    É descrita a evolução da mortalidade por doenças cerebrovasculares de adultos acima de 20 anos. residentes no Município de São Paulo, SP (Brasil), de 1950 a 1981, mediante a análise dos coeficientes de mortalidade por sexo e idade. Esta mortalidade é sempre maior em todas as idades para o sexo masculino e crescente com a idade. Entre 1970 e 1981, evidencia-se declínio de 17,3% para ambos os sexos (17,8% para homens e 16,8% para mulheres) que não foi atribuído a artefatos. Sugerem-se outros estudos para analisar as causas desse declínio.

    The evolution of mortality from cerebrovascular diseases in adults above the age of 20, in S.Paulo, SP, Brazil, from 1950 to 1981, is described, using the age and sex-specific death rates. Stroke mortality is always greater for males and increases with age. From 1970 to 1981 a decline in the mortality has been observed (17.3% for both sexes, 17.8% for males and 16.8% for females). This decline has not been attributed to artifacts. Other studies are suggested to analyse the causes of this decline.

    Estuda-se as características sócio-econômicas dos indivíduos acometidos pela malária, em 1983, na área urbana de Camaçari - Bahia, Brasil, com destaque para os subgrupos migrantes e não migrantes. As condições de vida precárias de ambos os subgrupos são apontadas como os determinantes do surto de malária desencadeado no local.

    It is proposed to study the socio-economic characteristics of individuals who were infected with malaria in the urban area of Camaçari, Bahia, Brazil, in 1983. Special emphasis being given to the subgroups of migrants and nonmigrants. The precarious living conditions of both subgroups are identified as determinative of the malaria outbreak which occurred there.
  • Levantamento planorbídico do Estado de São Paulo: sexta Região Administrativa Artigos Originais

    Com o objetivo de conhecer a distribuição dos planorbídeos na sexta Região Administrativa do Estado de São Paulo, Brasil, com sede na cidade de Ribeirão Preto, foram realizadas, durante sete meses, coletas de moluscos em ambientes hídricos de toda área. Foi verificado que os planorbídeos mais comuns no território, por ordem de freqüência são: Drepanotrema lucidum, Biomphalaria intermedia, Biomphalaria peregrina e Biomphalaria tenagophila. A última espécie ocorre em poucos criadouros mas vem sendo responsabilizada pela transmissão natural da esquistossomose em Bebedouro, desde 1976. B.straminea prolifera em um córrego no município de Serrana e, em uma vala, de Ribeirão Preto. Os dados epidemiológicos e o baixo número de hospedeiros intermediários encontrados levam a crer que a possibilidade da doença disseminar-se pela região é pequena.

    A systematic survey was carried out in the area of the 6th Administrative Region of the State of S.Paulo, in order to discover the distributional patterns of planorbidic snails. Biomphalaria tenagophila is incriminated as the intermediary of Schistosoma mansoni in Bebedouro county where some human cases of the disease have been observed since 1976. Biomphalaria straminea lives at two breeding places - in Serrana and Ribeirão Preto. It is concluded that there is little possibility of schistosomiasis spreading in the studied area.
  • Esquistossomose mansônica em camundongos experimentalmente subnutridos Artigos Originais

    Realizou-se estudo sobre o desenvolvimento da esquistossomose mansônica em camundongos submetidos à dieta hipoprotéica. Foram constituídos 4 grupos de Mus musculus "Swiss" da seguinte forma: 1) não infectados, normoprotéicos; 2) infectados, normoprotéicos; 3) não infectados, hipoprotéicos e 4) infectados, hipoprotéicos. Os animais foram sacrificados com 60 dias de infecção, aos 90 dias de idade. Verificou-se que os esquistossomos sofreram os efeitos da subnutrição do hospedeiro, principalmente os vermes machos, que além de terem seu desenvolvimento prejudicado, tiveram seu número reduzido aproximadamente pela metade. O número de granulomas foi menor nos roedores subnutridos e o tamanho da lesão foi reduzido. Houve acentuada leucopenia nos animais submetidos à dieta hipoprotéica, principalmente nos infectados subnutridos. A linfopenia e a eosinopenia acentuadas sugeriram que o sistema imunológico do hospedeiro foi afetado pela subnutrição. A taxa de mortalidade foi muito mais elevada nos animais infectados submetidos à dieta hipoprotéica. Concluiu-se que os camundongos subnutridos resistiram menos à infecção esquistossomótica apesar de terem apresentado menor número de lesões granulomatosas.

    Mansoni schistosomiasis was studied in mice fed on a low protein diet. Four groups of the Swiss breed Mus musculus were used in an experiment with two factors, each with two levels: 1-non-infected, normal diet; 2 - infected, normal diet ; 3 - non-infected, low protein diet; 4 - infected, low protein diet. The mice were killed for observation at age 90 days, after 60 days of infection, for those infected. It was found that the worms suffered the effects of malnutrition, mainly males, whose population count was cut by half, in addition to poor individual growth. The hepatic granuloma count was found to be smaller in the undernourished group; while the corresponding lesions were also generally smaller. There was a marked leukopenia in the malnourished mice, even more so in the infected mice. The severe lymphopenia and eosinopenia observed suggest that the immunologic system of the host was affected by malnutrition. Mortality rates were far larger in the infected, low protein diet group. One may then surmise that the malnourished mice offered lower resistance to the infection, their lower number of granuloma notwithstanding.

    Foi realizado estudo na área rural do Nordeste brasileiro com uma amostra de 689 famílias estratificadas segundo o acesso e a quantidade de terra possuída, em quatro categorias: famílias sem terra (ST), pequenos proprietários (PP) que possuíam até 10 hectares de terra, médios proprietários (MP) de 10 a 50 hectares e grandes proprietários (GP) com mais de 50 hectares de terra. A média de ingesta energética dos ST foi de 1.605 calorias, observando-se que à medida que aumentava a quantidade de terra possuída, maior era a média de ingestão calórica, chegando a atingir mais de 3.000 calorias entre os GP que corresponde a média dos valores encontrados nos países desenvolvidos. Em termos médios o déficit protéico não foi encontrado em nenhum dos grupos estudados. O valor calórico total (VCT) da dieta, para os quatro grupos, está representado, em média, por 13% de protídeos, 11% de lipídeos e 76% de glicídeos, observando-se tendência crescente à medida que aumenta a posse da terra com relação aos protídeos e lipídeos, e, decrescente, com relação aos glicídeos. O grupo de alimentos que mais contribuiu para o VCT foi o de "grãos e raízes", embora apresente tendência decrescente com o aumento do tamanho da propriedade. Comportamento inverso foi observado para o grupo dos "produtos animais". Não foram encontradas diferenças em relação a outros grupos de alimentos: verduras, frutas, açúcares e gorduras.

    A total of 689 families from rural areas of Northeast Brazil were studied; they were divided into four categories, according to land tenure: no land owner (n - LO), small land owner (s - LO) who had up 10 hectare of land, intermediate land owner (i - LO) 10 to 50 hectares and big land owner (b - LO) who had more than 50 ha of land. The mean energy intake of the n - LO was 1,605 calories, which represented 79,4 % of the requirement of this group. For the other groups (s - LO, i - LO and b - LO) this average increased in relation to the area of land owned. The energy intake of the b - LO groups is quite high (about 3,000 cal) corresponding to the average values of the developed countries. Or the average, protein deficiency was not detected among the groups. The total calory value (TCV) of the diet was as follows: 13% protein, 11% lipids an 76% carbohidrates. Protein and lipid intakes increase with the area of the land owned, but carbohidrates decrease. Roots and grains were the main contributors to the TCV but their contribution decreased as the area of the land owned increased. On the other hand, the importance of the animal products increased in proportion to land size. No differences were detected among the categories concerning two other groups of food stuffs: vegetables and fruit, sugar and oil.
  • Métodos de amostragem para estimação da cobertura vacinal Artigos Originais

    Apresentam-se sucintamente dois métodos de amostragem que visam a seleção de uma amostra de crianças de fixada faixa de idade, residentes em determinada área geográfica de interesse, para estimação de cobertura vacinal: o método de R.H. Henderson e T. Sundaresan, o método do Departamento de Epidemiologia e de Métodos Quantitativos em Saúde da Escola Nacional de Saúde Pública. Um terceiro método de amostragem é proposto. O primeiro método apresentado (Henderson e Sundaresan) é empregado no Programa Ampliado de Imunização e nesse programa é considerado eficiente, simples e não dispendioso. O segundo e o novo método, que apresentam modificações do primeiro, constituindo alternativas deste, visam diminuir o erro quadrático médio nas estimativas, conquanto sejam menos simples e mais dispendiosos que aquele. Visto que, na estimação da cobertura vacinal, o estimador empregado pressupõe amostra autoponderada, a preocupação maior do método aqui proposto foi a de proporcionar uma amostragem segundo a qual se tenha equiprobabilidade de seleção para qualquer criança do grupo etário estudado residente na área de interesse, independentemente de qualquer condição.

    It is presented two sampling methods for selecting a sample of children of a given age range, living in a geographical area of interest, to estimate vaccinal coverage: the R.H. Henderson and T. Sundaresan method, the Department of Epidemiology and Quantitative Methods of National Public Health School method. A third method of selection is then proposed. The geographical area of interest is divided into parts which are used as primary sampling units (PSU's). First method: 30 PSU's are selected with probability proportional to the population living in the PSU; a starting point ("household") is selected by random selection within each PSU selected: selection of 7 children from within each of the PSU's, begins with the starting household, and then continues to the next nearest household until a total of 7 individuals is obtained (all individuals of the appropriate age living in the last household falling within the sample are included). Second method: the first stage is like that of the first method; previously to the second stage there is a census in each PSU selected in order to prepare a list of all dwelling units where one or more children live; applying systematic selection, 7 dwelling units are selected from this list. Third method: 30 PSU's are selected with probability proportional to Di (number of dwelling units in the PSU i) given by the Census; the dwelling units in the selected PSU are considered in a fixed order; at the second stage, each selected PSU is exhaustively visited, according to the order of the dwelling units; during this visit, only the dwelling units where one or more children live are listed; to these dwellings a systematic selection is applied, the interval Di/b of which is such that with these two stages there is an equal probability selection method (epsem); the over-all sampling fraction is n/^C with n given by 210/a% (a%is the expected response rate) and ^C is the estimate of the number of children living in the area of interest during the survey. The probability of selection of each individual of the appropriate age, in the first method, depends on the ratio Pi/Di'(Pi is the population of the i th PSU; Di' is the number of dwelling units in this PSU, during the survey), it depends too on the number of dwelling units with no children living in them, situated between two dwellings with children, as well as on the number of children living in each dwelling unit. When the ratios Pi/Di' are equal for all the PSU's, and the second item has the same values for the whole area of interest and this happens also with the third item, we shall then have a selection method of equal probability (epsem) to the first. The second method is a probability one, since Pi/D'ci can be known for all the PSU's of the area of interest (D'ci is the number of dweling units with children living in, during the survey; this method will be an equal probability selection when that ratio is the same for all PSU's. The third method is an epsem, independently of the values of Di. The first method is used by the Expanded Programme on Immunization and is considered effective, simple and inexpensive. The second and third present some changes and consist of alternative methods which attempt to diminish the mean square error of the estimator, although they are less simple and more expensive than the first one. The third requires more data than the second. Since the estimator used to estimate the vaccinal coverage presupposes self-weighing samples, the principal purpose of the third method was to provid a method which will be epsem, i.e., will give to every child of the age group studied, living in the area of interest, equal probability of selection.
  • Mecanismos e significado epidemiológico da domiciliação Atualização

    Com freqüência tem-se observado populações animais sobreviverem sob alterações artificiais do ambiente. A preocupação fundamental do fato reside na seleção de populações com base numa evolução para a domiciliação ou na formação de biocenoses artificiais tendo como conseqüência a urbanização de doenças contendo focos naturais. Sob este ponto de vista, discute-se os fundamentos dos mecanismos de sinantropia, baseados em informações biogeográficas, evolução das espécies e cauística humana. A ênfase é dada às doenças metaxênicas retratando-se eventos passados e presentes. Neste particular, questiona-se as relações interespecíficas entre o homem e os insetos, através das modalidades de caráter associativo das espécies vetoras. O significado epidemiológico desta concepção está nos tipos de exposição do homem às doenças e identificação de atributos envolvidos nesses processos infecciosos. Ressalta-se também a domiciliação como fenômeno biológico juntamente com fatores intrínsecos das populações e extrínsecos do ambiente, nos quais incluem-se as influências antrópicas.

    Various animal populations have, frequently, been observed surviving under artificial alterations of environment. The main concern in this regard relates to the selection of populations based on the stage of their adaptation to the domiciliary environment or to the formation of artificial biocenoses that have natural foci, and which, consequently, mean that these diseases can become urbanized. The establishment of the synanthropy mechanisms based on biogeographical information, the evolution of the species and the human casuistic are here discussed from this viewpoint. Emphasis is given to the arthropod vector of the disease in present and past times. The interspecific relations between man and insects through the associative character of vetorial species are examined with regard to this point. The epidemiological significance of this concept lies in the types of human exposure to disease and the identification of the attributes involved in these infections. Finally, emphasis is given to domiciliation as a biological phenomenon together with intrinsic factors of populations and extrinsic factors of environment, these last including anthropic influences.
  • Desarrollo de la educación en salud: una actualización de conceptos

    Es definida la educación como una forma de promover el desarrollo del hombre como un individuo y como parte de un ambiente complejo, incluyendo los aspectos biológicos, psicológicos, sociales, económicos y físicos que componen el hilo de la existencia. La información científica y las técnicas por si solas no pueden cambiar los estilos de vida del hombre. La educación, como una tarea total, depende de la aquisición por las personas de nuevas maneras de relacionarse, de circunstancias que permitan decisiones libres y selección de alternativas en un contexto adecuado de información, habilidades cognitivas y suporte social. Cambios parciales de comportamientos, sin la adesión a una nueva forma de ser y a un nuevo proyecto de vida non son duraderos.

    In this article education is defined as a way to promote the development of man as an individual and as part of a complex environment, including the biological, psychological, social, economical and physical aspects that together make up the thread of existence. Scientific information and techniques alone do not change man's life styles. Education as a total task is only accomplished when people acquire new ways of relating to each other, and there is an area for free decisions and choices within a proper frame of valid information, adequate cognitive skills and valid social support. Partial changes in specific aspects of behaviour, without a commitment to a new way of being and a new project of life are not enduring.

    Certos problemas do uso do cinto de segurança ainda não foram resolvidos, sendo importante a determinação precisa das forças nos cabos. Existem vários métodos que, da maneira que são aplicados, são de pouca precisão. Com o objetivo de suspender essa imprecisão são apresentados três métodos e aperfeiçoado um outro de determinação da força exercida no cabo. Um desses métodos aplica-se a países subdesenvolvidos.

    Certain problems related to the use of safety belts remain unsolved, important among them being the exact determination of forces acting on cables. Various methods of calculation, severely limited because of the way in which they are applied, are presented. Three methods with a view to surpassing these limits are presented and another for the calculation of the forces acting on a cable is perfected. One of these methods is applicable in developing countries.
