizar el curso. MÉTODO: Estudio de evaluación del tipo pre-post, sobre una muestra de 500 manipuladores que acudieron al Centro de Salud Pública de la ciudad de Gandía (Valencia) a realizar el curso de formación, entre octubre de 1997 y febrero de 1998. Se utilizó un cuestionario autoadministrado, antes y después del curso, que medía, entre otras variables, los conocimientos. RESULTADOS: Se observó diferencia significativa entre los resultados del pre-test y el post-test, excepto para la materia de higiene personal. Los manipuladores con más años de trabajo o con formación sanitaria previa contestaban correctamente con mayor frecuencia. CONCLUSIONES: Los cursos de formación aumentan el nivel de conocimientos sobre prácticas de manipulación de alimentos. Se debe poner más énfasis, durante la formación, en las materias de conservación, preparación y servicio de alimentos que en las de higiene personal o de instalaciones. No obstante sería interesante estudiar si hay cambios en las actitudes, como una primera aproximación para evaluar la efectividad de la formación y comprobar si hay intención de poner en práctica los conocimientos adquiridos.

    BACKGROUND: Food poisoning is a major Health Care issue. Health training is one of the mechanisms available for preventing these illnesses. The purpose of this study is that of ascertaining whether food handlers improve their knowledge regarding food handling practices by attending the courses which are offered for being awarded the handler card and of analyzing the impact of sociodemographic variables on the degree of knowledge prior to taking the course. METHOD: Pre-Post type assessment study on a sample of 500 handlers who came to the Public Health Center in the town of Gandía (Valencia) to take a training course in October 1997-February 1998. A self-test questionnaire taken prior to and following the course to gauge knowledge and other variables was used. RESULTS: A significant difference was found between the pre-test and the post-test, except with regard to the subject of personal hygiene. Those handlers having worked at this occupation for a longer number of years or who had prior health training answered correctly more often. CONCLUSIONS: The training courses heighten the degree of knowledge regarding food-handling practices. In training courses, greater emphasis must be placed on the subjects of food preservation, preparation and serving than on those of personal hygiene or facility cleanliness. Nonetheless, it would be of interest to research whether any changes in attitudes occur as an initial approach to assessing the effectiveness of the training and ascertaining whether any intention exists of putting the knowledge acquired into practice.
