Atualmente, existe preocupação notória dos pesquisadores em debater questões geradoras de conflitos éticos, no âmbito assistencial, direcionada ao ser humano em fase de terminalidade, em particular, acerca da eutanásia, distanásia e ortotanásia. Este estudo objetivou caracterizar a produção científica, no âmbito nacional, acerca da eutanásia, distanásia e ortotanásia. Trata-se de uma revisão integrativa da literatura. O universo do estudo foi constituído por 41 publicações relacionadas ao tema investigado, mediante a realização de um levantamento online, na Biblioteca Virtual em Saúde, no Portal Capes e na Revista Bioética. Desses, 25 artigos compuseram a amostra, considerando-se critérios de inclusão e exclusão estabelecidos. A coleta de dados ocorreu em março de 2013, por meio de um instrumento com informações pertinentes ao objetivo proposto. Os descritores utilizados foram eutanásia, distanásia e ortotanásia. No que se refere ao enfoque das publicações, emergiram três temas: Tema I - Eutanásia; Tema II - Distanásia e Tema III - Ortotanásia. Os estudos analisados expressam a preocupação atual diante de dilemas éticos, no que concerne ao cuidar do ser humano na fase final de vida. Dessa forma, espera-se que esta pesquisa possa contribuir para fortalecer as leituras críticas a respeito da temática.

There is currently widespread concern among researchers in debating questions that generate ethical conflicts within the scope of health care geared to the human being in the terminal phase, especially euthanasia, dysthanasia and orthothanasia. This study sought to characterize the scientific production at the national level on euthanasia, dysthanasia and orthothanasia. It involves an integrative review of the literature. The study universe consisted of 41 publications related to the theme in question by means of a survey conducted online in the Virtual Health Library in the Capes Portal and in the Bioethical Magazine. Of these, 25 articles comprised the sample taking into consideration the established inclusion and exclusion criteria. Data collection occurred in March 2013, by means of an instrument containing information pertinent to the proposed objective. The key words used were euthanasia, dysthanasia and orthothanasia. With respect to the focus of the publications, three themes emerged: Theme I - Euthanasia; Theme II - Dysthanasia and Theme III - Orthothanasia. The studies analyzed reflected the current concern in terms of ethical dilemmas concerning care of the human being in the end of life phase. Thus, it is hoped that this research can contribute to bolster the critical reading with respect to the theme.

Com objetivo de discutir e compreender as experiências de oncopediatras diante da tarefa de comunicar notícias difíceis (ND) a pacientes e familiares procedeu-se uma revisão bibliográfica a partir das bases de dados da Biblioteca Virtual em Saúde (BVS) que contém artigos divulgados por PubMed, Lilacs, Scielo, Scopus e SciVerse, buscando a produção sobre o tema. Os artigos disponíveis, avaliados pela análise temática, evidenciam as seguintes questões: carência de estudos focando a percepção dos profissionais sobre as notícias difíceis; a comunicação como estratégia pela qual, aspectos psicossociais, culturais, bioéticos, legais e emocionais são tratados; expectativa de habilidades médicas para as quais esses profissionais não foram treinados. Observou-se também que informações e treinamento dos profissionais sobre estratégias a cerca dos modos de lidar com a comunicação de ND são insuficientes para acompanhar o dinamismo das ocorrências e das relações pessoais na área de oncologia onde o paciente costuma estar ou sentir-se em constante ameaça de vida.

In order to discuss and understand the experiences of oncologists faced with the task of communicating difficult news to children and their close family members, a bibliographical review was conducted in the Virtual Health Library databases containing the literature published by PubMed, Lilacs, Scielo, Scopus and SciVerse with input on the theme. The articles available evaluated by thematic analysis reveal the following aspects: the lack of studies focusing on the perception of professionals about imparting difficult news; communication as a strategy by which psychosocial, cultural, bioethical, legal and emotional aspects are handled; that more is expected from physicians than they were trained for. It was also seen that the information and training of professionals in strategies regarding how to handle the communication of difficult news needs to be ongoing in order to keep up with the dynamism of the occurrences and the physician-patient relations in the oncological field in which patients tend to be or feel under constant life-threatening conditions.

Profissionais de enfermagem são os integrantes da equipe de saúde que mais mantêm contato direto e prolongado com pacientes que vivenciam a terminalidade. Este estudo de revisão integrativa da literatura teve por objetivo investigar a atitude frente à morte em profissionais e estudantes de enfermagem. Os dados foram coletados por meio de buscas nas bases Lilacs, MedLine, PsycINFO e CINAHL, utilizando os descritores "nurses" e "attitude to death", no período de 2000 a 2011. Das 1376 referências levantadas, 262 foram selecionadas para extração dos dados e 35 foram recuperadas na íntegra, constituindo o corpus da pesquisa. Os resultados evidenciaram predomínio de artigos publicados em periódicos brasileiros. Os estudos indicam que o assunto morte e morrer têm sido negligenciados pelas instituições de formação, o que gera sofrimento entre profissionais e estudantes quando enfrentam a questão na prática, além de condutas inapropriadas diante dos pacientes que vivenciam o processo de morte. Como conclusão ressalta-se a necessidade de futuras pesquisas, que possam fornecer esclarecimentos mais pormenorizados a respeito do tema e busquem estratégias para suprirem a falta de preparo e respaldo dos profissionais de enfermagem ao vivenciarem a morte e o morrer.

Nursing professionals are integral members of the healthcare team and they maintain the most direct and prolonged contact with patients experiencing a terminal illness. This integrative review of the literature sought to investigate the attitude towards death among nursing professionals and students. Data were collected through searches in Lilacs, Medline, PsycINFO and CINAHL databases using the key words "nurses" and "attitude to death" in the period from 2000 to 2011. Of the 1376 articles identified, 262 were selected for data extraction and 35 were downloaded in full, constituting the corpus of research. The results showed a predominance of articles published in Brazilian journals. Studies indicate that the subject of death and dying has been neglected in training institutions, which causes hardship among professionals and nursing students when faced with the issue in practice, in addition to inappropriate conduct when dealing with patients who are experiencing the end of life process. In conclusion, the need for future research that may provide more detailed clarifications on the subject and seek strategies to address the lack of preparation and support of the nursing staff when they cope with death and dying was emphasized.

O despreparo para lidar com a morte e com a ausência do ente querido pode implicar em reações orgânicas e psicológicas que, em decorrência da capacidade adaptativa do indivíduo ao período de luto, acaba por interferir na condição de alimentação e, consequentemente, no estado nutricional. Este artigo aborda os efeitos do luto recente no comportamento alimentar, seguindo-se da análise de alguns pontos de vista da entrevista dietética; da condição de nutrição e de alimentação pós-morte do ente querido; e das implicações desse processo na fome, na sede e na culinária familiar, com enfoque nas condutas nutricionais e nas decisões que os norteiam. Trata-se de um estudo de revisão de literatura na temática morte e luto, o qual objetiva contextualizar o tema em reflexões pautadas nessa vivência, enfatizando a interação da nutrição com a ciência da tanatologia, área ainda pouco explorada e carente de estudos. A identificação dessa influência e de suas implicações permite melhor planejamento das estratégias alimentares, contribuindo sobremaneira para ações de enfrentamento e de apoio ao luto.

The lack of preparation for dealing with death and the absence of the loved one may lead to organic and psychological reactions that, due to the adaptive capacity of the individual to the period of mourning, may result in interference in feeding habits and consequently on the person's nutritional status. This article addresses the effects of recent mourning on feeding behavior, followed by the analysis of the dietary interview from various standpoints. This includes the postmortem nutrition and feeding habits of the bereaved and the implications of this process on hunger, on thirst and on family cooking, with a focus on nutritional behavior and on the decisions that surround it. This is a review of the literature on the theme of death and mourning, which seeks to contextualize this theme around reflections based on this experience. It emphasizes the interaction of nutrition with the science of thanatology, which is an area still not properly examined and lacking study. The identification of this influence and its implications enables better planning of food strategies, contributing greatly to actions for coping and support during mourning.
